ATA 3220

THE PILOT REPORTED THAT WHEN THEY LOWERED THE GEAR HANDLE ONLY THE 2 MAIN GEAR GREEN LIGHTS ILLUMINATED. THE NOSE GEAR GREEN LIGHT DID NOT ILLUMINATE, AS WELL AS NO RED IN TRANSIT LIGHT ILLUMINATED. MAINTENANCE COULD NOT DUPLICATE THE NOSE GEAR DISCREPANCY ON JACKS AS THE LANDING GEAR CYCLED NORMALLY ON MULTIPLE ATTEMPTS. MAINTENANCE THEN REPLACED THE NOSE GEAR DOWN HYDRAULIC ACTUATOR, PERFORMING SEVERAL GEAR SWINGS WHILE THE AIRCRAFT WAS ON JACKS AND AGAIN THE LANDING GEAR CYCLED NORMALLY. AN OPERATIONAL CHECK FLIGHT (OCF) WAS PERFORMED ON 12/20/2021 FOR LANDING GEAR OPERATION WITH THE OCF UNSUCCESSFUL. THE NOSE GEAR DID NOT EXTEND ON THE FIRST ATTEMPT, THE LANDING GEAR CYCLED NORMALLY ON THE NEXT 5 ATTEMPTS. BQN MX CONTINUES TO TROUBLESHOOT THE SYSTEM, FURTHER INFORMATION WILL BE PROVIDED WHEN THE AIRCRAFT IS REPAIRED.

AFTER DEPARTURE, ABOUT 150 MILES OUT PILOT LOST COMMUNICATIONS WITH ATC. COMMUNICATIONS WERE RE-ESTABLISHED AFTER 10 MINUTES. THE PILOT ELECTED TO CONTINUE ON & LANDED WITHOUT INCIDENT. TROUBLESHOOTING REVEALED A CHAFED WIRE ON THE PILOT’S SIDE MICROPHONE SOCKET. REPAIRED THE CHAFED WIRE & PERFORMED AN OPERATIONAL CHECK OF BOTH COMM 1 & 2 RADIOS. SYS CHECKED SATISFACTORY.

AFTER DEPARTING SJU, WHEN GEAR WAS SELECTED UP AFTER T/O, GEAR WOULD NOT LOCK IN THE UP POSITION AND THE AIRCRAFT LOST HYDRAULIC PRESSURE. DECLARED AN EMERGENCY, EXTENDED THE GEAR USING THE EMERGENCY GEAR PROCEDURES, RETURNED TO SJU AND LANDED WITHOUT INCIDENT. MX INSPECTED THE HYDRAULIC SYSTEM, MX NOTED THE NLG UP HYDRAULIC TUBE UNDER THE COCKPIT FLOORBOARD HAD A LOOSE B-NUT WHERE THE FITTING GOES THROUGH THE FUSELAGE FLOOR. REMOVED THE B-NUT AND INSPECTED THE FLARED FITTING, NO DISCREPANCIES WERE NOTED. TIGHTENED FITTING, AND CHECKED THE OTHER FITTINGS IN THAT AREA FOR SECURITY. CONDUCTED SEVERAL GEAR SWINGS AND NO FURTHER LEAKS WERE NOTED. LAST LETTER CHECK E WHICH INSPECTS THE UNDER FLOOR AREA OF THE COCKPIT WAS CONDUCTED 09/29/2011, 507.7 FLIGHT HOURS AGO.
