DURING CRUISE FLIGHT AT 43,000 FEET LOWER EIED DISPLAY WENT BLANK FOLLOWED BY BAGGAGE FIRE LIGHT AND AURAL WARNING. CREW STARTED DESCENT AND SENT FO TO INSPECT BAGGAGE AREA WITH NO SMOKE OR FIRE FOUND. DURING DESCENT, LEFT ENGINE FLAMED OUT. DECLARED EMERGENCY AND DIVERTED INTO KILM, FOLLOWED RELIGHT CHECKLIST, ENGINE RESTARTED NORMALLY PRIOR TO LANDING. NOTE- AS OF THIS DATE, HONEYWELL HAS NOT MADE A DETERMINATION OF CAUSE. THE ENGINE IS STILL IN THE TEST CELL AT HON.
LEFT MAIN TIRE FOUND FLAT AFTER PARKING. TECH REPORTED 5 OUT OF 7 WHEEL HALF THROUGH BOLTS LOOSE CAUSED WHEEL HALF O-RING TO SQUEEZE OUT AND TIRE WENT FLAT. THE TIRE THAT WENT FLAT WAS INSTALLED 11/26/2014 AS AN ASSEMBLY OVERHAULED BY A VENDOR WITH TIRE INSTALLED BY SAME VENDOR DURING OVERHAUL. DOCUMENTATION INCLUDING 8130 TAG FROM VENDOR WILL BE SUBMITTED WITH THIS SDR.
FLIGHT CREW NOTED ABNORMAL BRAKING AND CONTROL ON LANDING, ACFT CLEARED RUNWAY AND STOPPED ON RAMP WITH NO INCIDENT. FOUND LT MAIN TIRE FLAT WITH TREAD MISSING, NO OTHER ACFT DAMAGE NOTED. MX INSPECTED ACFT, AND REPLACED TIRE AND WHEEL ASSY. MX FOUND TREAD CAP SEPARATED FROM TIRE, AND FOUND THROUGH BOLTS IN WHEEL HALVES FINGER TIGHT. MX DETERMINED THAT WHEEL THROUGH BOLTS BACKED OFF, CAUSING TIRE FAILURE. WHEEL ASSY O/H BY VENDOR 1.3 HOURS SINCE O/H.
DURING ENGINE MINOR INSP, SENT OIL FILTER TO LAB FOR ROUTINE ANALYSIS, LAB REPORTED METAL IN OIL. CONTACTED MFG FOR EVALUATION, MFG INSTRUCTED TO INSTALL NEW OIL AND FILTER, PERFORM GROUND RUN FOR 1 HOUR, AND SEND FILTER OUT FOR REPEAT ANALYSIS. SECOND ANALYSIS FOUND MORE METAL (BEARING MATERIAL) IN OIL FILTER. MFG RECOMMENDED ENGINE REPLACEMENT. ENGINE HAS 958.2 HOURS SINCE O/H.
FOUND DURING INSPECTION. NOSE GEAR FOLLOW UP DOOR PN 45A31615-21 ON BRACKETS PN 45A31613-9 AND 45A31613-7 AT ATTACHING RIVET HAS CRACKS GOING FORWARD TO AFT THROUGH THE FIRST RIVET ALONG THE RIVET ROW. RELATIVE WIND PUSHING AFT ON THE DOOR WHILE GEAR IS EXTENDED FOR TAKEOFF AND LANDING AND VIBRATION. USING THICKER MATERIAL DURING MFG AND OR ADDING A DOUBLER. (K)
AFTER TAKEOFF, DEPARTURE INSTRUCTIONS WERE TO CLIMB TO 2000 FEET AND TURN LEFT TO HEADING 120. A LEFT TURN WAS INITIATED A 1000 FEET AND AT APPROXIMATELY 1300 FEET AND HEADING 180 IN LIGHT TURBULENCE, A BUMP WAS FELT BY BOTH PIC AND SIC. PIC VERIFIED FLIGHT CONTINUITY AND NORMAL ENGINE INDICATIONS...ALL NORMAL. AFTER LANDING AT DESTINATION AIRPORT (PBI), CREW FOUND DURING POSTFLIGHT THAT RT ENGINE LOWER COWLING WAS MISSING AND EVIDENCE OF BIRD STRIKE ON RT ENGINE UPPER COWLING.
