DURING DISASSEMBLY FOR SCHEDULED OVERHAUL A CRACK WAS IDENTIFIED AT A SINGLE BLADE ROOT. MULTIPLE BEARING RACES WERE FOUND TO BE CORRODED AND FAILED.
PILOT REPORTED THE AUXILIARY CABIN FAN SWITCH FELT HOT WITH A BURNING SMELL AND NOTED SPARKS. COMPANY MAINTENANCE INSPECTED AND FOUND A BURNT WIRE TERMINAL. THE TERMINAL WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING MAINTENANCE A CRACK WAS DISCOVERED ON THE RIGHT HAND CLOSEOUT RIB (P/N: 5122115-56) AT R.W.S. 106.79. THIS CRACK ORIGINATES FROM A HOLE IN THE FORWARD ROW OF FASTENERS COMMON TO THE AFT MAIN LANDING GEAR TRUNNION SUPPORT PLATE AND TRAVELS FORWARD AN APPROXIMATE LENGTH OF .625 INCHES. THE CRACK IS CONSISTENT WITH OTHERS VISUALLY IDENTIFIED IN THIS SAME AREA. THIS RIB WILL BE REPLACED.
DURING MAINTENANCE A CRACK WAS VISUALLY IDENTIFIED ON THE RH UPPER FWD SPAR CAP PN: 5122031-6. THIS CRACK IS LOCATED NEAR THE FORWARD RADIUS OF THE SPAR CAP AT APPROXIMATE RWS 70.94. THIS CRACK WAS CONFIRMED BY SURFACE EDDY CURRENT, MEASURES APPROXIMATELY 1 INCH LONG, AND IS CONSISTENT WITH OTHER CRACKS VISUALLY IDENTIFIED IN THIS AREA. THE CAP WILL BE REMOVED AND REPLACED USING NEW PARTS.
DURING MAINTENANCE ON N223PB, A MULTIPLE CRACKS WERE VISUALLY IDENTIFIED ON THE RH NACELLE CROSSOVER BEAM (P/N: 5654104-2). ONE CRACK ON THE FRONT FACE OF THE BEAM AT APPROXIMATE RWS 87.44 WITH AN APPROXIMATE LENGTH OF 3.780 INCHES. A SECOND CRACK WAS IDENTIFIED ON THE LOWER CHANNEL PERPENDICULAR TO THE LENGTH OF THE CHANNEL ALSO AT THE SAME WING STATION, EXTENDING FROM THE FRONT FACE ACROSS THE TOP OF THE CHANNEL AND DOWN THE REAR FACE. A THIRD CRACK WAS VISUALLY IDENTIFIED ON THE AFT FACE OF THE BEAM AT THE SAME WING STATION STARTING FROM THE BOTTOM EDGE TRAVELING UP APPROXIMATELY 1.704 INCHES BEFORE TRAVELING UP 2.262 INCHES FURTHER AT AN APPROXIMATE ANGLE OF 30 DEGREES. THIS NACELLE HAD SPECIAL INSPECTION 402-4 COMPLIED WITH 238.5 HOURS AGO, AND SPECIAL INSPECTION 402-1 COMPLIED WITH 60.7 HOURS AGO. THIS BEAM WILL BE REPLACED USING NEW PARTS.
WHILE PERFORMING THE LIGHTED BOROSCOPE INSPECTION PROCEDURE AS OUTLINED IN TEXTRON MEL-54-02R2 TO SATISFY AD2017-10-19, A CRACK WAS IDENTIFIED ON THE OUTBOARD RADIUS OR THE LEFT HAND OUTBOARD NACELLE FITTING. THIS CRACK MEASURES APPROXIMATELY .119 INCHES LONG AND IT'S LOCATION IS CONSISTENT WITH OTHERS FOUND USING THIS SAME METHOD. THIS FITTING HAS BEEN REMOVED AND WILL BE REPLACED USING NEW SUPERSEDED P/N: 5292029-31.
THE PILOT REPORTED A BURNING SMELL DURING CLIMB OUT. MAINTENANCE INSPECTED AND FOUND THE HEATER BLOWER INOPERATIVE. THIS WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
CORROSION WAS DISCOVERED ON ONE OF THE LEFT SIDE VERTICAL STABILIZER AFT ANGLES. REMOVED AND REPLACED THE ANGLE.
PILOT REPORTED RT MLG DOWN AND LOCKED LIGHT INOPERATIVE AFTER EXTENSION, EMERGENCY T-HANDLE PULLED. MAINTENANCE INSPECTED AND REPLACED THE RT MLG ACTUATOR ASSEMBLY IAW 32-30-02, PERFORMED SEVERAL GEAR SWINGS AND OPERATIONALLY CHECKED GOOD. SERVICED THE EMERGENCY BLOW DOWN BOTTLE.
DURING SCHEDULED INSPECTION, AREAS OF EXFOLIATING CORROSION WERE FOUND AT LT WING LOWER FWD SPAR CAP STRAP, OEM PN 5292029-1, PREVIOUSLY INSTALLED VIA SERVICE KIT SK402-47 ON MARCH 10, 2004 TO SATISFY REQUIREMENT OF AD2005-12-13 CONCURRENT WITH MEB05-2 AS TERMINATING ACTION FOR AD2005-05-52. THE AFFECTED PARTS WILL BE REPLACED WITH FACTORY NEW FOLLOWING A DETAILED INSPECTION OF SURROUNDING STRUCTURE UPON DISASSEMBLY.
