THE PILOT REPORTED THAT AFTER LANDING, THE AXLE BROKE WHILE EXITING THE RUNWAY. MAINTENANCE INSPECTED AND FOUND THE RIGHT AXLE TO BE CRACKED. REMOVED AND REPLACED THE RIGHT LOWER BARREL AND AXLE ASSEMBLY. NO FURTHER DEFECTS NOTED.
DURING MAINTENANCE A CRACK WAS IDENTIFIED ON THE OUTBOARD LEG OF THE LEFT HAND OUTBOARD NACELLE FITTING (P/N 5292029-09.) THIS CRACK MEASURES APPROXIMATELY .149 IN LENGTH, AND WAS DISCOVERED USING THE LIGHTED BORESCOPE INSPECTION PROCEDURE AS OUTLINED IN MEL-54-02R2 TO SATISFY AD2017-10-09. THIS CRACK IS CONSISTENT WITH OTHERS THAT HAVE BEEN IDENTIFIED USING THIS SAME INSPECTION PROCEDURE. THIS FITTING WITH BE REPLACED WITH FACTORY NEW PARTS.
DURING NDT INSPECTION AS NOTED IN SID 57-10-16 A CRACK WAS IDENTIFIED ON THE RIGHT HAND INBOARD NACELLE FITTING (P/N: 5292029-22). THIS CRACK LOCATED ON THE LOWER LEG OF THE OUTBOARD SIDE OF THE FITTING MEASURES APPROXIMATELY .083" LONG AND IS CONSISTENT TO OTHER CRACKS FOUND USING THE LIGHTED BORESCOPE INSPECTION TECHNIQUE (MEL54-02R2) TO SATISFY THE REQUIREMENTS OF AD2017-10-09. THIS CRACK WAS NOT IDENTIFIED VISUALLY DUE TO LAYERS OF PRIMER PREVENTING VISUAL DISCOVERY. THIS FITTING WILL BE REMOVED AND REPLACED USING NEW PARTS.
DURING A ROUTINE INSPECTION, A CRACK WAS DISCOVERED IN THE RT OUTBOARD NACELLE FITTING. THE CRACK WAS FOUND ON THE OUTBOARD SIDE OF THE FITTING, JUST FORWARD OF THE SPAR STRAP. THE CRACK MEASURED .123 IN LENGTH.
PILOT REPORTED BURNING SMELL ON DESCENT, NO SMOKE, BUT WAS UNABLE TO IDENTIFY THE SOURCE. INSPECTED WIRING ON INSTRUMENT PANEL, SIDE CONSOLE & CO-PILOT CONSOLE. NO EVIDENCE OF BURNING FOUND AT THIS TIME.
PILOT REPORTED RT BRAKE FAILURE WHILE MAKING RIGHT 180 ON RUNWAY. LT MAIN GEAR EXITED RUNWAY. INSPECTED THE ACFT, NO DAMAGE NOTED. RE-SEALED RT MASTER CYLINDER, SERVICED & BLED. TAXI CHECKED GOOD.
PILOT REPORTED ON LANDING, RT BRAKE PEDAL WENT TO THE FLOOR & APPLIED NO BRAKE PRESS. INSPECTED RT BRAKE SYS WITH NO DEFECTS OR LEAKS NOTED. SERVICED RT BRAKE MASTER CYLINDER WITH HYD FLUID IAW MM CH 32-40-00. OPERATIONALLY CHECKED GOOD.
THE LT NACELLE OTBD ENG BEAM WAS FOUND TO BE CRACKED JUST FWD OF AFT ENG MOUNT, TRANSVERSE TO LENGTH OF BEAM, ACROSS UPPER FACE OF BEAM, DURING NDT PRESCRIBED WITHIN MEL-54-01. REMOVE BEAM ASSEMBLY, PERFORM DETAILED VISUAL INSPECTION OF SURROUNDING STRUCTURE & INSTALL NEW BEAM ASSEMBLY. A SUPPLEMENT TO THIS SDR WILL BE SUBMITTED ILLUSTRATING RESULT OF X-RAY.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5 WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE OF MEB91-11R1. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.
ON LANDING, PILOT NOTED SPARKS ON THE RT ENGINE COWL. SHUTDOWN RT ENGINE ON LANDING. WHEN INSPECTED, DISCREPANCY WAS CLEARED BY REROUTING POWER WIRE HARNESS AND INSOLATED IT IAW STANDARD PRACTICE.

THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT OF MEB91-11R1. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
AFTER TAXI IN, ON SHUTDOWN, LT ENG CONTINUED TO CRANK UNTIL MASTER SWITCH WAS SHUTOFF. MX REMOVED AND REPLACED STUCK START RELAY AND REPLACED ASSOCIATED WIRING AS A PRECAUTIONARY MEASURE. SYS CHECKED OK, ACFT RELEASED BACK TO SERVICE.
LEFT BRAKE FAILED DURING TAXI. MAINTENANCE INSPECTED BLED AND SERVICED LEFT BRAKE SYSTEM.
ON TAKEOFF FROM MAZ, AT 2400 FEET AND CLIMBING TO 3500 FEET, PILOT HEARD LOUD BOOM WITH RIGHT ENGINE SHUT DOWN AND PROPELLER FEATHERING. THE PILOT PERFORMED ENGINE FAILURE FLOW CHECK LIST AND RETURNED TO MAZ. PRELIMINARY INSPECTION SHOWS CATASTROPHIC FAILURE OF THE RIGHT ENGINE. MAINTENANCE WILL BE REMOVING AND REPLACING THE ENGINE. ENGINE WILL BE SENT TO TCM FOR TEARDOWN AND INSPECTION.
CREW REPORTED AVIONICS MASTER SWITCH INOP. MX CHANGED CIRCUIT BREAKER SWITCH.
CREW REPORTED LEFT ENGINE FAILED TO START. MECHANIC REMOVED AND REPLACED STARTER CLUTCH. ON INSPECTION OF REMOVED STARTER CLUTCH, MECHANIC FOUND DRIVE GEAR TOOTH SHEARED. RECOVERED SINGLE TOOTH FROM OIL PUMP. RETURNED AIRCRAFT TO SERVICE AFTER FURTHER INSPECTION OF ACCESSORY GEARS. CLUTCH TO BE RETURNED TO MANUFACTURER FOR FURTHER EVALUATION.
