IMMEDIATELY AFTER TAKEOFF WE HAD SMOKE IN THE COCKPIT. WHEN PACKS WERE SHUTOFF SMOKE STOPPED. AFTER TRANSFERING BLEED AIR TO ENGINES AND TURNING PACKS BACK ON THERE WAS NO MORE SMOKE. WE HAD BEEN DEICED JUST PRIOR TO TAKING OFF WITH TYPE I AND TYPEIV FLUID. CORRECTIVE ACTION: REMOVED AND REPLACED LEFT AND RIGHT ACM COOLING TURBINES IAW CH-650 AMM 21-51-16. REINSTALLED WATER SEPERATORS PER CH-650 AMM 21-51-13. PERFORMED LEAK CHECK AND OPERATIONAL / TEST, NO DEFECTS NOTED.
MAIN CABIN DOOR FOOT PEG DID NOT EXTEND WHEN DOOR WAS OPENED. DOOR DID NOT BANG ON GROUND. CORRECTIVE ACTION: REMOVED AND REPLACED THE MAIN DOOR SUPPORT PUSH ROD IAW CL650 AMM 52-11-25, CHECKED GOOD. ALL MEASUREMENTS WITHIN SPECIFIED LIMITS.
ON GEAR RETRACTION AS THE NLG ENTERED THE MLG WW COCKPIT CREW FELT & HEARD THE SPIN DOWN OF NOSE WHEEL THE TIME FRAME APPROXIMATELY A MINUTE. IT WAS MUCH LOUDER THEN NORMAL & COULD FEEL EXCESSIVE VIBRATION THROUGH PEDALS. THIS IS A REPEAT WRITE UP. AFTER PERFORMING THE TPC & NOTING NOSE TIRES WERE BOTH AT 163 PSI, AXLE JACKED NLG IAW MM 7-10- 07. PERFORMED A GVI OF NOSE WHEELS WITH NO OBVIOUS DEFECTS NOTED. PERFORMED REQUESTED SPIN-DOWN TEST OF BOTH WHEELS. REMOVED SUSPECTED FAULTY NR 1 NOSE WHEEL ASSY IAW MM 32-42-01. REMOVED SUSPECTED FAULTY NR 2 NOSE WHEEL ASSY IAW MM 32-42-01. DURING REMOVAL OF WHEELS, TECHS NOTED THE BREAK-AWAY TORQUES WERE 140 INCH IBS FOR THE NR 1 WHEEL, & 180 INCH LBS FOR THE NR 2 WHEEL. INSPECTED BEARINGS & NOTED GREASE APPEARED NORMAL IN COLOR & A SUFFICIENT AMOUNT PRESENT. CLEANED BEARINGS, BEARING SEALS, WHEELS, & AXLES WITH ONE DISCREPANCY NOTED; ONE GREASE SEAL, INBD SIDE NR 2 WHEEL, HAD A SMALL CUT. PER MCC, THE LAST EVENT INVOLVED A RETORQUE AND WERE ADVISED THE BREAK-AWAY TORQUE AT THAT TIME WAS FOUND AT 500 INCH LBS FOR ONE OF THE WHEELS. THIS MAY HAVE DAMAGED THE WHEEL. PLEASE REPLACE THE WHEELS AT THIS TIME. INSTALLED A REPAIRED NOSE WHEEL ASSY WITH TWO NEW BEARING SEALS, THE NR 1 POSITION IAW MM 32-42-01. INSTALLED AN OVERHAULED NOSE WHEEL ASSY WITH TWO NEW BEARING SEALS IN NR 2 POSITION IAW MM 32-42-01. VERIFIED A NORMAL SPIN DOWN OF BOTH WHEELS THAT WAS EQUAL WITH BOTH WHEELS. NO FURTHER ACTION IS REQUIRED.
AFTER TURNING ON THE APU AND AIR-CONDITIONING, PACS, STRONG OIL/SMOKE ODOR FROM AIR-CONDITIONING. NOTICED IN COCKPIT. PERFORMED A BOROSCOPE INSPECTION. PERFORM OPS CHECK OF A/C SYS WITH APU BLEED & VERIFIED OIL ODOR IN COCKPIT. PERFORMED BOROSCOPE INPECTION OF APU NOSE CONE. APU MUST BE REPLACED DUE TO REPEAT WRITE UP. REMOVED APU AND INSTALLED INSPECTED APU, SERVICED WITH 3QT APU AND STARTER GEN IAW 0MM 12-13-49. ALL LEAKS & OPS CHKD GOOD. NO DEFECTS. WORK COMPLETED IAW AMM 49-10-00. PERFORMED A FINAL OPS CHECK OF APU. & AIRCONDITIONING SYS IAW 49-50-00. CHECKED GOOD. NO OIL, ODOR/SMOKE IN COCKPIT.
