Maintenance disclosures
AFTER ENGINE SHUTDOWN, THE CREW NOTICED A BURNING ODOR WHEN EXISTING THE AIRCRAFT AND SAW SMOKING COMING FROM THE NR 1 NACELLE. WHEN THE NACELLE WAS OPENED, IT WAS DETERMINED THAT AN EXTERNAL ENGINE FIRE HAD OCCURRED. DURING THE SHUTDOWN, NO FIRE INDICATION WAS NOTED AND NO FIRE EXTINGUISHING MEASURE WERE TAKEN. THE QUICK PRELIMINARY INSPECTION NOTED DAMAGE TO THE TOP ENGINE MOUNT, SEVERAL "SOFT" FLUID LINES, THE FUEL NOZZLE MANIFOLDS AND ADEL CLAMPS. THE MANUFACTURER WAS CONTACTED TO NOTIFY THEM OF THE ISSUE AND THEY RECOMMENDED TO REMOVE ALL ENGINE COMPONENTS FORWARD OF THE FIREWALL TO SEND IN FOR INSPECTION. THE ENGINE, ENGINE COMPONENTS, ENGINE TRUSS, WIRING HARNESS AND ALL FLUID LINES WERE REMOVED AND REPLACED.

RIGHT ENGINE TRUSS OTBD TOP TUBE HAS CORROSION PITTING PAST LIMITS.
ON CLIMB OUT, RETRACTED GEAR AND THE RT MLG WOULD NOT LOCK IN THE UP POSITION, WOULD EXTEND AND RE-CYCLE. EXTENDED GEAR NORMALLY AND HAD NORMAL GEAR EXTENSION AND DOWN AND LOCKED INDICATION. LANDED AT DEPARTURE AIRPORT WITHOUT INCIDENT. FOUND LANDING GEAR UPLOCK TO BE DIRTY AND STIFF, CLEANED AND LUBRICATED SAME, OPS CHECK SATISFACTORY FOR RETURN TO SERVICE.
PILOT REPORTED SMOKEY ODOR IN COCKPIT AND CABIN AREA, (HAZE). AN EMERGENCY WAS DECLARED, AND RETURNED TO DEPARTURE. MX ACCOMPLISHED ENGINE & BLEED SYSTEM AND CHECKS NORMAL, FULL COLD TO FULL HOT, AUTO & MANUAL WITH NO HAZE OR SMOKE NOTED. RT ENGINE & BLEED SYSTEM OPERATIONAL CHECKS WERE NORMAL, FULL COLD TO FULL HOT, AUTO & MANUAL WITH NO HAZE OR SMOKE NOTED. BOTH ENGINE AND BLEED SYSTEMS WERE CHECKED TOGETHER FROM FULL COLD TO FULL HOT IN AUTO MODE WITH NO HAZE OR SMOKE NOTED. DURING MANUAL SETTING AT FULL HOT DURING COMBINED SYSTEM CHECKS MAINTENANCE NOTED THAT WHEN THE INTERIOR TEMP OF THE AIRCRAFT WAS APPROACHING (UNBEARABLE), MINOR HAZE OBSERVED. AS THE TEMP ALLOWED TO COOL, THE HAZE DISSIPATED. IT IS SUSPECTED THAT THE PILOT HAD PLACED THE TEMP CONTROL SYSTEM IN MANUAL FULL HOT TO CREATE THE NOTED HAZE, (REPORTED SMOKE). NO CORRECTIVE ACTION WAS TAKEN.
PDX- ON TAKEOFF ROLL, NR 2 ENGINE TORQUE INDICATION STARTED TO FLUCTUATE. ABORTED TAKEOFF AND RETURNED TO THE GATE. AIRCRAFT UNDER REPAIRS.
PDX - RIGHT BRAKE REQUIRES PUMPING TO FEEL AS FIRM AS THE LEFT BRAKE. REMOVED NR 4 WHEEL AND ADJUSTED NR 3 AND NR 4 BRAKE SELF ADJUSTERS, OPERATIONAL CHECK GOOD.
PDX- RIGHT BRAKE REQUIRES PUMPING FOR FULL APPLICATION ON ROLLOUT. BLED RIGHT BRAKES, OPERATIONAL CHECK GOOD.
OIL SMELL IN BLEED AIR SYSTEM. GROUND RAN DETERMINED SMELL COMING FROM NR 2 BLEED. PLACED ON MEL. MEL CLEARED BY REPLACING THE NR 2 ENGINE GEARBOX CARBON SEAL. PERFORMED GROUND RUN ON AIRCRAFT, CHECKED GOOD.
GEG- NR 2 MAIN LANDING GEAR DRAG BRACE H MEMBER CENTER 2 LUGS FOR POSITIONING CAM/UPLOCK BROKEN. AIRCRAFT FERRIED FROM GEG-BOI. AIRCRAFT UNDER REPAIR.
