CREW REPORTED- ON INITIAL DEPARTURE FROM KSDL, AT 4000â MSL MASTER WARNING LIGHT ILLUMINATED AND DOUBLE BELL SOUNDED WITH AN AURAL âWARNING, GEARâ THAT WOULD NOT SILENCE. THERE WAS NO EICAS WARNING, AND ALL THREE GEAR SHOWED UP AND LOCKED. CONTINUED THROUGH 6000â MSL AND WE RECYCLED THE LANDING GEAR. WHEN GEAR WAS DOWN AND LOCKED, WARNING CEASED. WHEN GEAR WAS UP AND LOCKED, WARNING RECOMMENCED. NO EMERGENCY WAS DECLARED AND WE DECIDED TO DO AN AIR RETURN FOR MAINTENANCE. THERE WAS NO QRH CHECKLIST TO RUN FOR THIS MASTER WARNING SINCE THERE WAS NO LANDING GEAR LEVER DISAGREE, AND NO NON-ANNUNCIATION PROCEDURE FOR LANDING GEAR. MX - PERFORMED TROUBLESHOOTING AND WAS DETERMINED TO NOT BE A GEAR ISSUE BUT A GPWS/ALTIMETER ISSUE. MX R&R'D ALL 4 RADIO ALTIMETER ANTENNAS. ALL OPS TESTS GOOD.
CREW REPORTED-AFTER TAKEOFF UPON LEVELING OFF AT 10,000FT RECEIVING OF COMMS BECAME NEARLY IMPOSSIBLE DUE TO HEAVY SCRATCH WHITE NOISE. MISSED MULTIPLE COMMUNICATIONS. TRIED BOTH COMMS, CHECKED TO MAKE SURE THE MASKS WERE SECURE AND NOT INTERFERING, CHECKED LIGHTS ETC TO SEE IF ITâS SOMETHING ELECTRICAL INTERFERENCE. TRIED FO ONLY WITH CA UNPLUGGED AND VICE VERSA TO NO AVAIL. CLIMBED TO 12,800 AFTER BRIEF MOMENTS OF COMM CLARITY BUT STILL HAD ISSUED AND DECIDED TO RETURN. WE TRANSMITTED JUST FINE BUT FOR 4-5 MINUTES IT WAS PURE WHITE NOISE AND COULD ONLY RECEIVE VERY FAINTLY BROUGHT WHITE NOISE. THROUGH 8,000 COMM CLEARED UP FOR A BIT BUT ON 6 MILE FINAL LOST COMMS AGAIN WITH OAKLAND TOWER FOR 10 SECONDS. MX REMOVED AND CLEANED COMMS UNIT 1 AND 2 ALL CHECKS GOOD.
CREW REPORTED- LAV SMOKE, RED EICAS MESSAGE, RIGHT AFTER WHEELS UP ON TAKEOFF WE RETURNED BACK TO BURBANK WHILE RUNNING THE CHECKLIST. CORR ACTION MX R&R'D LAV SMOKE DETECTOR OPS CHECK GOOD.
CREW REPORTED; UPON TAKEOFF @700 FT , WARNING MESSAGE LAV SMOKE INDICATED AND PERSISTED FOR AT LEAST A MINUTE, UPON LEVELING OFF AND VECTORING BACK AROUND, MESSAGE DISAPPEARED ON THE DESCENT DOWN. CORR ACTION- MX PERFORMED INSPECTIONS AND TEST OF LAV SMOKE SYS IAW EMB AMM. NO FAULTS OR DEFECTS NOTED AT THIS TIME. NO VISUAL OR ODOR INDICATION OF SMOKE/FIRE IN LAVATORY.
ISSUE- CREW REPORTED PRESSURIZATION AUTOMATIC FAILURE DURING CLIMB OUT IN MOUNTAINOUS TERRAIN, AT NIGHT, AND WEATHER. REGAINED CONTROL OF PRESSURIZATION SYSTEM IN THE MANUAL MODE. DECLARED AN EMERGENCY, RETURNED TO KBJC, AND LANDED SAFELY. CORR ACTION MX CND WHILE PERFORMING AUTO & MANUAL PRESSURIZATION SYSTEM CHECKS ALL OPS TESTS GOOD.
ISSUE-CREW REPORTED DURING CLIMB RECEIVED LG DISAGREE EICAS MESSAGE. CLIMBED TO A SAFE ALTITUDE KEEPING AIR SPEED BELOW 200KTS. RAN QRH AND CYCLED GEAR A SECOND TIME AND RECEIVED THE SAME EICAS MESSAGE. RETURNED TO BURBANK NO FURTHER ISSUES. CORR ACTION- MX TROUBLESHOT IT TO A BAD LG CONTROL LEVER ASSEMBLY. R&R'D LEVER ASSEMBLY ALL OPS TESTS GOOD.
ISSUE-CREW REPORTED UPON TAKEOFF, VERY LOUD STATIC/FEEDBACK COMING THROUGH HEADPHONES AND UNABLE TO HEAR ATC, FLIGHT DECK, FA, & PAX. ONLY WAY TO HEAR ATC WAS THROUGH SPEAKER. STILL WAS UNABLE TO ESTABLISH COMMS INSIDE THE CABIN (PILOT TO PILOT) FA NOTED WHEN PILOT PA WAS MADE TO PAX ANNOUNCEMENT, IT WAS UNINTELLIGIBLE. MX REPAIRED SUSPECT SOLDER CONNECTION ON FO'S SIDE HEADPHONE JACK TERMINAL AND REPAIRED BROKEN SOLDER CONNECTION ON CAPTAIN HAND MIC AUDIO JACK. MX ALSO RERACKED BOTH ICU'S ALL OPS TEST COMPLETED WITH NO DEFECTS NOTED.
CREW REPORTED JUST AFTER TAKEOFF EXTREMELY LOUD STATIC NOISE ENVELOPED THE ENTIRE COMM 1&2 AND INTERCOM SYSTEM SO AS TO BE UNABLE TO COMMUNICATE WITH EITHER ATC OR FELLOW CREW MEMBERS. SLOWLY RETURNED TO NORMAL OPERATION ON 5 MILE FINAL. MX TROUBLESHOT BY CHECKING PIC & CP AUDIO JACK PANELS AND FOUND WIRE ROUTED ACROSS PIC'S PHONE JACK TERMINAL TANG, RE-ROUTED WIRE CLEAR OF TERMINAL. PERFORMED OPS TEST OF COMM I OPS CHECKS GOOD.
