IAH - DURING FLIGHT THE CREW REPORTED THAT THEY SMELLED BURNING / SMOKE. RAN QRH TURNED OFF THE RECIRC FAN AND THE SMELL DISSIPATED. THE AIRCRTAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NUMBER 1 ACM, OPERATIONALLY TESTED WITH NO DEFECTS OR SMOKE SMELL NOTED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 4087, THE CREW REPORTED THAT THE EMERGENCY STRIP INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT STRIP, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 4403, THE CREW REPORTED SMOKE IN THE CABIN, SUSPECT ACM. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE #2 ACM, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING A MX INSPECTION, THE CEILING EMERGENCY LIGHT AT PANEL 253CLC AND 263ALC WERE FOUND INOPERATIVE. MX REMOVED AND REPLACED THE NR 3 BATTERY CHARGER, OPS CHECKED GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING A C-CHECK, THE RT SIDE ENGINE YOLK FWD. UPPER AFT FITTING HAS MECHANICAL DAMAGE INSIDE THE HAT BUSHING OF THE YOLK FITTING. MAINTENANCE REMOVED AND REPLACED THE YOLK FITTING IAW EA48-2018-026.
LT INBOARD FLAP DELAMINATED AREAS ON THE UPPER AND LOWER SIDES. REPAIRED LT INBOARD FLAP DELAMINATED AREAS IAW SRM 57-52-00. REPAIR IS CONSIDERED MAJOR PER SOP 5702.
DURING C-CHECK THE CENTER OMEGA BEAM WAS FOUND TO BE CORRODED BETWEEN FRAMES 59-61. MAINTENANCE REMOVED THE CORROSION AND PERFORMED A DOUBLER REPAIR IAW SRM 53-24-10. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE WASTE DRAIN SUPPORT WAS FOUND TO BE CORRODED BETWEEN FRAMES 60-61 STRINGERS 18R-20R. MAINTENANCE R & R THE SUPPORT IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE CARGO DOOR CUTOUT LOWER BEAM WAS FOUND TO BE CORRODED. MAINTENANCE R & R THE BEAM IAW SRM 51-40-02. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED ELECTRICAL BURNING ODOR AT ROW 4CD IN LIGHT AREA. OPEN
FUSELAGE DENT BETWEEN 7-8 AND STR 11L-13L. CUT OUT DENT INSTALLED FILLER, DOUBLER AND TRIPLER.
2 DENTS ON FUSELAGE BETWEEN FRM 7 & 8, STR 11L-13L. CUTOUT DAMAGED SKIN INSTALLED FILLER DOUBLER AND TRIPLER IAW ETD2014-145-060373.
THE CREW REPORTED STRONG ELECTRICAL BURNING ODOR WITH BOTH PACKS RUNNING. MAINTENANCE FOUND BURNED LIGHT SOCKET AT ROW 5A. THE ACFT WAS RELEASED FOR SERVICE IAW MEL. SUPPLEMENT TO FOLLOW.
DURING C-CHECK, FOUND A CHAFE IN THE VENTRAL TANK. MX BLENDED THE CHAFE AREA AND FOUND TO BE IN LIMITS. PLACED A DOUBLER OVER THE AREA AFTER FABRICATING THE PART, ALLODINED AND PAINTED IAW EC2810-01053. PERFORMED LEAK CHECKS IAW EC 2810-01053. NO LEAKS NOTED. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK.
THE CREW REPORTED A BURNING ODOR COMING FROM FRONT OF CABIN. MX INSPECTED AND REPLACED WIRING HARNESS FOR SEAT 1A SIDEWALL LIGHT. THEY ALSO REPLACED THE SIDEWALL LIGHT. OPS CHECKED GOOD. THE ACFT WAS RELEASED FOR SERVICE.
