AMF FLT# 6905 EXPERIENCED A LH ENGINE FLAMEOUT AT 1500 FT DURING CLIMB, DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. SJU MAINTENANCE TROUBLESHOT STARTING L/H ENGINE NORMALLY AND FOUND L/H ENGINE BYPASSING AND SURGING IN IDLE WITH NO OTHER INPUTS. DURING TROUBLESHOOTING FOUND THAT THE L/H SRL INTERFACE WAS CAUSING THE SURGE AND BYPASS ISSUES. REPLACED SRL INTERFACE BOX AND RAN L/H ENGINE FROM IDEL TO TAKEOFF POWER AND SYSTEM FUNCTION CHECKED GOOD. THEN AN OPERATIONAL CHECK FLIGHT OF AIRCRAFT WAS COMPLETED AND L/H ENGINE PARAMETERS WERE FOUND NORMAL.

ATA 2910

DEPARTED KORL TO KSUS. DURING THE CLIMB THE RT HYD RED ANNUNCIATOR LIGHT ILLUMINATED. THE HYD PRESSURE GAUGE WAS SHOWING 1900 PSI AND STEADY IN THE GREEN. REVIEWED THE QRH AND DECIDED TO CONTINUE EN ROUTE SEEING THE HYD PRESSURE REMAINED IN THE GREEN. WHILE APPROACHING FL200, LT HYD RED ANNUNCIATOR LIGHT ILLUMINATED. HYD PRESSURE GAUGE STARTED TO FLUCTUATE BETWEEN 1900 PSI AND 300 PSI. AFTER SOME DISCUSSION AND REVIEWING THE QRH, DECIDED TO CONTINUE TO KSUS WITH THE UNDERSTANDING THAT A MANUAL GEAR EXTENSION AND FLAPS UP LANDING MAY BE REQUIRED. PERFORMED A MANUAL GEAR EXTENSION AND CONTINUE ON APPROACH WITH FLAPS IN THE UP POSITION. KSUS ATC WAS NOTIFIED OF THE SITUATION AND THE TOUCHDOWN WAS WITHOUT INCIDENT. WHILE TAXING ON THE RUNWAY, AT A SPEED APPROXIMATELY 40KTS, THE AIRCRAFT DEPARTED FROM THE CENTERLINE AND ROLLED TO THE LEFT. THE AIRCRAFT DID ROLL TOWARDS THE EDGE ON THE RUNWAY AND CAME TO REST. THE CREW WAS ABLE TO USE DIFFERENTIAL POWER AND TAXI BACK TO CENTERLINE AND TO THE RAMP WITHOUT ANY FURTHER INCIDENT. MX INSPECTED THE AIRCRAFT AND FOUND A CRACKED MAIN HYD LINE AFT OF THE RT ENGINE. HYD LINE, PART NUMBER 27-8132-357 WAS REPLACED ALONG WITH BOTH LT AND RT HYD PUMPS, PART NUMBER PV3-044-26. ADDITIONAL VISUAL INSPECTION OF THE FUSELAGE AND LANDING GEAR WAS PERFORMED, AND NO OTHER DEFECTS NOTED. THE RESERVOIR WAS SERVICED, AND AN OPERATIONAL CHECK OF THE HYD SYSTEM WAS PERFORMED AND OPERATED NORMALLY. THE AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE. NO ISSUES HAVE BEEN REPORTED ON SUBSEQUENT FLIGHTS.

THE PILOT REPORTED THAT WHILE CLIMBING, THAT THE COCKPIT WAS VERY HOT. AFTER TURNING THE TEMPERATURE CONTROLLER FROM AUTO TO MANUAL COLD, THE COCKPIT BECAME HAZY. A SHORT TIME LATER THE SMOKE STARTED TO CLEAR. THE PILOT ADVISED ATC, RETURN TO DEPARTURE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED BLEED AIR HEAT, COOLING COMPONENTS FOR CONDITION AND OPERATION. THE LEFT ECS SLAVE VALVE ACTUATOR WAS FOUND BROKEN. REMOVED AND INSTALLED OVERHAULED VALVE ASSEMBLY. GROUND RUN OPS CHECK WAS NORMAL. AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE.

