AFTER TAKEOFF, FIRST OFFICER AND CAPTAIN SUSPECTED SMELL OF OIL. SMELL WAS ONLY SLIGHTLY EVIDENT AS ALT INCREASED TO FL380. STRONGER SMELL BY 15,000 THROUGH 10,000. ODOR WAS MOST EVIDENT AT 4,000. FLIGHT CONTINUED UNAFFECTED WITH NO VISUAL OR AURAL INDICATIONS. AIRCRAFT LANDED WITHOUT INCIDENT OR PRECAUTIONARY ACTIONS. POST FLIGHT, NO SMELL ON GROUND. CONDITIONAL INSPECTION FOR SMOKE OR FUMES IN THE CABIN IAW B767 AMM -05-51-56 AND AMM TASK 21-00-21-102-030 SATISFACTORY. NO FAULTS FOUND. APU DEFERRAL MAINTENANCE ITEM 38063 REMAINS ACTIVE. AIRCRAFT RELEASED TO SERVICE NON-ETOPS.
MAIN CARGO DOOR LIGHT IN FLIGHT AT FL 19. ABNORMAL CABIN RATE CHANGE BETWEEN FL18.0 AND FL20.0, ALTERNATIVELY, NO LOSS OF CABIN PRESSURIZATION. FAULT NOT CREW DEFERRABLE. AIR TURN BACK TO DEPARTURE STATION WITH EMERGENCY DECLARATION. LANDING WAS UNEVENTFUL WITH NO ADDITIONAL PRECAUTIONARY ACTIONS. ON ARRIVAL, AIRCRAFT GROUNDED FOR UNSCHEDULED MAINTENANCE. POST FLIGHT, LINE MAINTENANCE VERIFIED THE INTEGRITY AND SECURITY OF MAIN CARGO DOOR. PRESSURIZATION CHECK IAW B767 AMM 21-31-00 GOOD. NO FAULTS FOUND. MAIN CARGO DOOR INDICATIONS SYSTEMS DEFERRED-38114 MEL CAT C. CONDITIONAL INSPECTIONS FOR VERIFICATION OF MAIN CARGO DOOR SECURITY AND INTEGRITY REQUIRED PRIOR TO DEPARTURE. DEFERRAL MAINTENANCE ITEM 38114 WORK IN PROGRESS. AIRCRAFT RELEASED TO SERVICE.

WHILE TROUBLESHOOTING APU DEFERRAL MAINTENANCE ITEM-38063 LINE MAINTENANCE REPORTED, WITH RT ENG START IAW AMM 71-00-00 & PACK RUNNING, SIGNS OF SMOKE IN COCKPIT. NO DAMAGE TO LT & RT ENGINES NOTED. EVIDENCE OF SMOKE APPEARED AFTER ENG SHUTDOWN. SMOKE ISOLATED TO BLEED AIR FROM APU. GENERAL VISUAL INSPECTION IDENTIFIED APU CASE DIFFUSER DAMAGE. CONDITIONAL INSPECTION-SMOKE OR FUMES IN COCKPIT SATISFACTORY. NO ADDITIONAL FAULTS FOUND . AIRCRAFT RELEASED TO SERVICE. WORK IN PROGRESS.
