DURING ROUTINE MAINTENANCE, THE EMERGENCY EXIT SIGN BY THE SERVICE DOOR WAS FOUND INOPERATIVE. MAINTENANCE REPLACED THE INVERTER. OPS CHECKED GOOD.
PILOT REPORTED THE LOWER EMERGENCY EXIT LIGHT ON THE GALLEY DOOR IS OUT. MX REMOVED AND REPLACED THE LAMP. OPS CHECKED GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED AFTER ENTERING ICING CONDITIONS AND ICE PROTECTION ACTIVATED, A LOUD THUD NOISE WAS HEARD FOLLOWED BY THE STRONG ODOR OF SMOKE IN THE COCKPIT AND WHITE CLOUD OF SMOKE BY THE LAST 5 ROWS IN BACK OF ACFT. MAINTENANCE INSPECTED AND REPLACED THE NR 11 MID COUPLING O-RINGS ON THE BLEED AIR SYSYTEM. PERFORMED OPS CHECKS OF BOTH PACKS. NO DEFECTS WERE NOTED.

PILOT REPORTED THE MAIN CABIN DOOR HANDLE HARD TO OPEN. MAINTENANCE ADJUSTED MCD CABLE TENSION AND ADJUSTED INTERCONNECT RODS, OPS CHECKED GOOD. ACFT WAS APPROVED TO RETURN TO SERVICE.
DURING PREFLIGHT INSPECTION, THE CREW REPORTED THE EMERGENCY EXIT LIGHT AT SERVICE DOOR AND BULKHEAD AT 3CD WERE FOUND TO BE INOPERATIVE. INSPECTED AND RESEATED THE BATTERY IN THE POWER SUPPLY, OPS TESTED WITH NO DEFECTS, AND THE AIRCRATF WAS APPROVED FOR RETURN TO SERVICE.
DURING FINAL APPROACH, CREW REPORTED A BURNING ELECTRICAL ODOR AT ROW 7. ACFT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND FOUND A BLOWN AND CRACKED DUCT DIRECTLY BELOW ROW 7.
DURING FINAL APPROACH, THE CREW REPORTED A BURNING ELECTRICAL ODOR AT ROW 7. THE ACFT LANDED WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE ACFT AND FOUND A BLOWN AND CRACKED DUCT DIRECTLY BELOW ROW 7.
AFTER TAKEOFF, CREW REPORTED SMOKE IN LAVATORY AND ELECTED TO RETURN TO DEPARTURE. LANDED WITHOUT INCIDENT. MAINTENANCE DETERMINED SOURCE OF SMOKE TO BE COMING THROUGH THE NR 2 ENGINE BLEED. ENGINE WAS REPLACED, OPS CHECK PERFORMED AND THE DISCREPANCY CLEARED.

THE AIRCRAFT LANDED AT MCI WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT AND PLACED THE EGPWS ON MEL 34-41-00-1, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THE AIRCRAFT SUBSEQUENTLY LANDED AT DEN WHERE MAINTENANCE REMOVED AND REPLACED THE EGPWS UNIT, OPERATIONALLY TESTED WITH NO DEFECTS, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

ORD FLT 2301 - THE CREW REPORTED THE BRAKES PULSATE ON LANDING CAUSING THE AIRCRAFT TO SHAKE VIOLENTLY, AIRCRAFT DIFFICULT TO CONTROL. MAINTENANCE INSPECTED ALL BRAKE ASSEMBLIES AND WHEEL SPEED X-DUCERS AND HARNESS AND REPLACED THE NR 3 BRAKE ASSEMBLY. PERFORMED OPERATIONAL CHECKS AND PERFORMED HIGH SPEED TAXI.
IAH - FLIGHT 2045, THE CREW REPORTED A LAV SMOKE DETECTOR WENT OFF IN FLIGHT. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND REMOVED AND REPLACED THE LAVATORY SMOKE DETECTOR, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 2914 - EWR - FLIGHT CREW REPORTED LAV SMOKE INDICATION DURING CLIMBING, MESSAGE CLEARED ABOUT 1 MIN LATER. EWR MX PERFORMED OPERATIONAL CHECK ON LAV SMOKE DETECTOR SYSTEM AND COULD NOT DUPLICATE MESSAGE. ACFT WAS APPROVED TO RETURN TO SERVICE.

