1.58 INCH CRACK IN LOWER RADIUS OF AFT SPAR GUSSET AT STA 00.000 OF LEFT HORIZONTAL STABILIZER.
2 INCH CRACK THROUGH 3 RIVETS IN LOWER AFT FLANGE OF FORWARD INBOARD RIB OF LEFT HORIZONTAL STABILIZER AT STA 00.000.
CABIN DOOR POPPED OPEN SHORTLY AFTER TAKEOFF OUT OF VCV HEADING TO AIRPORT. PILOT DEVERTED TO THE NEAREST AIRPORT, ACFT LANDED, PILOT SECURED DOOR AND CONTINUED WITH FLIGHT.
ACFT ENROUTE, HAD TO LAND AT BECAUSE CABIN DOOR POPPED OPEN SECURED DOOR AND CONTINUED TO DEPARTURE. (K)
ACFT ENROUTE, WHEN THE CABIN DOOR POPPED OPEN, PILOT THEN LANDED. SECURED DOOR AND CONTINUED. (K)
ON TAKEOFF, PILOT SELECTED GEAR UP AND NOTHING HAPPENED, THEN RETURNED TO DEPARTURE, NO PROBLEMS. MAINTENANCE FOUND LT MAIN GEAR SQUAT SWITCH PACKED WITH SNOW, THAWED OUT SWITCH OPS CHECKS GOOD.
GOING INTO (DET) HAD NO NOSE GEAR DOWN AND LOCK LIGHT, LANDED OK. FOUND NOSE GEAR DOWN AND LOCK SWITCH BAD. REMOVED AND REPLACED SWITCH OPS CHECKS GOOD.
RT ENGINE STAYED AT 2100 RPM ON APPROACH, SHUT ENGINE DOWN. INSTALLED NEW CONTROL LEVER ARM TO SHAFT THROTTLE ASSEMBLY OPS CHECKS GOOD.
GOING INTO MDW, NOSE GEAR NOT SHOWING DOWN AND LOCK. CYCLED GEAR, STILL NO DOWN AND LOCK INDICATION. AIRCRAFT LANDED, NOSE GEAR COLLAPSED. FOUND ROD END ON RETRACT ROD SPLIT OPEN AT GREASE FITTING SEPARATING ROD END FROM NOSE GEAR.
DURING APPROACH, THE NOSE RETRACT LANDING GEAR RT ROD ASSY ROD END PULLED APART FROM THE GEAR BRACKET. THE NOSE GEAR COLLAPSED. THE CAUSE OF THE ROD END FAILURE IS UNKNOWN AT THIS TIME.
