Maintenance disclosures
ON 6/28/2022 AT 0610Z, N209TR DEPARTED KTOL FOR KCHA. WHILE CLIMBING THROUGH FL220, "A" SYSTEM HYD LOW LEVEL LIGHT ILLUMINATED AND REMAINED ON. ALSO, THE "A" SYSTEM QTY INDICATOR DROPPED TO 2.2 GALLONS AND STOPPED AT THAT VALUE. CREW PERFORMED MEMORY ITEMS FOR HYD SYSTEM "A" FAILURE/LEAK AND COMPLETED CHECKLIST ITEMS. PRESSURE DID NOT DROP BELOW 2,800 PSI. "A" SYSTEM HYD WAS TURNED BACK ON AS TO EXTEND LANDING GEAR AND FLAPS. AT THAT TIME SYSTEM QUANTITY THEN INDICATED NORMAL. THE FLIGHT CREW ELECTED TO AIR TURNBACK TO KTOL AND LANDED SAFELY AT 0705Z. WHEN MX GOT TO THE AIRCRAFT AT 1100Z, WE NOTED NO HYD LEAKS OF ANY KIND AND THE "A" SYSTEM HYD RESERVOIR WAS AT THE "FULL" MARK BOTH ON THE COCKPIT GAUGE AND ON THE DIRECT READING "BOBBER" GAUGE MOUNTED ON THE SIDE OF THE RESERVOIR. THE ENGINES WERE RAN AND CHECKED FOR HYD LEAKS WITH NO LEAKS NOTED. WE SUSPECT A FAILING QUANTITY TRANSMITTER IS AT FAULT AND THAT IS BEING REPLACED NOW.
ON 3/2/2019, DEPARTED, APROX 3 MINUTES AFTER TO, THE CREW GOT A NR 1 ENGINE FIRE INDICATION IN THE COCKPIT. THE CREW REDUCED THE POWER SETTING TO IDLE AND THE FIRE INDICATION WENT AWAY. THEY RETURNED TO KMFE SAFELY. AFTER TROUBLESHOOTING BY IJW MECHANICS, IT WAS DETERMINED THAT NO FIRE HAD OCCURRED BUT THAT A BLEED AIR LEAK HAD BLOWN AIR ONTO THE FIRE LOOP AND CAUSED THE FIRE WARNING TO GO OFF.
AFTER TAKEOFF AND DURING CLIMBOUT FROM KMFE, THE FLIGHT CREW EXPERIENCED A NR 1 ENGINE FIRE WARNING. THEY PULLED THE THROTTLE BACK TO IDLE AND THE WARNING WENT OUT. THEY DUMPED APPROX 5,000 OF FUEL AND LANDED SAFELY BACK AT KMFE. TROUBLESHOOTING ON 3/3/2019 REVEALED THAT A BLEED AIR LEAK CAUSED THIS FIRE WARNING AND NOT AN ACTUAL FIRE EVENT.
DURING CLIMB OUT OF KELP, #1 ENGINE FIRE WARNING WENT OFF WITH A SPIKE IN OIL TEMP AND PRESSURE. THE A/C DID AN AIR TURN-BACK AND LANDED SAFELY BACK IN KELP. TO DATE, THERE'S NO CLEAR FAULT FOR WHAT CAUSED THE PROBLEM AND THE AIRCRAFT REMAINS IN KELP.
DURING A ROUTINE SERVICE CHECK, IJW MX FOUND FOD DAMAGE TO THE #1 ENGINE FAN.
TRAILING EDGE FLAP SKIN GOUGED WBL 95 RIGHT SIDE.
CORROSION ON FUSELAGE SKIN AT BS 328 STRINGER 19R JUST AFT OF FWD LAV SERVICE PANEL.
LOWER RUDDER LT SIDE HAS A SPOT OF CORROSION 2.5" IN LENGHT.