DURING CLIMB OUT THE WHITE EXTERNAL DOOR OPEN CAS MESSAGE ILLUMINATED. ON THE POST FLIGHT WE DISCOVERED THE LAVATORY SERVICE DOOR WAS MISSING
LOUD WINING SOUND PASSING THROUGH FL 320. STOPPED CLIMB AT FL 360. DESCENT TO FL340. CABIN ALT 5300 FT. THE SOUND DECREASED BUT STILL THERE UNTIL CABIN ALT WAS BELOW 4800 FT. THE SOUND WAS COMING FROM THE LEFT SIDE OF THE MAIN ENTRANCE DOOR LEFT SIDE WHEN LOOKING FROM THE INSIDE. TECHS REMOVED THE DAMAGED SEAL, REPLACED WITH A NEW SEAL, AND APPLIED REQUIRED SEALANT AT 1730. POST SEALANT RURE OPS CHECKS LEAK CHECKS SATISFACTORY REF DRAWING PL 601R38624. REMOVED DAMAGED PRESSURE VALVE DOOR SPRING ROD AND REPLACED WITH A NEW SPRING ROD REF AMM 52·11-07. PERFORMED REQUIRED ADJUSTMENTS OF THE NEW SPRING ROD REF AMM 52-11-00. PERFORMED SATISFACTORY LEAK CHECKS OF THE PRESSURE VAVLE DOOR REF AMM 21-00-00.
MAIN CABLE DOOR UPLIFT ASSIST IS JAMMED BETWEEN THE PULLEY AND THE PULLEY BRACKET. DOOR IS STUCK HALF WAY UP AND WILL NOT GO DOWN DUE TO THE CABLE BEING LODGED. PULLEY FOR POWER ASSIST CABLE AND BRACKET TO BE DAMAGED. REMOVED THE PASSENGER POWER ASSIST DOOR CABLE -1 ,AND BRAKET, DOOR PULLEY, ACTUATOR SYSTEM ALL WORK IAW AMM 52- I L â¢22-000--801. LNSTALLCD PASSENGER DOOR POWER ASSIST DOOR CABLE CUSTOMER SUPPLIED NEW END BRACKET DOOR PULLEY, ACTUATOR SYSTEM WITH NEW COTTER KEYS ALL WORK IAW AMM 52-11-22--400-801. PERFORMED A SATISFACTORY OPERATIONAL TEST OF THE PASSENGER DOOR ACTUATOR MOTOR WITH NO DEFECTS FOUND, ALL WORK IAW AMM 52-11·19-710.801.
LAV DOOR: LAV DOOR IS JAMMED OPEN AND WILL NOT MOVE . DAMAGE INFLICTED ON DOOR RESCUING TRAPPED PASSENGER. R/R REPAIRED LAV DOOR. NO FURTHER DEFECTS
FLAPS FAIL AMBER CAS: WHEN FLAPS WERE SELECTED FROM 20 DEGREES TO 30 DEGREES IN FLIGHT "FLAPS FAIL" CAS MESSAGE ILLUMINATED. FLAPS REMAINED AT 20 DEGREES REGARDLESS OF SELECTION. AFTER LANDING FLAPS SELECTED TO ZERO BUT HAVE REMAINED IN 20 DEGREE POSITION. NO CIRCUIT BREAKERS WERE OBSERVED TO HAVE POPPED. // SIGNOFF// POWERED AIRCRAFT WITH APU IAW CL650 AFM. NOTED "FLAPS FAIL" CAS MESSAGE. PERFORMED FLAP RESET PROCEDURE REF. SMARTFIX TROUBLESHOOTING. RESET FLAP CONTROL LEVER TO O DEGREES AND FLAPS RETRACTED TO FULLY STOWED POSITION WITH NO DISCREPANCIES NOTED. PROCEEDED TO CYCLE FLAPS TO EACH POSITION 5 TIMES NOTING THAT FLAP MOVEMENT WAS SMOOTH AND NO ADDITIONAL CAS MESSAGES APPEARED. NOTIFIED MCC OF OUR FINDINGS AND WAS REQUESTED TO REMOVE AND REPLACE BOTH LH AND RH BRAKE DETECTOR UNITS. AWAITED PARTS. ARRIVED AT AIRCRAFT. REMOVED THE LEFT AND RIGHT OUTBOARD FLAP BUTE DOOR IAW CL650 MM TASK 27-54-13-000-801. REMOVED THE LEFT AND RIGHT AILERON SHROUD IAW CL650 MM TASK 57-21-04-000-805. REMOVED THE OUTBOARD FLAP HINGE-FAIRING IAW CL650 MM TASK 57-26-05-000-801. REMOVED FAULTY LEFT FLAP BRAKE DETECTOR UNIT P/N 2022280-8 S/N 2632A AND INSTALLED REPAIRED UNIT P/N 2022280- 8 S/N 1502AB IAW CL650 MM TASK 27-51-07-000-801. REMOVED FAULTY RIGHT FLAP BRAKE DETECTOR UNIT P/N 2022280-8 S/N 2633A AND INSTALLED REPAIRED UNIT P/N 2022280-8 S/N 1786A IAW CL650 MM TASK 27-51-07-000-801. POWERED AIRCRAFT ON APU POWER REF CL650 AFM AND PERFORMED THE OPERATIONAL TEST OF THE INBOARD AND OUTBOARD FLAPS IAW CL650 MM TASK 27-54-00- 710-801. 