FLT 4489 EWR-JAX CREW REPORTED E1 FIRE DETECT MESSAGE ON EICAS DURING TAKEOFF. CREW PERFORMED AIR RETURN BACK TO EWR WITHOUT INCIDENT. EWR MX PERFORMED ALL ELECTRICAL CHECKS OUTLINED IN FIM 26-11-00-810-803-A WITH NO FAULTS NOTED. MX REMOVED AND REPLACED ENG 1 AND ENG 1 PYLON FIRE LOOPS IAW EMB145XR AMM 26-11-01 AND 26-11-02. INSPECTED VISIBLE HARNESSES WITH NO DEFECTS NOTED. OPERATIONAL CHECKS SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.

SHV - DURING C-CHECK STRINGER 21R HAS CORROSION BETWEEN FRAMES 7-13. MAINTENANCE REMOVED AND REPLACED STRINGER 21R BETWEEN FRAMES 7-13 IAW EMB-145XR SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2601, THE CREW REPORTED THE AUTO PRESSURIZATION TO BE INOPERATIVE. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE ELECTRO PNEUMATIC OUTFLOW VALVE, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
QRO, FLIGHT 2100, THE CREW REPORTED THAT THE CAPTAINS WINDSHIELD INNER PLY CRACKED ON CLIMB. THE AIRCRAFT RETURNED TO QRO WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT, AND REMOVED AND REPLACED THE CAPTAINS WINDSHIELD, OPERATIONAL AND LEAK CHECKS COMPLIED WITH SHOWING NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTINE MAINTENANCE THE LEFT OVERWING EMERGENCY EXIT WAS FOUND HARD TO OPEN. MAINTENANCE INSPECTED AND FOUND THE AFT UPPER SHROUD BRACKET SCREW HAD BACKED OUT AND WAS CATCHING ON THE DOOR FRAME. THE SCREW WAS REPLACED AND OPERATIONAL CHECKS OF THE EMERGENCY EXIT WERE GOOD.
CLE FLT 2958 THE CREW REPORTED A BLEED LEAK NR 1 ENGINE AND NR 1 PACK VALVE FAILURE ON CLIMBOUT. THE AIRCRAFT RETURNED TO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE LEFT PACK BLEED TUBE BETWEEN THE PACK VALVE AND DUAL TEMP CONTROL VALVE. OPERATIONAL CHECKS WERE GOOD. ALSO REPLACED RELAY KO435.
SHV - DURING C-CHECK BAGGAGE DOOR UPPER CUTOUT HAS CORROSION (LEVEL 2 LOCAL) AT FRAME 65. MAINTENANCE PERFORMED REPAIR TO BAGGAGE DOOR UPPER CUTOUT IAW EC5320-01535. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK BAGGAGE COMPARTMENT RAMP PANEL FWD ATTACHMENT BRACKET AT FRAME 65 HAS CORROSION (LEVEL 2 LOCAL). MAINTENANCE PERFORMED AND COMPLETED REPAIR OF FRAME BAGGAGE COMPARTMENT RAMP PANEL FWD ATTACHMENT BRACKET AT FRAME 65 IAW EMB-145XR SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK RT OMEGA BEAM CORRODED (LEVEL 2 LOCAL) FRAME 20 TO 23. MAINTENANCE INSTALLED REPAIR DOUBLER RT OMEGA BEAM AT FRAME 23 IAW EMB-145 SRM 51-40-02 & EC5320-01443. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
PRESSURE FLUCTUATING BETWEEN 8000-8300 AT 37000 FT. FLIGHT DIVERTED. AUTO AND MANUAL PRESSURIZATION INOP COULD NOT RECOVER CONTROL W/ THE MANUAL SYSTEM PER CHECK LIST. REMOVED AND REPLACED PNEUMATIC AND ELECTRO PNEUMATIC OUTFLOW VALVES IAW 21-31-03 AND 21-31-04. COMPLIED WITH OPS CHECK IAW 21-31-00, OPS CHECK GOOD. PERFORMED TEST FLIGHT AT FL 37 NO DISCREPANCIES NOTED. AIRCRAFT RELEASED AND RETURNED TO SERVICE.
