CREW REPORTED SMOKE COMING FROM THE CO-PILOT AUDIO CONTROL PANEL. EMERGENCY WAS DECLARED AND AIRCRAFT DIVERTED TO KBWD. MX FOUND EVIDENCE OF BURNING ON BACK OF AUDIO CONTROL PANEL. REPLACED THE AUDIO CONTROL PANEL, ALL OPS CHECK NORMAL.
DURING A SCHEDULED TEST FLIGHT, WHILE IN CRUISE THE SMOKE DETECTOR SOUNDED WITH SMOKE OCCURRING IN THE CABIN. THE PACK SUBSEQUENTLY FAILED AND THE SMOKE DISSIPATED. QRH PROCEDURES WERE COMPLIED WITH AND THE AIRCRAFT LANDED SAFELY. MAINTENANCE RAN THE AUXILIARY POWER UNIT, PERFORMED ENGINE GROUND RUNS AND INSPECTED THE PACKS. NO DEFECTS WERE NOTED. AIRCRAFT OK FOR SERVICE. (M)
AN EMERGENCY LANDING WAS PERFORMED COMING INTO DFW DUE TO A BIRD STRIKE TO THE CAPTAINS WINDSHIELD. MAINTENANCE REMOVED AND REPLACED A DAMAGED PIECE OF SKIN IN FRONT OF THE WINDSHIELD. OPERATIONAL CHECKS GOOD. AIRCRAFT OK FOR SERVICE. (M)
DURING FLIGHT, CAPTAIN RECEIVED A FLAP CONTROL FAULT. AN EMERGENCY WAS DECLARED AND THE A/C RETURNED, FOR UNEVENTFUL LANDING. MAINT RANG OUT WIRING AND MEGGED THE WIRING ON THE ENTIRE FLAP SYSTEM, ANNUNCIATOR CIRCUIT AND STALL WARNING CIRCUIT, FROM THE FLAP CONTROL UNIT TO THE ACTUATOR AND FROM THE ANNUNCIATOR TO THE FLAP CONTROL UNIT. NO DEFECTS NOTED. MAINT THEN JACKED UP A/C, INPUT DIFFERENT IN FLIGHT CONFIGURATIONS, AND RAN FLAPS WITH PITOT TEST BOX HOOKED UP. ONLY DEFECT NOTED WAS A VARIANCE IN THE RT OB ACTUATOR WHEN IN THE PARKED POSITION. MAINT REMOVED AND REPLACED THE RT OB FLAP ACTUATOR AND ALSO THE FLAP CONTROL UNIT. FLT 4869 - DURING FLIGHT FROM DFW-FSM, CREW NOTICED SMOKE COMING FROM THE F/O SIDE OF THE AUDIO CONTROL PANEL. THE CAPTAIN AND F/O DONNED MASKS AND GOGGLES. NO EMERGENCY WAS DECLARED. THE AIRCRAFT LANDED SAFELY IN FSM. MAINTENANCE FOUND THE F/O CHART HOLDER RHEOSTAT CONTACTS SHORTING TO THE GLARESHIELD STRUCTURE. MAINTENANCE REMOVED EXCESSIVE SOLDER AND APPLIED NON-CONDUCTIVE ANTI-CHAFE COMPOUND TO RHEOSTAT CONTACTS. NO FURTHER DEFECTS WERE NOTED. OPS CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE. (M)

FLT 4195 - CREW REPORTED AFTER TAKEOFF, GOT SMOKE IN CABIN. CREW FOLLOWED CHECK LIST AND RETURNED TO ATL AIRPORT. MAINTENANCE RESECURED RAM AIR INLET VALVE ON RIGHT PACK. OPERATIONAL CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE. (M)
FLT 4898 - ON THE RAMP, THE CABIN FILLED QUICKLY WITH SMOKE. CREW REPORTED LAVATORY SMOKE DETECTOR ALERTED. AIRCRAFT RETURNED TO GATE AND PASSENGERS WERE INSTRUCTED TO USE EMERGENCY EXITS. MAINTENANCE REMOVED AND REPLACED THE LEFT PACK GROUND COOLING FAN. OPERATIONAL CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE. (X)
FLT 4063 - CREW NOTICED SMOKE IN AIRCRAFT DURING TAXI. THERE WERE NO ASSOCIATED PACK, DUCT, OR BLEED INDICATION WARNING LIGHTS ILLUMINATED. MAINTENANCE RAN BOTH NR 1 AND NR 2 ENGINES FOR 15 MINUTES, CYCLED BOTH PACKS, AND SHUT BLEEDS ON AND OFF. MAINTENANCE ALSO INSPECTED ELECTRIC BAY, ACM HOSE AND ENGINE BLEEDS FOR OIL THEN RAN ENGINES NR 1 AND NR 2 FOR 15 MINUTES AGAIN. MAINTENANCE COULD NOT DUPLICATE DISCREPANCY. AIRCRAFT RETURNED TO SERVICE. (X)
FLT 4028 - AFTER TAKEOFF, CREW SMELLED SOMETHING BURNING. CAPTAIN TURNED AUTOPILOT ON AND TURNED ON COCKPIT READING LIGHTS AND SAW THIN WHITE SMOKE. F/A REPORTED BURNING SMELL, BUT SAW NO SMOKE IN CABIN AFTER INSPECTION. CREW UNABLE TO IDENTIFY SOURCE OF BURNING SMELL. CAPTAIN TURNED RECIRCULATING AND GASPER FANS OFF AND CLOSED EYEBALL VENTS. CREW DECLARED EMERGENCY AND RETURNED TO ATL WHERE AIRCRAFT LANDED SAFELY. MAINTENANCE REMOVED AND REPLACED THE LEFT PACK GROUND COOLING FAN. OPERATIONAL CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE. (M)
CLIMBING THROUGH 15,000 FT OUT OF DFW ON THE WAY TO AEX, CABIN REPAIDLY LOST PRESSURIZATION. CREW PERFORMED RAPID DEPRESSURIZATION PROCEDURES AND ADVISED ATC OF THE NEED TO DESCEND TO 10,000. CREW PERFORMED EMERGENCY DESCENT TO 10,000 FT AND RAN POH PROCEDURES. ATTEMPTS TO RETAIN PRESSURIZATION FAILED. FCC ADVISED CONTINUATION TO AEX. AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE RESECURED THE CARGO DOOR SEAL. OPERATION CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE. (M)

DURING INSPECTION, FOUND THE LEFT WING NR 10 STRINGER INBOARD OF NACELLE CRACKED. DETERMINED TO BE A MAJOR REPAIR IAW SP663 AND FAR 43 APPENDIX A. REPAIRED STRINGER IAW SRM 57-20-03, FIGURE 201. AIRCRAFT LEAK CHECKED GOOD AND WAS RELEASED FOR SERVICE. (M)
