FUEL SMELL IN HOVER AND WHEN BANKING RIGHT. PILOT COULD NOT SMELL FUEL. ONE CREW CAN SMELL FUEL IN AFT CABIN. NO FUEL LEAKS PRESENT. CANNOT SEAL HOSE ON END TO LEAK CHECK
ON THE P/N 206-033-426-001 SUPPORT FITTING ASSY., THE MOUNTING EARS WHICH PROVIDES THE PIVOT POINT FOR T/R CONTROL BELLCRANK P/N 206-001-763-001, WE DISCOVERED THAT THE PIVOT BOLT WAS SEIZED INSIDE THE MOUNT BUSHINGS AND ITS BUSHINGS MOVING WITHIN THE MOUNTING EARS ALLOWING THE BELLCRANK TO "WOBBLE" ON ALL AXIS. ON FURTHER INSPECTION WE FOUND THAT THE LOWER EAR OF THE MOUNT WAS CRACKED.
FORWARD TUB ASSEMBLY CORRODED.
RED M/R BLADE S/N: BH468848 HAS ONE SPOT OF CORROSION ON BOTTOM SIDE TRAILING EDGE NEAR OUTBOARD TRIM TAB. REMOVED AND REPLACED MAIN ROTOR BLADE WITH SERVICEABLE.
WHITE M/R BLADE S/N: BH468847 HAS MULTIPLE SPOTS OF CORROSION ON TRAILING EDGE. REMOVED AND REPLACED MAIN ROTOR BLADE WITH SERVICEABLE.
DURING A ROUTINE TAILROTOR GEARBOX CHANGE, WHILE COMPLETING THE TAILROTOR BALANCING PROCEDURES. AFTER MX OBTAINED THE VIBRATION READING AT 100 PERCENT, THE PILOT DECELERATED THE ACFT TO FLIGHT IDLE AND APPROX 10 SECONDS INTO THE COOL DOWN PROCEDURES (AT FLIGHT IDLE) THE NR 2 TAILROTOR SEGEMENTED DRIVESHAFT FAILED AT THE CENTER OF THE DRIVESHAFT.
FOUND PITTING AND CORROSION, SOME THAT GOES THROUGH OUTER LAYER OF HONEY COMB TUB, IN THE AREA JUST BELOW AND AFT OF THE LEFT HAND REAR DOOR JAM AT THE BOTTOM AFT CORNER.
