WHILE AT 28K ALTITUDE, CREW TURNED PACKS ON HIGH (HEAT), AFTER 20MIN RT PACK FAILED INDICATION ILLUMINATED, CREW FOLLOWED QRH. ABOUT A MIN LATER WITH PACKS ON LOW SETTING, CREW STARTED GETTING SMOKE. CREW FOLLOWED QRH FOR SMOKE BUT SMOKE DIDNâT EVAC FAST ENOUGH. CREW DESCENDED TO 10K & EMERGENCY DECLARED BY ATC. BY THE TIME 10K WAS REACHED, SMOKE WAS CLEAR. WHILE ON APPROACH TO KMQY CREW RAN THE PACKS IN LOW & NORMAL WITHOUT INCIDENT. SMOKE WAS MINIMIZED BY SHUTTING OFF PACKS, RECIRC FANS, AND GASPER FANS. SMELL WAS DESCRIBED AS SIMILAR TO WHEN A HOUSES HEATER IS USED FOR THE FIRST TIME. PERFORMED OPS CHECK OF BOTH PACKS IAW MM 21-60-00, NO DISCREPANCIES NOTED. PERFORMED VISUAL INSPECTION OF BOTH PACKS WITH NO DISCREPANCIES NOTED. CONTINUED OPS IAW MEL 21-50-2.
LOSS OF GREEN HYDRAULIC SYSTEM PRESSURE, RESULTING IN EMERGENCY PROCEDURES. EMERGENCY DECLARED AND GEAR WERE EXTENDED AND LOCKED. SMOKE APPEARED IN COCKPIT OF GEAR WAS DOWN AND LOCKED. INSPECTED AIRCRAFT AN FOUND ENGINE DRIVEN PUMP PRESSURE LINE ELBOW FITTING WITH EXCESSIVE CHAFFING, CAUSING A LEAK AND LOSS OF HYDRAULIC FLUID. REMOVED AND REPLACED FITTING AND SERVICED HYDRAULIC SYSTEMS. OPS CHECK NORMAL. INSPECTED AIRCRAFT ENGINE AND DETERMINED THAT THE SMOKE WAS GENERATED FROM THE HYDRAULIC FLUID LEAK BEING INGESTED INTO THE ENGINE INTAKE AND SUBSEQUENTLY INTO THE AIRCRAFT BLEEDS.
DURING TAKEOFF, LH ENG T6 OVER HEAT, ENGINE WAS SHUT DOWN AS A PRECAUTION. SMOKE WAS PRESENT IN THE COCKPIT. UPON WALKAROUND OIL WAS PRESENT ON THE EXTERIOR OF ENGINE COWING. MAINTENANCE PERFORMED A BORESCOPE AND IDENTIFIED OIL IN THE COMBUSTION CHAMBER WITH POSSIBLE #6 BEARING DETERIORATION BEING THE CAUSE. ENGINE WAS REPLACED.
SHORTLY AFTER TAKEOFF FROM KACK THE CREW NOTICED WHAT SEEMED TO BE SMOKE OR FOG COMING FROM BEHIND THE GLARESHIED ON THE CAPTAINS SIDE. THE CREW DONNED THEIR OXYGEN MASKS, DECLARED AN EMERGENCY, AND RETURNED BACK TO KCAK AND LANDED WITHOUT FURTHER ISSUE. KCAK BASED MAINTENANCE FOUND DE-ICING FLUID IN THE INTAKE OF THE APU WHICH UPON ROTATION MADE ITS WAY INTO THE BLEED AIR DUCTS CAUSING THE SMOKE. MAINTENANCE CLEANED ALL DE-ICING FLUID FROM THE APU INLET AND PERFORMED AN OPERATIONAL CHECK OF THE APU BLEEDS, WITHOUT ANY DEFECTS NOTED. MAINTENANCE ALSO INSPECTED ALL ELECTRICAL COMPARTMENTS IN THE COCKPIT FOR ANY BURNT WIRES OR COMPONENTS, WITH NO DEFECTS NOTED. A MAINTENANCE TEST FLIGHT WAS THEN PERFORMED TO VERIFY THE FIX, WITH NO DEFECTS NOTED. THE AIRCRAFT WAS THEN RETURNED TO SERVICE.

SHORTLY AFTER TAKEOFF, THE PILOTS TURNED THE BLEEDS TO NORMAL IAW AFTER TAKEOFF CHECKLIST AND RECEIVED A RED LT DUCT LEAK ANNUNCIATION. PERFORMED THE QRH PROCEDURES AND RETURNED TO THE AIRPORT. MAINTENANCE TROUBLESHOT THE ISSUE TO THE LEFT CROSS BLEED TUBE ASSEMBLY BEING RUPTURED. MAINTENANCE REMOVED AND REPLACED THE TUBE ASSEMBLY AND PERFORMED OPERATIONAL CHECKS IAW AMM. NO FURTHER DEFECTS WERE NOTED IN THE AREA.
DURING APPROACH THE LANDING GEAR DID NOT EXTEND WHEN THE GEAR HANDLE WAS PLACED IN THE DOWN POSITION. THE FLIGHT CREW BEGAN EMERGENCY PROCEDURES AND ACTIVATED THE ELECTRICAL OVERRIDE SYSTEMS AND THE GEAR EXTENDED AND THE FLIGHT CREW RECEIVED THREE GOOD DOWN AND LOCKED INDICATIONS. THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE TROUBLESHOT THE SYSTEMS AND FOUND THE LANDING GEAR RELAY BOX TO BE FAULTY. REMOVED AND REPLACED THE LANDING GEAR RELA BOX AND PERFORMED OPERATIONAL CHECKS. OPS CHECKS GOOD.

FA THEN CALLED BECAUSE THE BATHROOM FIRE ALARM WAS GOING OFF, NO INDICATION/EVIDENCE OF A FIRE WAS FOUND. FIRE ALARM STOPPED GOING OFF SHORTLY AFTER.
