DURING DESCENT INTO KFHU PILOT NOTED SMOKE COMING FROM PILOTâS YOKE. PILOT DID DECLARE AN EMERGENCY. PHX MAINTENANCE TRAVELED TO KFHU AND INSPECTED YOKE AUTO PILOT/PUSHER DISC. HOT MIC SWITCH, STEERING DISC, AUTO PILOT SYNC AND TRIM SWITCH AND FOUND NO EVIDENCE OF DAMAGE. AFTER THOROUGH CHECK OUT AND OPERATION OF SYSTEM, EVERYTHING IS OPERATING AS IT SHOULD AND NO EVIDENCE OF SMOKE COMING FROM YOKE. AIRCRAFT IS RETURNED TO SERVICE. NO DEFECTIVE PARTS AT THIS TIME.
FOUND A DEFECTIVE PROX SENSOR. REMOVED AND REPLACED SENSOR. ISSUE REMAINED. REMOVED AND REPLACED WARNING COMPUTER WITH NO CHANGE NOTED. FOUND SIGNAL WIRE SHORTING WITH SHIELDING GROUND, REPAIRED WIRE. ALL OPS CHECKS GOOD.
AFTER T/O FROM KONT AND RETRACTING THE FLAPS AT APPROX 1000 FEET, FLAP DISAGREEMENT LIGHT ILLUMINATED. CREW PERFORMED A SYSTEM CHECK PER QRH AND WAS UNABLE TO CLEAR THE FLAP DISAGREEMENT WARNING. DECLARED AN EMERGENCY, RETURNED TO KONT AND LANDED WITHOUT INCIDENT. MX INSPECTED AND FOUND A DEFECTIVE RT OUTBOARD FLAP ACTUATOR. IT WAS REMOVED AND A SERVICEABLE RT OUTBOARD FLAP ACTUATOR WAS INSTALLED. THE OPERATIONAL FLAP SYSTEM CHECKED NORMAL ON THE GROUND. THE AIRCRAFT WAS RELEASED FOR AN OPERATIONAL CHECK FLIGHT. THE CREW PERFORMED AN OPERATIONAL CHECK FLIGHT AND CYCLED THE FLAPS SEVERAL TIMES THROUGH ALL POSITION. THE FLAP SYSTEM CHECK WAS NORMAL AND THE AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE.
THE CREW REPORTED AFTER TAKEOFF FROM KLAX, A FLAP DISAGREEMENT WARNING OCCURRED. THEY PERFORMED A SYSTEM CHECK IN ACCORDANCE WITH QRH AND RECYCLED THE FLAPS, BUT THE FLAP DISAGREEMENT WARNING RETURNED. THE CREW DECLARED AN EMERGENCY, AND LANDED AT KONT WITHOUT INCIDENT. MAINTENANCE PERFORMED A VISUAL INSPECTION OF THE FLAPS AND FOUND NO DEFECTS. THEY ALSO PERFORMED AN OPERATIONAL AND SYSTEM BUILT-IN TEST AND FOUND NO ANOMALIES OR DUPLICATION OF FLAP DISAGREEMENT. THE AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE.
ON DEPARTURE, THE PILOT REPORTED THAT THE LT MAIN GEAR DOORS REMAINED OPEN. THE PILOT RETURNED & UPON GEAR EXTENSION REPORTED THAT THE NOSE GEAR DID NOT GET A GREEN DOWN & LOCKED INDICATION. PERFORMED THE EMER EXTENSION PROCEDURES & STILL DID NOT GET A GREEN DOWN & LOCKED INDICATION ON THE NOSE GEAR. THE PILOT DECLARED AN EMER & LANDED WITHOUT INCIDENT. AFTER LANDING THE NOSE GEAR DOWN & LOCKED LIGHT DID ILLUMINATE. PERFORMED A GEAR SWING ON JACKS & FOUND THE LG SOLENOID VLV, PN 18190-003, DEFECTIVE. R & R THE LG SOLENOID VLV. THE LG FUNCTIONAL CHECK WAS COMPLETED & THE LG FUNCTIONED NORMALLY.
AFTER TAKEOFF, THE RT ENG AUTO FEATHERED. THE RT ENG TORQUE READOUT WAS ZERO. ALL OTHER ENG PARAMETERS SHOWED NORMAL ENG OPERATING CONDITIONS. THE PILOT RETURNED & LANDED WITHOUT INCIDENT. INSPECTED THE RT ENG TORQUE SYS & FOUND THE RT ENG TORQUE SIGNAL CONDITIONER WAS DEFECTIVE. THE RT ENG TORQUE SIGNAL CONDITIONER WAS REPLACED. THE RT ENG WAS GND RUN, THE RT ENG TORQUE INDICATOR FUNCTIONALLY CHECKED NORMAL. TORQUE SIGNAL CONDITIONER IS AN ON CONDITION PART. TIME IN SERVICE IS UNKNOWN.
FLT 4939 - DURING A GROUND INSPECTION IN DFW, THE RIGHT AFT EXTERIOR EMERGENCY LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REPAIRED THE DEFECTIVE WIRE SPLICE ON THE RIGHT AFT EXTERIOR EMERGENCY LIGHT. OPERATIONAL CHECKS GOOD. AIRCRAFT OK FOR SERVICE. (M)
FLT 4888 - DURING CLIMB-OUT FROM AEX-GTR, AT APPROXIMATELY 14,000 FEET THE CABIN PRESSURE BEGAN TO CLIMB AT 2,000 FEET PER MINUTE. THE AIRCRAFT DESCENDED TO 10,000 FEET DURING WHICH TIME THE CAPTIAN NOTICED A GAP HAD OCCURRED BETWEEN HIS WINDSHIELD AND THE AIRCRAFT ALONG THE LOWER EDGE. THE CREW COMPLIED WITH QRH PROCEDURES, DECLARED AN EMERGENCY AND THE AIRCRAFT RETURNED TO AEX, LANDING SAFELY. MAINTENANCE REMOVED AND REPLACED THE CAPTAINS WINDSHIELD. SPEED TAPE WAS APPLIED TO EXPEDITE CURE TIME IAW MM 56-10-01. OPERATIONAL AND LEAK CHECKS GOOD. AIRCRAFT OK FOR SERVICE. (M)

DUE TO SUSPECTED OVER-TORQUE, THE RIGHT PROP ASSEMBLY WAS REPLACED AND THE RIGHT ENGINE INSPECTED IAW PWA MM. OPERATIONAL CHECKS WERE GOOD AND THE AIRCRAFT WAS RELEASED FOR FURTHER SERVICE. (M)

THE FOLLOWING MORNING, THE CREW RAN THE ENGINE AT 90 PERCENT TORQUE WITH THE BLEED OFF AND THE LIGHT REMAINED OFF. AN FBO MECHANIC MADE A LOG ENTRY FOR A FERRY FLIGHT TO ATL. REMOVED AND REPLACED THE PRESSURE REGULATOR FOR THE DE-ICE SYSTEM AT THE WATER SEPARATOR. LEAK AND OPERATIONAL CHECKS WERE GOOD, AND THE AIRCRAFT WAS RELEASED FOR FURTHER SERVICE. (M)
