DURING FLIGHT, THE PILOT NOTED A SUDDEN VIBRATION IN THE ANTI-TORQUE CONTROLS AND PRECAUTIONARILY LANDED THE HELICOPTER. UPON INSPECTION, ONE TIP WEIGHT WAS FOUND MISSING FROM TAIL ROTOR BLADE PN: 2062200-301.
DURING FLIGHT, THE PILOT NOTED A SUDDEN VIBRATION IN THE ANTI-TORQUE CONTROLS AND PRECAUTIONARILY LANDED THE HELICOPTER. UPON INSPECTION, ONE TIP WEIGHT WAS FOUND MISSING FROM TAIL ROTOR BLADE PN: 2062200-301.
RT ELEVATOR HAS CORROSION ON INBOARD LEADING EDGE. REMOVED AND REPLACED AFFECTED RT ELEVATOR WITH NEW PART, P/N: 206-023- 119-179, IAW BHT-206L1-MM-1, CH. 27.
WHILE PERFORMING PREFLIGHT OF THE AIRCRAFT FOUND ONE CYCLIC TUBE TO HAVE THE FIXED AND NONADJUSTABLE CLEVIS ON THE SERVO END OF THE CONTROL TUBE TO BE ROTATING WITHIN THE TUBE.
WHILE IN FLIGHT, PILOT AND CREW REPORTED A STRONG FUEL ODOR. THEY DECIDED TO TERMINATE THE FLIGHT AND RETURN TO BASE. UPON INVESTIGATION THE AFT FUEL PROBE WAS REPOSITIONED IN ITS OPENING. FURTHER OPS CHECKS DETERMINED FUEL ODOR TO BE GONE.
CRACK IN NECK OF PART AT RELIEF HOLE. CRACK IS 0.9" LONG. PART CONNECTS PILOT'S YOKE TO ACFT FLOOR.
