Maintenance disclosures
DURING MAINTENANCE, INTERGRANULAR CORROSION WAS VISUALLY DISCOVERED ON THE RIGHT HAND SPAR STRAP OF N26514 AS PART OF SK402-47B (P/N: 5292029-2). THIS CORROSION IS LOCATED AT APPROXIMATE RIGHT WING STATION 106.79 NEAR THE OUTBOARD ENGINE BEAM NACELLE FITTING, AND IS OUTSIDE ALLOWABLE LIMITS FOR REPAIR PER SID 57-10-16 INSTRUCTIONS. THIS STRAP WILL BE REPLACED USING NEW PARTS.
PILOT REPORTED LEFT AND RIGHT ALTERNATOR LIGHTS ON IN FLIGHT. UPON INSPECTION MAINTENANCE DETERMINED THE LEFT ALTERNATOR FIELD WIRE HAD BECOME DISCONNECTED AT LEFT ALTERNATOR SWITCH. THE WIRE WAS RE-CONNECTED AND OPERATIONALLY CHECKED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED LEFT PROPELLER VIBRATES ABOVE 2500 RPM. MAINTENANCE INSPECTED AND FOUND THE LEFT ENGINE #1 CYLINDER EXHAUST VALVE ERODED AND RECESSED INTO CYLINDER HEAD, AND #4 THE CYLINDER TO HAVE LOW COMPRESSION AND THE #4 CONNECTING ROD BEARING TO HAVE A CRACK. THE CYLINDERS AND CONNECTING ROD WERE REPLACED, WITH NO FURTHER DISCREPANCIES.
PILOT REPORTED RIGHT OUTBOARD FLAP DID NOT EXTEND. INSPECTION FOUND THE RIGHT OUTBOARD FLAP ACTUATING ROD BROKEN AT THREADS. ROD WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING CLIMB OUT OF STT THE CREW REPORTED A DECREASE RT ENGINE MANIFOLD PRESSURE. SUBSEQUENT INCREASE IN RT THROTTLE LEVER DID NOT INCREASE MANIFOLD PRESSURE THOUGH MANIFOLD PRESSURE STABILIZED AT 26". THE FLIGHT WAS CONTINUED. RT ENGINE OIL TEMPERATURE BEGAN TO REDLINE AND OIL PRESSURE DROPPED DURING APPROACH ACCOMPANIED BY A VIBRATION FROM THE RT NACELLE. THE ENGINE WAS NOT INTENTIONALLY SHUTDOWN AND THE PROPELLER DID NOT ENTER A FEATHER CONDITION DUE TO LACK OF OIL PRESSURE. THE FLIGHT LANDED SAFELY AT THE INTENDED DESTINATION. SUBSEQUENT INSPECTION REVEALED THE NR 3 CYLINDER AND PISTON ASSEMBLY WERE SEVERELY DAMAGED. THE PISTON WAS COMING APART IN PIECES AND THE CYLINDER WALLS WERE SUBSTANTIALLY DAMAGED. THE INTAKE AND EXHAUST VALVES DID NOT EXHIBIT SIGNS OF DAMAGE. THE DAMAGE APPEARS TO HAVE BEEN CAUSED BY A PRE-IGNITION EVENT IN THE NR 3 CYLINDER. THIS ENGINE WILL BE REMOVED FROM SERVICE FOR OVERHAUL AND THE AIRFRAME SHOWER OF SPARKS SYSTEM WILL BE FUNCTIONALLY TESTED AND INSPECTED PRIOR TO FURTHER SERVICE.
PILOT REPORTED BURNING RUBBER SMELL IN MAIN CABIN, MAINTENANCE INSPECTED AND REPLACED THE AVIONICS MASTER SWITCH DUE TO SUSPECTED INTERNAL FAILURE (SUSPECTED DUE TO ODOR- NON REPAIRABLE PART).
