CLE FLIGHT 2501 THE CREW REPORTED A VERY LOUD NOISE FROM THE FORWARD SECTION OF THE AIRCRAFT. MAINTENANCE INSPECTED AND FOUND THE NOSE GEAR DOORS WOULD NOT CLOSE AFTER GEAR RETRACTION. THE NOSE GEAR DOOR SEQUENCE PROXIMITY SWITCH WAS REPLACED. OPERATIONAL CHECKS WERE GOOD.
CLE - UPON GEAR RETRACTION OF FLIGHT 2127 THE LANDING GEAR AURAL WARNING INDICATION CAME ON. REMOVED AND REPLACED THE FLAP SELECTOR LEVER IAW EMB 135/145 AMM 27-51-03. OPS CHECKS WERE GOOD OF THE FLAPS AND LANDING WARNING SYSTEM IAW EMB AMM 27-50-00 AND 32-60-00. A/C WAS RELEASED TO SERVICE.
DURING ROUTINE MAINTENANCE THE LT OVERWING EMERGENCY EXIT WOULD NOT OPEN. MAINTENANCE INSPECTED AND FOUND THE LT OVERWING INNER DOOR LINER TO BE MISALIGNED. MAINTENANCE REALIGNED LINER AND OPERATIONAL CHECKS WERE GOOD.

ALL GEAR INDICATED DOWN AND LOCKED AND THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE NOSE GEAR MAIN DRAG STRUT ASSEMBLY. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT 2438 THE CREW REPORTED, COULD NOT OPEN THE LANDING GEAR EMERGENCY EXTENSION ACCESS DOOR LOCATED IN THE COCKPIT. MAINTENANCE INSPECTED AND LUBED AND UNSTUCK THE ACCESS DOOR. OPERATIONAL CHECKS WERE GOOD.
AIRCRAFT AIR-RETURNED TO CLE, EMERGENCY DECLARED, THE LEFT ENGINE FAILED IN FLIGHT, ITT SPIKED AT 1100 DEG C, WITH A LOUD BANG. MAINTENANCE REMOVED AND REPLACED THE LEFT ENGINE, OPS AND LEAK CHECKED GOOD IAW EMB135-MM-71-00-00 AND TASK CARD 6271001. AIRCRAFT RELEASED TO SERVICE.

THEY REPORTED THE ODOR SEEMED TO BE ASSOCIATED WITH THE APU AND THE NR 2 PACK.THE AIRCRAFT WAS RELEASED IAW MEL 21-51-00/1. MAINTENANCE INSPECTED AND PERFORMED OPERATIONAL CHECKS OF THE APU AND NR 2 PACK. NO DEFECTS WERE FOUND. MAINTENANCE FOUND THE AFT EMERGENCY FLASHLIGHT CHARGER ASSEMBLY BURNED. THE CHARGER ASSEMBLY WAS REPLACED.
CLE FLIGHT 3085 THE CREW REPORTED A STRONG BURNING ODOR DURING ICE TEST AND WHEN NR 2 PACK IS ON. MAINTENANCE REPLACED THE NR 2 ENGINE FUEL NOZZLES. OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.

CLE FLIGHT 3193 THE CREW REPORTED NR 2 PACK HAD A BURNING SMELL. THE SMELL WENT AWAY AFTER THE PACK WAS TURNED OFF. MAINTENANCE INSPECTED NR 2 PACK AND PERFORMED SYSTEM CHECKS. NO DEFECTS WERE FOUND. THE AIRCRAFT WAS RELEASED FOR SERVICE.
CLE FLIGHT# 3194 THE CREW REPORTED ALL 4 GENERATORS FELL OFF LINE AFTER GEAR RETRACTION.THE #4 GENERATOR DID COME BACK ONLINE. THE AIRCRAFT RETURNED TO CLEVELAND AND LANDED WITHOUT FURTHER INCIDENT.MAINTENANCE INSPECTED AND FOUND ALL FOUR GENERATORS TO BE CONTAMINATED WITH GLYCOL AND SAND.ALL FOUR GENERATORS WERE REPLACED AND OPERATIONAL CHECKS WERE GOOD.THE AIRCRAFT WAS RELEASED FOR SERVICE.

ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED UPON COMPLETION OF ALL INSPECTIONS.

MAINTENANCE ACCOMPLISHED A BLEND-OUT REPAIR IAW EMB-135 SRM 51-20-03, 51-40-02 AND CPM 51-21-04,05,06. THE REPAIR WAS FOUND TO BE SATISFACTORY. THE AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.

MAINTENANCE ACCOMPLISHED A BLEND-OUT REPAIR AND PERFORMED A DYE PENETRANT INSPECTION IAW EMBRAER GST 3877/01, EMB-135 CPM 51-21-04, 51-21-05 AND 51-21-06. THE REPAIR WAS FOUND TO BE SATISFACTORY, THE AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.

MAINTENANCE REWORKED THE NICKS (BLEND-OUT) IAW EMB-135 SRM 54-20-01 AND EMBRAER GST 3879/01, THE REPAIR WAS FOUND TO BE SATISFACTORY. THE AIRCRAFT WILL BE RELEASED UPON COMPLETION OF INSPECTION.

TYS During a scheduled inspection the pottable water line was found corroded at the fuselage to tube fitting. Maintenance removed and replaced the potable water overflow line I.A.W.-EMB-135-AMM-38-10-07. Repairs where satisfactory and the aircraft will be released upon complection of the inspection.

CLE FLIGHT#3128 THE CREW REPORTED INFLIGHT WITH GEAR UP INDICATION.THERE WAS A LOUD NOISE AND VIBRATION COMING FROM THE NOSE AREA.THE AIRCRAFT LANDED IN CLEVELAND WITHOUT FURTHER INCIDENT.MAINTENANCE INSPECTED AND FOUND THE RIGHT HAND NOSE LANDING GEAR DOOR ACTUATOR LEAKING.THEY REPLACED THE ACTUATOR AND OPERATIONAL CHECKS WERE GOOD.THE AIRCRAFT TEST FLIGHT WAS GOOD.THE AIRCRAFT WAS RELEASED FOR SERVICE.