PILOT REPORTED LT BRAKE FAILURE. ACFT IS CURRENTLY IN HEAVY MX, CORRECTIVE ACTION NOT AVAILABLE AT THIS TIME.
PILOT REPORTED A FUEL ODOR IN THE COCKPIT. PERFORMED A FUEL PRESSURE TEST & NO DEFECTS WERE NOTED, NO ACTION TAKEN.
PILOT REPORTED THAT DURING PREFLIGHT INSPECTION, THERE WAS A STRONG ODOR OF FUEL IN THE COCKPIT. MX INSPECTED THE ENTIRE FUEL SYS IN ACFT AND IN THE HEATER. FOUND A LOOSE B-NUT IN LT WHEEL WELL WITH EVIDENT STAINS. B-NUT WAS TIGHTENED AND NO OTHER LEAKS WERE NOTED.
THE PILOT REPORTED THAT DURING FLIGHT, THERE WAS A FUEL ODOR PRESENT IN THE CABIN. ON LANDING AND INSPECTION, A B-NUT ON THE RT CROSSFEED LINE IN THE FUEL BOWL BAY WAS TIGHTENED. THIS RESOLVED THE ISSUE WITHOUT FURTHER DISCREPANCY.
DURING CLIMB, PILOT EXPERIENCED A MOMENTARY BURNING ODOR IN COCKPIT DURING GEAR RETRACTION. AFTER INSPECTION, THE DISCREPANCY WAS RESOLVED BY REPAIRING AND REROUTING WIRE BUNDLE IN NOSE GEAR WHEEL WELL.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, OEM PN 5142002-5, WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
IN CRUISE FLIGHT, CREW REPORTED, WITH LANDING GEAR UP AND LOCKED, AND ALL GEAR INDICATIONS NORMAL, NUMEROUS CAUTION/WARNING LIGHT FLICKERED AND THEN REMAINED ILLUMINATED. GEAR WARNING HORN ALSO SOUNDED FOR A SECOND. ANNUCIATOR LIGHTS THAT ILLUMINATED WERE, LOW VOLTAGE, LT AND RT ALTERNATOR INOP LIGHTS, HYD PRESSURE LIGHT, LT AND RT HYD FLOW LIGHTS, TURN AND BANK TEST, ALL 3 DOWN AND LOCK LIGHTS AND GEAR UNLOCK (IN TRANSIT) LIGHT. NO EVIDENCE OF ACTUAL GEAR EXTENSION WAS NOTED. AFTER COMPLETEING GEAR WILL NOT EXTEND HYD CHECKLIST AND PERFORMING EMERGENCY GEAR EXTENSION, GEAR WARNING HORN STILL SOUNDED WITH THROTTLES RETARDED OR WITH FLAPS BELOW 15 DEGREES. EMERGENCY WAS DECLARED, BUT LANDING WAS UNEVENTFUL. MX INSPECTED ACFT, CLEANED AND RESECURED CONNECTIONS TO ANNUCIATOR DIODE BOARD ASSY. ALL SYS CHECKED NORMAL.
CREW REPORTED, AFTER TAKEOFF, HYD PRESSURE LIGHT AND GEAR UNLOCK LIGHT REMAINED ILLUMINATED. SECOND ATTEMPT RESULTED IN SAME INDICATIONS. CREW SELECTED GEAR DOWN WITH NORMAL INDICATIONS (3 GREEN DOWN AND LOCKED LIGHTS). AFTER LOW APPROACH, TOWER REPORTED GEAR SEEMED TO BE DOWN. TO INSURE THAT GEAR WAS DOWN AND LOCKED, CREW ELECTED TO USE EMERGENCY EXTENSION PROCEDURES. EMERGENCY WAS DECLARED, LANDING WAS UNEVENTFUL. MX INSPECTED ACFT, FOUND NOSE GEAR STRUT UNDER INFLATED. SERVICED NOSE STRUT, PERFORMED SEVERAL EXTENSION AND RETRACTION TEST, SYS CHECKED NORMAL. ACFT RELEASED BACK TO SERVICE.
CREW REPORTED AFTER TAKE OFF AND GEAR RETRACTION,(DOWN LOCK LIGHTS EXTINGUISHED) THE HYD PRESSURE LIGHT AND GEAR UNLOCK LIGHT REMAINED ILLUMINATED. RECYCLED GEAR, SAME INDICATION. CREW ELECTED TO EXTEND GEAR, NORMAL EXTENSION (3 GREEN ILLUMINATED AND HYD PRESSURE LIGHT EXTINGUISHED). RETURNED TO BLOCK, LANDING UNEVENTFUL. MX PERFORMED SEVERAL EXTENSION AND RETRACTION TEST, COULD NOT DUPLICATE LANDING GEAR MALFUNCTION. AS A PRECAUTIONARY MEASURE, CLEANED AND LUBED ALL UPLOCK SWITCHES.
ON 08-05-07, LEFT ALTERNATOR OUT LIGHT CAME ON IN FLIGHT AND RT ALT OUT CAME ON INTERMITTENTLY. MX INSPECTED AND REPAIRED WIRING TO LT ALT AND SECURED WIRE TO RT ALT FUSE IN COCKPIT, SECONDARY INSP AND RUN UP WAS PERFORMED ON 08-06-07 WITH NO DEFECTS NOTED.
CREW REPORTED AVIONICS MASTER SWITCH INOP. MX CHANGED CIRCUIT BREAKER.