RT WING INBD FOREFLAP INBD END GOUGED. REPAIRED IAW FAS NR 85781 W/O N209TR-02-01.
RIGHT WING T/E O/B AILERON UPPER SURFACE O/B SIDE T/E DELAMINATED. REPAIRED IAW FAS NR 85831 W/O N209TR-02-01.
RIGHT WING O/B FORE FLAP O/B END HAS SEVERAL GOUGES ON LEADING EDGE.
PERFORMED DVI INSPECTION OF LT AND RT ELEVATOR REAR SPARS AND FOUND TWO STIFFENERS CRACKED AT ELEVATOR STATION 63.3
RIGHT WING FIXED T/E STA WBL 92.8 STIFFENER LOWER END IS COORODED.
NR 1 ATC ANTENNA MOUNT ON THE LOWER FUSELAGE SKIN HAS CORROSION ALONG THE MATING SURFACE OF SKIN.
RIGHT WING T/E INBD AILERON OTBD SIDE WEB IS CRACKED.
LEFT WING NR 4 SLAT AFT SIDE, LIGHTING HOLE HAS A CRACK ALONG THE BOTTOM EDGE, CRACK LENGTH IS 1/8".
DURING A RECENT FLIGHT, AT A POINT APPROXIMATELY 100 NM SW OF DESTINATION AND AT AN ALTITUDE OF 28,000 FT, THE NR 2 ENGINE OIL FILTER BYPASS LIGHT ILLUMINATED. THE CREW REDUCED THRUST TO ABOUT 70 PERCENT N1 AND THE LIGHT BENT OUT. THE NR 2 OIL FILTER BYPASS LIGHT THEN CAME BACK ON. THE THROTTLE WAS THEN PLACED TO IDLE AND ABNORMAL SHUTDOWN PROCEDURES WERE FOLLOWED. AN EMERGENCY WAS DECLARED AND THE ACFT LANDED SAFELY WITHOUT INCIDENT. UPON INSPECTION OF THE NR 2 ENGINE, IJW MAINTENANCE FOUND THE THERMAL ANTI-ICE TUBE BROKEN AND SOME CARBON BUILD-UP IN THE MAIN OIL FILTER. BOTH THE ANTI-ICE TUBE AND OIL FILTER WERE CHANGED ALONG WITH THE OIL FILTER BYPASS SWITCH. THE ENGINE WAS RAN TO ENSURE THAT NO FAULTS WERE STILL PRESENT IN THE OIL FILTER BYPASS WARNING SYSTEM, NONE WERE NOTED AND THE ACFT WAS RETURNED BACK TO SERVICE.
SHORTLY AFTER TAKEOFF, ACFT EXPERIENCED A LOSS OF CAPTAIN'S AIRSPEED INDICATOR, ALTIMETER, IVSI, TAWS, NR 1 TRANSPONDER FOLLOWED BY A CARGO SMOKE DETECTOR ILLUMINATION, FOLLOWED BY RT AFT NR 2 FUEL BOOST PUMP CIRCUIT BREAKER POPPED FOLLOWED BY BOTH CAPTAIN'S AND F/O'S ATTITUDE INDICATORS IN DISAGREEMENT WITH THE EMERGENCY HORIZON WITH GYRO FLAGS DISPLAYED ON BOTH CAPTAIN'S AND FIRST OFFICER'S ATTITUDE GYROS. THE FLIGHT WAS TERMINATED BY NOT CONTINUING ON TO THE PLANNED DESTINATION, AND INSTEAD DECLARING AN EMERGENCY AND LANDING AT THE CLOSEST AIRPORT THAT COULD ACCOMMODATE THE ACFT. SECTION 5 AND 6 BELOW WILL NOT BE COMPLETED AS THE ROOT CAUSE OF THE DEFECT HAS YET TO BE DETERMINED. MAINTENANCE HAS NOT YET TAKEN PLACE ON THE ACFT.
