ALL REQUIRED CHECKLIST WAS PERFORMED AND FLIGHT DIVERTED TO BURBANK . LANDED SAFELY IN BURBANK AND DECLARED EMERGENCY . MX RESET FUEL INDICATION CB, OPS CHECK FI PASSED, EXECUTED FUEL TRANSFERS, NO FAULT NOTED, CHECKED BOTH ENGINE PUMPS FUEL FILTER INDICATIONS, BOTH WERE NOT POPPED, AND NO FAULT NOTED. WRITE UP REPEATED 3/16/26. R & R FUEL CONDITIONING UNIT. OPS TEST GOOD. THEN 3/23 FO SIDE FUEL INDICATION DASHED OUT. MX INSPECTED FUEL REPEATER PANEL FOR ACTIVE FAULTS, NONE NOTED. RESET FUEL QUANTITY C/B'S AND PERFORMED ELECTRICAL FUEL QTY INDICATING, ADJUSTMENT/TEST OPS CHECK GOOD. THEN 3/26 TANK 1 WAS DASHED OUT ON THE REPEATER WITH FAULT HI-ZL SHORT. MX FOUND PROBE BROKEN 1A FUEL PROBE, R & R THE LT 1A PROBE AND LT PROBE HARNESS. OPS & LEAK CHECKED GOOD. AIRCRAFT HAS FLOWN 4 LEGS NO ISSUES.
CREW REPORTED- DURING CLIMB WE HAD THE FOLLOWING LIGHTS: NO ICE- A/ICE ON; E1A/ICE FAIL; (BLEED TEMP RISE) ALSO, E1 AIR INLET STUCK OPEN. CORR ACTION- MX INSPECTED SYSTEM AND FOUND THE ANTI-ICE VALVE STUCK IN THE CLOSED POSITION. REMOVED LEFT ENGINE ANTI-ICE VALVE P/N C146009-X4 S/N 1495 , INSTALLED P/N C146009-X4 S/N 2581. OPS TEST GOOD.
CREW REPORTED DURING THE TAKEOFF ROLL AT APPROXIMATELY 90 KTS , THE RIGHT WINDSHIELD WIPER MOVED FROM ITS STOWED POSITION TO THE BOTTOM OF THE WINDSHIELD. CORR ACTION- MX PERFORMED AN OPS TEST OF THE WIPER SYSTEM OP CHECK GOOD AND INSPECTED WIPER AND MOUNTING POST NO DEFECTS NOTED AIRCRAFT WAS RTS'D.
CREW REPORTED-UPON ROTATION WE RECEIVED A LANDING GEAR AIR TO GROUND FAIL EICAS MESSAGE. TRIED TO MOVE THE LANDING GEAR LEVER UP AND IT WAS STUCK AND WOULD NOT RAISE. WE RAN THE QRH FOR THE EICAS MESSAGE AND IT DID NOT HELP. MADE AN AIR RETURN TO VEGAS FOR AN UNEVENTFUL LANDING. CORR ACTION- MX R&R'D NLG PROX HARNESS PN 145-21495-403 AND NLG PROX SWITCH OFF PN 8-527-01 SN 18854 ON PN 8-527-01 SN A20530 ALL OPS CHECKS GOOD. PERFORMED MX CHECK FLIGHT AND ALL CHECKED GOOD AS WELL.
CREW REPORTED-LG/ LEVER DISAGREE - WARNING MSG AND TRIPE CHIME DURING CLIMB PASSING 3,000 JUST AFTER TAKE OFF. AFTER SAFE ALTITUDE RAN QRH PER GUIDANCE RECYCLED GEAR. LDG INDICATORS WENT FROM WHITE TO AMBER, THEN RED FOLLOWED BY SAME WARNING MSG PASSING THROUGH FL180. CORR ACTION- MX TROUBLESHOT MESSAGE LG/LEVER DISAGREE ON EICAS, FOLLOWED FIM LEAD US TO BLOCK 22 TRANSITORY CONDITION OF LANDING GEAR EXTENSION/RETRACTION. PERFORMED CONTINUITY CHECKS OF ALL THE "OUTPUT" PINS OF ALL THE UP LOCK AND DOWN LOCK PROXIMITY SWITCHES TO THE LGEU. ALSO PER MOC, REMOVED AND REPLACED THE LGEU. REMOVED: PN: 355-022-003 SN: 4188 INSTALLED: PN: 355-022-003 SN: 4134. OPS CHECK GOOD AT THIS TIME, PERFORMED 5 GEAR SWINGS, ALL CHECKED GOOD, NO EICAS MESSAGES PRESENT, NO LG/LEVER DISAGREE AT THIS TIME.
