Maintenance disclosures
DURING LANDING, THE NOSE LIGHT DOWN AND LOCKED SAFE INDICATION FAILED TO ILLUMINATE. THE FLIGHT CREW FOLLOWED EMERGENCY EXTENSION PROCEDURES AND LANDED THE AIRCRAFT SAFELY. MAINTENANCE FOUND A PIN ON THE LANDING GEAR EXTENSION HANDLE ASSEMBLY RECESSED AS WAS ABLE TO RESET THE PIN INTO THE CORRECT POSITION. EXTENSION AND RETRACTION CHECKS ON AIRCRAFT JACKS WERE ALL SUCCESSFUL AFTER THE REPAIR WAS MADE AND THE AIRCRAFT HAS BEEN RETURNED TO SERVICE.
DURING APPROACH TO KIST, THE FLIGHT CREW NOTED A STRONG SMELL AND A SMALL CLOUD OF WHITE SMOKE STARTED COMING OUT FROM THE INSTRUMENT PANEL MOMENTARY AND THEN DISAPPEARED. THE PILOT GOT DISTRACTED AND DECIDED EXECUTING A GO AROUND. WHEN THE SMELL STARTED GETTING WORSE, THE PILOT NOTIFIED THE ATC REGARDING A SITUATION AND REQUESTED TO LAND. THE ATC ESCALATED TO AN EMERGENCY CATEGORY. THE AIRCRAFT LANDED IN KIST WITHOUT INCIDENT. SJU MAINTENANCE TRAVEL TO KIST, INSPECTED AND FOUND COPILOT ATTITUDE GYRO EXTERNAL LIGHTING SHORTED. REPOSITIONED AND RESECURED WIRE AS REQUIRED. RAN BOTH ENGINES PERFORMED OPERATIONAL CHECKS ON ALL ELECTRICAL SYSTEM, NO SMOKE OR SMOKE SMELL NOTED.
AFTER LANDING PILOT NOTICED A LOST IN BRAKING ACTION & DIRECTIONAL CONTROL, PILOT DECIDED TO STOP ACFT ON TAXI WAY & HAVE ACFT TOWED BACK. TROUBLESHOOTING REVEALED LEAKING O RING ON NR 1 BRAKE & FOUND NR 3 BRAKE AT WEAR LIMIT. R & R NR 1 & NR 3 BRAKE ASSEMBLIES, BLED BRAKES SYS, & PERFORMED OPERATIONAL CHECKS OF BRAKE SYS. OPERATIONAL CHECKS WERE NORMAL.
