UPPER BOX BEAM FWD STIFFENERS CRACKED. PN 206-033-110-229 & 206-033-110-197
HOSE CRACKED AT THE ATTACHMENT FITTING CAUSING A LOSS OF HYDRAULIC FLUID AND THE LOSS OF HYDRAULIC ASSISTED FLIGHT CONTROLS.
WHILE PERFORMING INSPECTION OF THE AIRCRAFT, FOUND ONE CYCLIC TUBE TO HAVE THE FIXEDAND NON ADJUSTABLE CLEVIS ON THE TRANSMISSION BELLCRANK END OF THE CONTROL TUBE TO BE ROTATING WITHIN THE TUBE.
DURING PREFLIGHT, LEFT TAILPIPE HOT LIGHT WOULD NOT TEST. REPLACED SERVICABLE OVERHEAD TEST PANEL NR 1.
ON GEAR RETRACTION CREW NOTED A VIBRATION, SUSPECTED POSSIBLY THE GEAR DOOR. THE AIRCRAFT RETURNED TO FIELD WITHOUT INCIDENT. MAINTENANCE REMOVED BOTH MAIN LANDING GEAR SAFETY PINS.
CARGO SMOKE LIGHT ILLUMINATED IN FLIGHT, CREW FOLLOWED QRH INSTRUCTIONS AND DISCHARGED NR 1 CARGO FIRE EXTINGUISHER. AIRCRAFT RETURNED TO FIELD WITHOUT INCIDENT. MAINTENANCE REPLACED BOTH FORWARD AND AFT CARGO SMOKE DETECTORS AND NR 1 CARGO FIRE BOTTLE.
AT APPROXIMATELY 200 FT AGL RT ENG LOST PWR, PWR FLUCTATED BRIEFLY BETWEEN 40 TO 90 PERCENT TORQUE. AT 2500 FT ON RETURN TO FIELD WITH PWR REDUCED TO 30 TO 40 PERCENT THE TORQUE FLUCTATED BETWEEN 0 TO 10 PERCENT. THE ENGINE WAS THEN SHUT DOWN. FOUND HP BLEED LINE HAD CLAMP FAILURE AND HAD SEPARATED. BORESCOPED ENG AND PERFORMED RUNS, ALL CHECKED OK.
FLT 4900 - ROC - ON TAKEOFF, THE TOWER CONTACTED THE CREW STATING THAT THEY OBSERVED A TIRE FALLING FROM THE A/C. THE CAUSE HAS BEEN DETERMINED TO BE A RIGHT NOSE WHEEL, P/N 5006227, BEARING FAILURE. REMOVED NLG LOWER TORQUE LINK, AXLE AND BOTH TIRE ASSEMBLYS. (M)
ON TAKEOFF THE TOWER CONTACTED THE CREW STATING THAT THEY OBSERVED A TIRE FALLING FROM THE A/C. THE CAUSE HAS BEEN DETERMINED TO BE A RT NOSE WHEEL BEARING FAILURE. REMOVED AND REPLACED NLG LOWER TQ LINK, AXLE, AND BOTH TIRE ASSYS.
FLT NR 4804 - YQB - AFTER TAKEOFF SELECTED GEAR UP INTRANSIT LIGHTS ILLUMINATED, BUT GEAR DID NOT COME UP. RETURNED TO YQB. MX FERRIED TO ALB WITH GEAR PINNED FOR REPAIR. COULD NOT DUPLICATE PROBLEM. REPLACED HDYARULIC PUMP AS A PRECAUTIONARY. (M)