MX FOUND LOOSE SCREW ON NR 1 VHF ANTENNA. MX TIGHTEN SCREWS AND FLIGHT CREW PERFORMED COMM OPERATIONAL CHECK AND ALSO A FLIGHT TEST SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
IAH - FLIGHT 2020, DURING PREFLIGHT INSPECTION THE CREW REPORTED AN EXIT LIGHT IN THE CABIN AS INOPERATIVE. MAINTENANCE CLEANED THE CONNECTOR ON THE AFFECTED EXIT LIGHT, OPERATIONALLY TESTED WITH NO DEFECTS. AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - DURING PREFLIGHT INSPECTION THE CREW REPORTED THAT THE EMERGENCY EXIT LIGHTS / SIGNS AT ROW 12 WERE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT INVERTER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - DURING PREFLIGHT INSPECTION THE CREW REPORTED THAT THE EMERGENCY EXIT LIGHT AT THE MAIN CABIN DOOR WERE FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT INVERTER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLIGHT 3146 - EWR - DURING CLIMBING PAST FL 240, FLIGHT CREW REPORTED NOISE AND VIBRATION APPEARING TO COME FROM THE WINDSHIELD SEAM INSULATION. CREW PERFORMED AIR RETURN AND LANDED ACFT SAFELY. EWR MX FOUND CAPTAIN WINDSHIELD HUMP SEALANT PEELING OFF. MX TRIMMED THE LOOSE SEALANT AND PERFORMED SEALANT REPAIR.
IAH - DURING PREFLIGHT INSPECTION THE CREW REPORTED AN EMERGENCY EXIT SIGN AT ROW 12 TO BE INOPERATIVE. MAINTENANCE INSPECTED THE EXIT SIGN, RESECURED THE CONNECTORS, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2060 - THE CREW REPORTED THAT THE AIRCRAFT WOULD NOT PRESSURIZE IN FLIGHT. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE DIGITAL CONTROLLER, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLT 0017 - TUS TO SAT - DURING CRUISE FLIGHT CREW REPORTED HYD SYS 1 INDICATED 0 PRESSURE AND 0 QUANTITY. CONTRACT AND ONT MX PERSONNEL WERE DISPATCHED. AIRCRAFT DIVERTED TO TUS AND DECLARED NO EMERGENCY AND LANDED SAFELY. MX PERSONNEL AFTER VISUAL INSPECTION FOUND HYD SERVICE LINE DISCONNECTED. MX RE-TIGHTENED SERVICE LINE OPERATIONALLY CHECK SATISFACTORY AND AIRCRAFT WAS RELEASED TO RETURN TO SERVICE.
SHV - DURING C-CHECK LT OMEGA BEAM HAS CORROSION ON FR 20. MX REPAIRED LT OMEGA BEAM AT FR 20 IAW EMB 145 SRM 53-20-10. THE AIRCRAFT WAS APPROVED TO RETURN FOR SERVICE.
FLT 419 - LAS TO COS - DURING CRUISE FLIGHT CREW REPORTED AND DECLARED EMERGENCY AND DIVERTED THE FLIGHT BACK TO LAS WHERE IT LANDED SATISFACTORY WITHOUT INCIDENT DUE TO NR 1 FUEL QTY IND 0, FUEL INBALANCE, FUEL 1 LOW LEVEL EICAS MSGS. MX PERSONNEL PERFORMED NR 1 FUEL LOW LEVEL OPERATIONAL CHECKS AND SATISFACTORY CHECK GOOD AND AIRCRAFT WAS RELEASE TO RETURN FOR SERVICE IN AIRWORTHY CONDITIONS.
CREW REPORTED "SMOKE ODOR SMELLED IN COCKPIT IN FLIGHT, ONLY FOR FIVE MINUTES, GONE AFTER A FEW MINUTES. MAINTENANCE ACCOMPLISHED VISUAL INSPECTION OF COCKPIT AREA AND RELAY PANEL. NO DEFECTS NOTED IAW EMB145MM NR 5-20-14. AIRCRAFT RETURNED TO SERVICE.