PILOT REPORTED THAT DURING FLIGHT CONTROL CHECK ON TAXI OUT FOR DEPARTURE, THEY NOTED THAT THE AILERON TRIM WAS STUCK IN THE LT WING DOWN POSITION WHEN THEY ATTEMPTED TO ADJUST IT. THE PILOT RETURNED TO THE GATE. UPON MX INSPECTION, THE AILERON TRIM HINGE AND TRIM ACTUATOR WERE LUBRICATED, AND THE TRIM TAB TRAVEL WAS VERIFIED.
PILOT REPORTED DURING CLIMB AT ABOUT 400 FT AGL, STARTED TO LOSE POWER AND ENCOUNTERED ROUGHNESS FROM THE LT ENGINE. PILOT SHUTDOWN THE ENGINE, FEATHERED PROP, AND RETURNED TO THE FIELD. ON MX INSPECTION, REMOVED AND REPLACED THE TURBOCHARGER. OPEN
PILOT REPORTED THAT WHILE TAXIING OUT TO RUNWAY 8, NOTICED A FAINT SMELL IN THE CABIN. THEY INITIALLY THOUGHT IT WAS COMING FROM OUTSIDE THE ACFT. THEY THEN REALIZED THE SMELL WAS FROM INSIDE THE CABIN WHEN THEY NOTICED SMOKE COMING FROM BEHIND A PANEL TO THE LT OF THE CAPTAINS SEAT. THE PILOT PULLED OFF THE TAXIWAY, SHUTDOWN, AND EXITED THE ACFT. UPON INSP, MX FOUND THE SMOKE TO BE COMING FROM A FAULTY STARTER RELAY WHICH WAS SUBSEQUENTLY REPLACED.

CREW REPORTED, ON FINAL APPROACH, A SMALL PUFF OF SMOKE CAME FROM THE NAV/COM PANEL. CREW ACCOMPLISHED SMOKE IN COCKPIT CHECKLIST, SMOKE QUICKLY SUBSIDED AND NO FURTHER SIGNS OF THE PROBLEM EXISTED. ADVISED THE TOWER, AND LANDED WITHOUT INCIDENT. MX INSPECTED ACFT, FOUND AUTO PILOT COMPUTER HAD DISTINCTIVE BURNED ODOR. AUTO PILOT COMPUTER CIRCUIT BREAKER WAS PULLED AND COLLARED, AUTOPILOT WAS PLACED ON MEL (22-1-1). AFTER ISOLATING AUTOPILOT SYSEM, NO OTHER SOURCES OF ODOR COULD BE IDENTIFIED. ACFT RELEASED BACK TO SERVICE. AUTO PILOT COMPUTER WAS LATER R & R AND MEL WAS CLEARED.
DURING PREFLIGHT, CREW REPORTED AVIONICS MASTER SWITCH INOP. MAINTENANCE REMOVED AND REPLACED SWITCH.
UPON ENGINE START, AFTER SEVERAL ROTATIONS, PROP STOPPED AND WOULD NOT TURN. MECHANIC FOUND CRANKSHAFT LARGE GEAR ATTACH BOLTS SHEARED AND GEAR DISLODGED FROM CRANKSHAFT. MECHANIC REMOVED AND REPLACED ENGINE AND RETURNED AIRCRAFT TO SERVICE. (NE01200502601) (K)
UPON START, RT ENGINE FAILED TO TURN. MECHANIC FOUND PROP COULD NOT BE TURNED BY HAND EITHER. LATER INSPECTION FOUND CRANKSHAFT LARGE GEAR MISSING TEETH, AND STARTER ADAPTER MISSING TEETH ON DRIVE GEAR. MECHANIC TO REMOVE AND REPLACE ENGINE.
CREW DECLARED EMERGENCY EN ROUTE ACK/BOS WHEN SMOKE CAME FROM AFT SIDE PANEL AFTER RE-SETTING AVIONICS MASTER SWITCH, FOR AVIONICS FAILURE. USING EMERGENCY AVIONIC SWITCH, RESUMED COMMUNICATIONS WITH BOS. APPROACH, LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED AVIONICS MASTER SWITCH. SYSTEM CHECKED OK.