THE PILOT REPORTED WHILE IN CRUISE FLIGHT TO KLAN THAT THE LEFT WING OVERHEAT ANNUNCIATOR ILLUMINATED STEADILY. THE PILOT IMMEDIATELY PERFORMED EMERGENCY PROCEDURES FROM THE QRH CHECKLIST. THE ANNUNCIATOR DID NOT EXTINGUISH AFTER TWO AND HALF MINUTES. AS PER THE QRH, THE PILOT PERFORMED AN IN-FLIGHT SHUT DOWN OF THE LEFT ENGINE, DECLARED AN EMERGENCY, AND DIVERTED TO KSUS. THE LANDING WAS WITHOUT INCIDENT. MAINTENANCE PERFORMED LEAK CHECK INSPECTION OF THE LEFT NACELLE AND UNDER CARGO FLOOR AREA. NO EXTERNAL LEAKS WERE NOTED. MAINTENANCE DISCOVERED THE WING OVER HEAT SENSOR LEADING INTO THE CABIN TO BE FAULTY. MAINTENANCE REMOVED AND REPLACED THE SENSOR WITH A SERVICEABLE UNIT. PART NUMBER “4344-221-22”. MAINTENANCE COMPLIED WITH GROUND RUNS AND LEAK CHECKS AND OPERATION OF THE LEFT BLEED AIR SYSTEM WAS NORMAL. THE AIRCRAFT WAS RELEASE FOR AN OPERATIONAL CHECK FLIGHT. CHECK FLIGHT WAS SUCCESSFUL AND THE AIRCRAFT WAS RETURNED TO SERVICE.

PILOT REPORTED AT CRUISE, THE HYDRAULIC PRESSURE ANNUNCIATION ILLUMINATED. EXECUTED QRH, LANDED WITHOUT INCIDENT. DURING INSPECTION, HYDRAULIC FLUID WAS LEAKING FROM RIGHT ENGINE NACELLE. SERVICED THE HYDRAULIC RESERVOIR. CYCLED GEAR AND FLAPS 10 TIMES CHECKING FOR LEAKS USING HYDRAULIC MULE. OPS CHECKED GOOD WITH NO LEAKS NOTED. PERFORMED LEFT AND RIGHT ENGINE RUNS. HYDRAULIC PRESSURE BEGAN TO FLUCTUATE AND A LEAK DEVELOP FROM OF THE RIGHT MAIN GEAR. FOUND HYDRAULIC LINES LEAKING FLUID INBOARD SIDE AND AFT OF THE RIGHT ENGINE FIREWALL. FOUND HYDRAULIC PUMP DRAIN LINES WITH EVIDENCE OF CHAFING. REMOVED AND REPLACED THE HYD LINES. OPS CHECKED GOOD. RETURNED TO SERVICE.

THE PILOT REPORTED AFTER TAKEOFF FROM KDFW THAT WHEN THE ENVIRONMENTAL SYSTEM WAS SELECTED TO THE MANUAL HOT POSITION THE CABIN FILLED WITH SMOKE. THE PILOT FOLLOWING THE QRH FOR SMOKE IN THE COCKPIT, PROCEDURES AND TURNED THE ENVIRONMENTAL SYSTEM OFF. THE SMOKE DISSIPATED AND THE PILOT NOTIFIED ATC TO REQUEST A RETURN TO KDFW. ATC GRANTED THE REQUEST AND DECLARED AN EMERGENCY AND CLEARED THE PILOT TO LAND. THE PILOT LANDED BACK IN KDFW WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT FOR EVIDENCE OF ACTUAL FIRE AND THOUGH THERE WAS NO ACTUAL FIRE, THE SOURCE OF THE SMOKE IS STILL UNDER EVALUATION IN THE ENVIRONMENTAL SYSTEM AND A SUPPLEMENTAL REPORT WILL NEED TO BE FILED.

THE PILOT HAD LEVELED OFF AFTER DEPARTURE FROM KDFW WHEN THE RIGHT HYDRAULIC LOW PRESSURE ANNUNCIATOR ILLUMINATED. THE PILOT FOLLOWING EMERGENCY PROCEDURES NOTIFIED ATC AND DECLARED AN EMERGENCY, THEN LANDED BACK IN KDFW WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT HYDRAULIC SYSTEM FOR OBVIOUS LEAKS AND NONE WERE NOTED AND THE QUANTITY LEVEL WAS OKAY. MAINTENANCE DEFERRED THE RIGHT LOW HYDRAULIC PRESSURE ANNUNCIATOR IN ACCORDANCE WITH THE MINIMUM EQUIPMENT LIST PROCEDURES AFTER VERIFYING THE RIGHT SYSTEM WAS IN FACT PRESSURIZING. THE HYDRAULIC PRESSURE SWITCH HAS BEEN PLACED ON ORDER AND SUPPLEMENTAL REPORT WILL HAVE TO BE FILED.

PILOT REPORTED THAT ON APPROACH INTO KMIA, THE RT MLG LIGHT REMAINED RED WHEN GEAR WAS SELECTED DOWN. THE PILOT USED THE MANUAL EXTENSION TO GET THE RT MLG INDICATING DOWN & LOCKED, LANDED WITHOUT INCIDENT. AIRCRAFT WAS FERRIED TO KOPF WHERE MX PERFORMED SEVERAL GEAR SWINGS BUT COULD NOT DUPLICATE THE PROBLEM. GEAR WAS SERVICED IAW THE ITEMS LISTED IN THE AMM 32-00-00 & RIGGING CHECKED AS A PRECAUTION.