2 COMPLETE CYCLES OF THE FLAPS WERE COMPLETED FOLLOWED BY A FLAPS FAIL CAS MESSAGE WHILE TRANSITIONING BETWEEN FLAPS 45 AND FLAPS 30. PERFORMED THE RESET PROCEDURE OF THE FLAPS SEVERAL TIMES REF. SMARTFIX TROUBLESHOOTING NOTING THAT THE FLAPS WOULD MOVE 2-3 DEGREES AND FLAPS FAIL CAS WOULD ILLUMINATE. SHUT DOWN APU AND POWERED DOWN AIRCRAFT REF CL650 AFM AND LET AIRCRAFT SIT FOR 30 MIN. POWERED UP AIRCRAFT ON APU POWER REF CL650 AFM. PERFORMED THE OPERATIONAL TEST OF THE INBOARD AND OUTBOARD FLAPS IAW CL 650 MM TASK 27-54-00-710-801. NOTED THAT THE FLAPS FAIL CAS WOULD IMMEDIATELY ILLUMINATE WHEN A DIFFERENT POSITION FOR THE FLAPS WERE SELECTED. SHUT DOWN APU AND POWERED DOWN AIRCRAFT REF CL650 AFM. CONTACTED MCC WITH OUR FINDINGS. PER MCC (JEREMY), "PLEASE REMOVE THE REPAIRED LH BDU DUE TO TEAR DOWN REPORT INDICATING THAT ONLY THE NAME PLATE WAS REPLACED AND OPS CHECKS WERE DEEMED GOOD AT THE TIME, AND RE-INSTALL THE ORIGINAL LH BDU TO SEE IF ISSUE GOES AWAY." REMOVED REPAIRED LEFT FLAP BRAKE DETECTOR UNIT P/N 2022280-8 S/N 1502AB AND INSTALLED ORIGINAL UNIT P/N 2022280-8 S/N 2632A IAW CL650 MM TASK 27-51- 07-000-801. POWERED UP AIRCRAFT ON APU POWER REF CL650 AFM. PERFORMED THE OPERATIONAL TEST OF THE INBOARD AND OUTBOARD FLAPS IAW CL650 MM TASK 27-54- 00-710-80110 TIMES WITH NO DEFECTS NOTED. SHUT DOWN APU AND POWERED DOWN AIRCRAFT REF CL650, AND LET AIRCRAFT SIT FOR 10 MINUTES. POWERED UP AIRCRAFT ON APU POWER REF CL 650 AFM. PERFORMED THE OPERATIONAL TEST OF THE INBOARD AND OUTBOARD FLAPS IAW CL650 MM TASK 27-54-00-710-801 AN ADDITIONAL 10 TIMES NOTING STILL NO DEFECTS AT THIS TIME. SHUT DOWN APU AND POWERED DOWN AIRCRAFT REF CL650. CONTACTED MCC WITH OUR FINDINGS AND DETERMINED THAT TECHS RECEIVED A PART BAD FROM STOCK. MCC PLACED TWO MORE BDU'S ON ORDER AND REQUESTED TECHS REPLACE THE LH BDU WITH A NEWLY ORDERED COMPONENT. AWAITED PARTS. ARRIVED AT AIRCRAFT. REMOVED EXISTING LEFT FLAP BRAKE DETECTOR UNIT P/N 2022280-8 S/N 2632A AND INSTALLED REPAIRED UNIT P/N 2022280-8 S/N 1481A IAW CL650 MM TASK 27-51-07-000-801. POWERED UP AIRCRAFT ON APU POWER REF CL650 AFM. PERFORMED THE OPERATIONAL TEST OF THE INBOARD AND OUTBOARD FLAPS IAW CL650 MM TASK 27-54-00-710-801 10 TIMES WITH NO DEFECTS NOTED. SHUT DOWN APU AND POWERED DOWN AIRCRAFT REF CL650. INSTALLED THE LEFT AND RIGHT OUTBOARD FLAP BDU DOORS IAW CL650 MM TASK 27-54-13-000-801. INSTALLED THE LEFT AND RIGHT AILERON SHROUDS IAW CL650 MM TASK 57-21-04-000-805. INSTALLED THE OUTBOARD FLAP HINGE-FAIRINGS /AW CL650 MM TASK 57-26-05- 000-801. APPLIED THE AERODYNAMIC CONTOUR SEALANT AROUND THE SHROUD, FAIRING, DOOR, AND THE ACCESS PANEL IAW CL650 SPM-MM TASK 51-23-00-390-812. POWERED UP AIRCRAFT ON APU POWER REF CL650 AFM. PERFORMED THE OPERATIONAL TEST OF THE INBOARD AND OUTBOARD FLAPS IAW CL650 MM TASK 27-54-00-710-801 10 TIMES WITH NO DEFECTS NOTED.
