THE FLIGHT CREW SELECTED THE LANDING GEAR TO THE DOWN AND LOCKED POSITION; THE RH MAIN LANDING GEAR DID NOT INDICATE THAT IT WAS SAFELY DOWN AND LOCKED ONCE IN POSITION. THE CAPTAIN PERFORMED AN EMERGENCY LANDING GEAR MANUAL EXTENSION TO ENSURE THAT THE POSITION OF THE GEAR WAS SAFE AND LANDED THE AIRCRAFT WITHOUT FURTHER INCIDENT. EL PASO MAINTENANCE FOUND THAT THE STOP FOR THE BELLCRANK WAS BROKEN AND THAT ALLOWED THE BELLCRANK TO SLIP FROM ITS NORMAL POSITION CAUSING A FALSE UNSAFE INDICATION IN THE COCKPIT. THE PART HAS BEEN ORDERED AND WILL BE REPLACED ONCE MAINTENANCE RECEIVES IT INTO STOCK.

ON APPROACH, LANDING GEAR WAS SELECTED TO THE DOWN POSITION, THE NOSE LANDING GEAR DID NOT EXTEND. THE PILOT PERFORMED A FLY BY OF THE CONTROL TOWER TO CONFIRM THE INDICATION. THE PILOT MANUALLY EXTENDED THE LANDING GEAR USING THE EMERGENCY PROCEDURES, THE NOSE GEAR EXTENDED AND THE PILOT RECEIVED THREE GREEN DOWN AND LOCKED INDICATION. THE PILOT LANDED WITHOUT INCIDENT. MAINTENANCE PLACED THE AIRCRAFT ON JACKS AND WAS ABLE TO DUPLICATE THE DISCREPANCY. MAINTENANCE FOUND THE NOSE LANDING GEAR DOOR ACTUATOR ASSEMBLY BINDING. MAINTENANCE REPLACED THE NOSE LANDING GEAR DOOR ACTUATOR ASSY. PERFORMED GEAR SWINGS AND OPERATION WAS NORMAL.

DURING CRUISE FLIGHT, THE PILOT NOTICED THE LT ENG OIL TEMP INDICATED ABOVE REDLINE, APPROX 110 DEGREES. R & R OIL TEMP BULB & PERFORMED ENG RUNNING OPERATIONAL CHECKS. ALL SYS CHECKED WERE NORMAL.

REPAIRED WIRE IAW WIRING MANUAL & PERFORMED OPERATIONAL CHECK FLIGHT. SYS CHECKS WERE NORMAL.

PILOT REPORTED THAT ON CLIMB-OUT THROUGH 6000 FT, THE LEFT ENGINE OIL TEMP ROSE INTO THE CAUTION RANGE. IT RETURNED TO NORMAL AFTER REDUCING POWER, BUT RETURNED TO THE CAUTION RANGE WHEN THE POWER WAS INCREASED TO CLIMB POWER SETTINGS AGAIN. THE PILOT DECLARED AN EMERGENCY AND SHUT DOWN THE ENGINE, AND RETURNED TO BORINQUEN, PR AIRPORT (BQN). MAINTENANCE FOUND A BAD OIL TEMP BULB AND REPLACED IT.

AFTER TAKE-OFF AT APPROXIMATELY 6000 FT AGL, PILOT NOTED LT ENGINE R.P.M. DROPPED FROM 100 PERCENT TO 97 PERCENT WITH SPEED LEVER FULL FORWARD. SHORTLY AFTER, COCKPIT FILLED UP WITH BLUE SMOKE AND LT ENGINE OIL PRESSURE WARNING LIGHT ILLUMINATED. PILOT SHUT DOWN THE LT ENGINE, DECLARED AN EMERGENCY, RETURNED TO CVG AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED LT ENGINE AND FOUND OIL LEVEL ABOUT 6 QUARTS LOW. OIL WAS LEAKING FROM PROPELLER HUB. INSPECTION OF THE PROPELLER HUB REVEALED PROPELLER PISTON O-RING P/N 7503503AF CUT IN HALF CAUSING THE OIL LEAK. THIS MASSIVE OIL LEAK FROM THE PROPELLER HUB INTO THE ENGINE INLET WAS THE CAUSE OF THE SMOKE IN THE COCKPIT.