LCH - DURING MX INSPECTION A GALLEY AREA EMERGENCY EXIT LIGHT WAS FOUND NOT ILLUMINATING. MAINTENANCE REMOVED AND REPLACED THE NUMBER TWO EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY TESTED WITH NO DEFECTS. AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
RT EMERGENCY SIDE WALL EXIT LIGHT AT 12BC INOP. EXIT MARKER SIGN ASSY WAS REMOVED AND REPLACED.
RIC - DURING A-CHECK, AIRCRAFT FAILED LANDING GEAR AIR-GROUND CHECK. RIC MX PERFORMED TROUBLESHOOTING AND FOUND LGEU NOT WORKING PROPERLY. MX REMOVED AND REPLACED LGEU AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. AIRCRAFT WAS APPROVED TO RETURN TO SERVICE.
FLT 2993 - EWR - CREW REPORTED TAT 2 HEAT INOP MESSAGE ON EICAS. CREW PERFORMED AIR RETURN AND LANDED ACFT UNEVENTFULLY. EWR MX REMOVED AND REPLACED NR 2 TAT PROBE IAW EMB145XR AMM 34-15-03 AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY.
CLE FLIGHT 2178 - THE CREW REPORTED A CLICKING NOISE FROM THE NOSE WHEN THE GEAR WAS RETRACTED AND THE MAIN GEAR CYCLED BETWEEN INTRANSIT AND LOCKED FOR ABOUT 15 SECONDS THEN LOCKED. GEAR EXTENSION NORMAL. MAINTENANCE INSPECTED AND REPLACED THE LANDING GEAR ELECTRONIC UNIT. OPERATIONAL CHECKS WERE GOOD.
AIRCRAFT HAD BIRDSTRIKE WHILE AIRBORNE AT 210 KNOTS AND 5000 FEET, AIRCRAFT LANDED UNEVENTFULLY. AFTER TEMPORARY REPAIR AND FERRY FLIGHT EWR TO TYS, TYS MAINTENANCE REPAIRED DAMAGE TO FRAMES 1A THROUGH FRAME 3 PART NUMBERS: 145-21705-403, 145-51202, AND 145-51203-403], STRINGER 1L, 4L, AND 6L PART NUMBERS: 145-54053-001, 145-54054-001, AND 145-54055-001 AND LT FORWARD NOSE SKIN, RT FORWARD NOSE SKIN, NOSE CENTER SKIN, SPLICED DAMAGED SKIN, PART NUMBERS: RT 145-54052-003 AND NOSE CENTER 145-30203-001, SEE LT NOSE SKIN BELOW. AIRCRAFT CURRENTLY UNDER GOING REPAIRS.
CLE FLIGHT 3150 THE CREW REPORTED THE AFT F/A FLASHLIGHT CHARGER WAS BROKEN. MAINTENANCE INSPECTED AND REPLACED THE FLASHLIGHT CHARGER. OPERATIONAL CHECKS WERE GOOD.
TYS DURING A 2C INSPECTION THE LEFT AILERON PCA BRACKET WAS MISSING A BUSHION AND BRACKET WAS GOUGED. MAINTENANCE REMOVED AND REPLACED THE BRACKET IAW EMB 145XR EC 5770-01177 AND SRM 51-40-02. ALL WORK WAS ACCOMPLISHED AND WAS RELEASED AT THE END OF THE C-CHECK INSPECTION.
LT AILERON DAMPNER BRACKET DAMAGED. REMOVED AND REPLACED LT AILERON DAMPNER BRACKET IAW EMB145 SRM 51-40-02 & CPM51-21-06.
PCA BRACKET MISSING BUSHINGS & DISPLACED METAL IN BUSHING HOLES. REMOVED AND REPLACED PCA BRACKET IAW EC5750-01177 REV B AND EMB145 SRM 51-40-02.
SDF - DURING 3A CHECK INSPECTION FOUND CABIN FLOOR FWD EMERGENCY PHOTOLUMINESCENT PATH LIGHTING STRIP COVER CRACKED.
