Maintenance disclosures
ON TAKEOFF THE ENGINE INGESTED A FROG, RETURNED TO BASE. MECHANIC INSPECTED THE ENGINE INLET, REMOVED FROG REMAINS, WASHED THE ENGINE. AND COMPLIED WITH POWER CHECK. FOUND SERVICEABLE. AC WAS RETURNED TO SERVICE.
TAXING FOR TAKE OFF, ENGINE CHIP LIGHT CAME ON TAXING FOR TAKE OFF. MAINTENANCE REMOVED, INSPECTED AND CLEANED THE CHIP DETECTORS IAW MM. AIRCRAFT GROUND RAN FOR 30 MINUTES NO DEFECTS FOUND.
WHILE HOVERING FOR TAKE OFF, A NOTICEABLE BANG CAME FROM THE ENGINE COMPARTMENT AND A RIGHT YAW OCCURRED. AIRCRAFT CONTROL WAS MAINTAINED AND SYSTEMS INSTRUMENTS WERE IN THE GREEN. AIRCRAFT WAS THEN HOVERED TO THE MAINTENANCE PAD AND SHUT DOWN. MAINTENANCE FOUND COMPRESSOR BLEED VALVE AIR LINE LOOSE, TIGHTEN AIR LINE AND COMPLIED WITH MFG PNEUMATIC AIR LINE CHECK. INSPECTED COMPRESSOR AREA FOR FOD DAMAGE FOUND NONE, AIRCRAFT WAS GROUND RUN AND LEAK CHECKED AND OPS CHECK FLOWN GOOD.
END CAP CRACKED. REPLACED WITH SERVICEABLE ASSY.
BEARING IN TRANSMISSION RESTRAINT CAME UNSTAKED. REPLACED WITH SERVICEABLE PART.