CREW REPORTED- AFTER TAKEOFF, POSITIVE RATE I CALLED GEAR UP. MOMENTS LATER LDG GEAR LEVER DISAGREE WITH NOSE GEAR INDICATING NEITHER GEAR UP OR DOWN. RAN QRH AND CYCLED GEAR , NO GOOD. DECLARED EMERGENCY AND RETURNED TO KLAS WITH NO FURTHER INCIDENT. CORR ACTION- MX FOUND LANDING GEAR LEVER DISAGREE AND NO INDICATION ON NOSE LANDING GEAR. DURING INSPECTION OF NLG FOUND ONE PROXIMITY SWITCH ON THE LG UPCLOCK PROXIMITY SWITCH FACE BROKEN, AND THE SECOND LG UPCLOCK PROXIMITY SWITCH GAP DISTANCE WAS O/O/L. OPS CHECK GOOD AT THIS TIME, GEAR SWING WAS COMPLIED WITH, NO FAULTS AT THIS TIME.
ISSUE CREW REPORTED- DURING CLIMB AT AROUND 17000, DEPRESSURIZATION OCCURRED SHOWING AMBER CABIN ALT AND CABIN RATE. CREW FELT THE PRESSURE DROP IN THEIR EARS, STARTED DESCENT AND EMERGENCY RETURN. TWO MORE DEPRESSURIZATION EVENTS AT 7000 AND 2000 FT OCCURRED ON APPROACH. CORR ACTION- MX R&R OF ELECTRO-PNEUMATIC OUTFLOW VALVE. PERFORMED FUNCTIONAL TEST OF PNEUMATIC OUTFLOW VALVES. CHECKS GOOD. PERFORMED PRESSURIZATION CONTROL SYSTEM - FUNCTIONAL TEST IN AUTOMATIC MODE. CHECKS GOOD.
CREW REPORTED AT APPROXIMATELY 500 FT, RECEIVED LG LEVER DISAGREE WITH LOUD AIR NOISE COMING FROM THE NOSE. WHAT SOUNDS LIKE THE NOSE WHEEL GEAR DOORS WERE STILL OPEN. FOLLOWED THE CHECKLIST, THE GEAR CYCLED UP CORRECTLY WITH NO MESSAGES AND CYCLED DOWN CORRECTLY WITH NO MESSAGES. MX INSPECTED ALL LANDING GEARS AND SURROUNDING AREAS. FOUND LT MLG DOWN LOCK PROXIMITY SENSOR NR 2 WIRING HARNESS AT CONNECTOR P2037 SHIELDING LOOSE FROM CONNECTOR. SECURED THE HARNESS SHIELDING AND ACCOMPLISHED SEVERAL GEAR SWINGS WITH NO PROBLEMS NOTED.
ISSUE CREW REPORTED THE FOLLOWING- LG LEVER DISAGREE MESSAGE. AFTER TAKEOFF ON 19L LAS EN ROUTE TO PHX. RETRACTED LANDING GEAR ON SCHEDULE. UPON REACHING ACCELERATION ALTITUDE, SUPPRESSED WARNING MESSAGES AND MASTER WARNING POPPED UP WITH EICAS MESSAGE "LG/LEVER DISAGREE". I RETAINED CONTROL OF THE AIRCRAFT, TOOK THE RADIOS AND INSTRUCTED THE FO TO RUN THE ASSOCIATED QRH PROCEDURE. MESSAGE DID NOT CLEAR. INFORMED ATC WE WOULD NEED TO RETURN TO LAS, NO EMERGENCY AND NO ASSISTANCE NEEDED, AS WE WERE ABLE TO GET LANDING GEAR DOWN/ 3 GREEN ON THE EICAS. AFTER VECTORS TO FINAL, SHOT VISUAL APPROACH TO 19R, LANDED WITHOUT INCIDENT. CORR ACTION- MX FOUND HYD SYSTEM WAY OUT OF BALANCE. MX BALANCED AND SERVICED HYD SYSTEM PER AMM. PERFORMED EXTENSION AND RETRACTION CHECKS, HYD SYSTEM LEAK CHECK AND ALL CONDITIONS NORMAL NO LG/LEVER DISAGREE EICAS MESSAGES PRESENT.
BAGGAGE DOOR OPEN MASTER CAUTION DURING CLIMB AROUND 6300 FEET, MESSAGE ILLUMINATED UNTIL DECENT BELOW 3000 FEET. CREW RTF TO SNA AND MX DEFERRED THE SYSTEM. 5/24/22 MX TROUBLESHOT AND ADJUSTED MICROSWITCH OPS TESTED GOOD.
ISSUE: BAGGAGE DOOR OPEN MESSAGE AT 15,000FT PILOT MADE THE DECISION TO RETURN TO BURBANK. CORR ACTION: UPON OPENING OF BAGGAGE DOOR, FOUND BAGGAGE DOOR CURTAIN STRAP LOOSE AND LYING OVER DOOR ROLLER TRACKS. SECURED AS NEEDED, OPS CHECK GOOD
ISSUE: LANDING GEAR LEVER DISAGREE MESSAGE AFTER TAKEOFF AND LANDING GEAR WOULDN'T RETRACT. EMERGENCY DECLARED BY BUR TOWER AIRCRAFT RETURNED TO BURBANK WITHOUT FURTHER INCIDENT. CORR ACTION: FERRIED TO LAS; LAS MX TEAM DUPLICATED DISCREPANCY AFTER JACKING/SWINGING GEAR. REPLACED LDG GEAR HANDLE, ALL OP CHECKS GOOD.
