THE PILOT TOOK OFF FROM KTCC AND ON CLIMB OUT AT 3000’AGL, 7000 MSL SHORTLY AFTER REACHING CLIMB POWER SETTINGS, THE PILOT HEARD A LOUD POP FROM THE LEFT ENGINE AND OBSERVED A LARGE AMOUNT OF ENGINE OIL ON TOP OF THE LEFT ENGINE COWLING. THE PILOT FOLLOWING EMERGENCY PROCEDURES SECURED THE LEFT ENGINE AND RETURNED TO KTCC AND LANDED WITHOUT INCIDENT. MAINTENANCE RECEIVED PHOTOS AND OBSERVED THAT THE LEFT ENGINE NUMBER TWO CYLINDER HEAD HAD SEPARATED FROM THE CYLINDER BARREL. MAINTENANCE IS DEVELOPING A PLAN TO REPLACE THE LEFT ENGINE AT OUTSTATION IN TCC. CYLINDER P\N LW-12966 TSN 496.5 CYCLES SINCE NEW 583

DURING CLIMB OUT, THE PILOT REPORTED A LOSS IN MANIFOLD PRESSURE ON THE LT ENGINE. VISUALLY THE PILOT SAW SMOKE COMING FROM THE LT ENGINE. THE PILOT SHUTDOWN THE LT ENGINE, RETURNED & LANDED WITHOUT INCIDENT. THE LT ENGINE TURBO CHARGER OIL SEAL FAILED CAUSING THE LOSS IN MANIFOLD PRESSURE & SMOKE. REMOVED & REPLACED LT ENGINE TURBOCHARGER. PERFORMED ENGINE RUN OPS CHECKED GOOD. ALL ENGINE CHECKS WERE NORMAL.

ON 9/11/08 DURING AN INSP, IT WAS FOUND THAT THE EXHAUST FLANGE WHERE THE MANIFOLD IS CLAMPED TO THE WASTE GATE HOUSING, THERE WAS A CRACK IN THE FLANGE RADIUS THAT RAN APPROXIMATELY .3333 OF THE WAY AROUND THE PIPE. THE MANIFOLD ASSY WAS REPLACED WITH A NEW ASSY. IT IS HIGHLY RECOMMENDED TO PAY CLOSE ATTENTION TO THE FLANGES AND WELDS ON THESE PMA ASSEMBLIES, AS FAILURE OF THE EXHAUST PIPES COULD CAUSE CONSIDERABLE DAMAGE TO THE AIRCRAFT. IT MUST BE NOTED THAT THE MFG WELD PROCESS IN THIS AREA HAS RECENTLY CHANGED. (K)