SEA - FLIGHT CREW NOTICED SMOKE COMING FROM FIRST OFFICERS GASPER VENT ON LANDING. NR 2 BLEED AIR SHUTOFF AND SMOKE SEEMED TO STOP. NR 2 COOLING TURBINE REPLACED I/A/W MAINTENANCE MANUAL. NR 2 BLEED AIR OPS CHECKED GOOD.
SEA - FLIGHT CREW NOTED, SMOKE SMELL FROM RIGHT BLEED. SMOKE SMELL NOTED FROM RIGHT BLEED ONLY. NO EXTERNAL OIL LEAKS NOTED AND VERIFIED PROPER OIL LEVEL I/A/W MAINTENANCE MANUAL. WATER SEPARATOR SOCK NOTED AS DIRTY BUT NO EVIDENCE OF OIL CONTAMINATION. SOCK REPLACED AND RIGHT BLEED AIR OPS CHECKED GOOD ON GROUND RUN, NO SMOKE SMELL NOTED.
SMOKE IN CABIN OF A/C AFTER TOUCHDOWN. INSPECTED AND FOUND AIR CYCLE MACHINE FROZEN UP. DEFFERED PER MEL 21-9.
NOSE GEAR DOWN/LOCKED INDICATOR DID NOT ILLUMINATE ON GEAR EXTINSION. LIGHT ILLUMINATED AFTER ALTERNATE GEAR EXTENSION PROCEDURES. AIRCRAFT LANDED IN FCA WITHOUT FURTHER PROBLEMS. AIRCRAFT FERRIED TO PDX FOR REPAIR. RIGHT HAND NOSE GEAR DOWN/LOCKED SWITCH REPLACED AND NOSE GEAR INDICATION OPS CHECKED GOOD ON GEAR SWING.
GTF - TAXIING FROM THE RAMP, THE LEFT PROPELLER STRUCK A TRAFFIC CONE. CONE WAS THROWN INTO THE FUSELAGE AT STA 166.5, WL 77. DENT SIZE WAS APPROXIMATELY 12 INCHES BY 12 INCHES AND .50 INCH DEEP. ONE PROP BLADE WAS BENT FROM THE IMPACT. THE PROP WAS REPLACED AND THE ENGINE WAS REPLACED BECAUSE OF THE PROP STRIKE. ENGINE AND PROP REPLACEMENT WAS IAW MM AND BOTH OPS CHECKED GOOD ON GROUND RUN. AIRCRAFT FERRIED TO PDX FOR REPAIR OF THE FUSELAGE DENT. DAMAGED ICE SHIELD, FUSELAGE SKIN, STRINGER SECTION, INTERCOSTAL, AND FRAME SECTION. REPAIRED IAW SRM. AIRCRAFT RETURNED TO SERVICE.
MWH - GPU PLUG-IN BEGAN SMOKING WHILE AIRCRAFT WAS PARKED AT THE GATE WITH GROUND POWER PLUGGED IN. GPU RECEPTACLE REPLACED AND SYSTEM OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
PDX - THE RED INTRANSIT LIGHT DOES NOT ILLUMINATE DURING RETRACTION OR EXTENSION ON NOSE WHEEL ONLY, BULB TESTS GOOD. REMOVED AND REPLACED UP POSITION SWITCH AND COMPLIED WITH GEAR SWINGS. OPERATIONAL CHECKED GOOD AND A/C WAS RETURNED TO SERVICE.
PDX - MUST USE OVERRIDE TO RETRACT LANDING GEAR. REPLACED LANDING GEAR CONTROL AND GEAR OPS CHECKED GOOD ON GEAR SWING. AIRCRAFT RETURNED TO SERVICE.
BZN - NOSE GEAR INTRANSIT LIGHT FAILED TO ILLUMINATE DURING GEAR EXTENSION. ALL OTHER GEAR FUNCTIONS NORMAL. AIRCRAFT LANDED WITHOUT INCIDENT. AIRCRAFT WAS FERRIED TO BOI WHERE LT UPLOCK SWITCH ASSY WAS REPLACED. GROUND TESTS PERFORMED SATISFACTORY AND AIRCRAFT RELEASED FOR SERVICE.
PDX - LANDING GEAR WOULDN'T EXTEND USING NORMAL SYSTEM, ALTERNATE GEAR EXTENSION PERFORMED AND GEAR EXTENDED. LANDING GEAR VALVE ON HYDRAULIC POWER PACK REPLACED IAW MAINT MANUAL. GEAR OPS CHECKED GOOD ON GEAR SWING AND AIRCRAFT RETURNED TO SERVICE.
