DURING INSPECTION, THE EMERGENCY LIGHTS WERE FOUND TO BE INOPERATIVE AT ROWS THREE AND FOUR. MAINTENANCE REPLACED THE NUMBER 2 EMERGENCY LIGHT BATTERY. OPERATIONAL CHECKS FAILED. MAINTENANCE THEN REPLACED THE NUMBER 3 INVERTER. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED. ****THE FINAL CORRECTIVE ACTION IS BEING SUBMITTED AS A SUPPLEMENT TO THE ORIGINAL REPORT FILED ON 12/17/21. PART INFORMATION HAS ALSO BEEN UPDATED.
DURING INSPECTION, SEVERAL EMERGENCY LIGHTS WERE FOUND TO BE INTERMITTENTLY INOPERATIVE IN THE FRONT SECTION OF THE AIRCRAFT. DURING INSPECTION, THE COCKPIT INBD AND OTBD FLOOR FLANGES WERE FOUND TO BE CORRODED ON THE F/O SEAT TRACKS. THE INBD AND OTBD FLOOR FLANGES WERE REPAIRED IAW ETD 2015-145-010166.
DURING INSPECTION, THE INBD AND OTBD FLOOR FLANGES WERE FOUND TO BE CORRODED ON THE CAPTAINS SEAT TRACK LOCATIONS. THE FLANGES WERE REPAIRED IAW ETD 2015-145-010166.
DURING INSPECTION, CORROSION WAS FOUND ON THE FORWARD PASSENGER DOOR ATTACH FITTING BELOW THE HINGE ARM. THE ATTACH FITTING WAS REPAIRED IN ACCORDANCE WITH ETD 2015-145-008649.
DURING INSPECTION, THE ATTACH ANGLE ON THE FORWARD SIDE OF THE APU FIREWALL WAS FOUND TO BE CRACKED AT FRAME 80 AND STRINGERS 5R-7R. THE ATTACH ANGLE WAS REPLACED IAW SRM 51-40-02.

CREW REPORTED THAT THEY RECEIVED A HYD 1 LOW QTY MSG FOLLOWED BY A HYD SYS 1 FAIL MSG. THE CREW PERFORMED A MANUAL EXTENSION ON THE LG. THE ACFT LANDED WITHOUT FURTHER INCIDENT. MX REPLACED THE HYD SYS 1 ENG DRIVEN PUMP & PERFORMED THE LG FREE FALL CHECK. ALL OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION CORROSION WAS FOUND ON THE LT OMEGA BEAM AT FRAME 20. THE FRAME WAS REPAIRED IAW SRM 53-0010 & SRM 51-40-02.
DURING INSPECTION CORROSION WAS FOUND ON THE LT OMEGA BEAM AT FRAME 23. THE LT OMEGA BEAM WAS REPAIRED IAW ETD 2014-145-014567.
DURING INSPECTION THE SERVICE DOOR BOTTOM STRUCTURE ATTACHMENT FOR THE LINER NUT PLATE AREAS WAS FOUND TO BE CORRODED. THE SERVICE DOOR WAS REPAIRED IAW ETD 2014-145-019393-A.
CREW REPORTED THAT THEY RECEIVED A "BLEED 2 VALVE CLOSED" MESSAGE IN FLIGHT. THE CREW RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE BLEED SYSTEM 2 PUSH BUTTON. OPS CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, EMERGENCY LIGHT ON MAIN ENTRY DOOR HANDRAIL WAS FOUND TO BE INOPERATIVE. MX REPLACED THE CAUTION INDICATION HARNESS OPS CHECKED GOOD.
CREW REPORTED THE PRIMARY FLIGHT DISPLAY NUMBER 1 FAILED IN FLIGHT WITH AN ELECTRICAL ODOR PRESENT IN THE COCKPIT. THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE SWAPPED THE NUMBER 1 PRIMARY FLIGHT DISPLAY WITH NUMBER 2 MULTI-FUNCTION DISPLAY AND DEFERRED THE ITEM. THE DEFERRAL WAS CLEARED ON 2/8/2013 WHEN THE DISPLAY WAS REPLACED. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, CORROSION WAS FOUND ON THE GUSSET AT RIGHT OMEGA FRAME 61. THE GUSSET WAS REPLACED IN ACCORDANCE WITH SRM 51-40-02 AND SRM 51-20-03.
DURING INSPECTION, CORROSION WAS FOUND ON THE RAMP ANGLE FRAME 20 AT THE CENTER OMEGA BEAM. THE ANGLE AT FRAME 20 WAS REPLACED IN ACCORDANCE WITH SRM 51-40-03.

CREW REPORTED THAT DURING CLIMBOUT, AT 10,000 FEET, THEY RECEIVED A RUDDER SYSTEMS 1/2 INOP MESSAGE, AUTO-PILOT FAIL, AND YAW DAMPER FAIL MESSAGES. THE CREW FURTHER REPORTED THAT THE AUTO-PILOT FAIL AND YAW DAMPER FAIL MESSAGES CLEARED WHILE THE RUDDER SYS 1/2 FAIL DID NOT. THE CREW RAN THE QUICK REFERENCE HANDBOOK AND ALL MESSAGES CLEARED. AT 14,000 FEET THE RUDDER SYS 1/2 INOP MESSAGE RE-APPEARED. THE CREW RAN THE QUICK REFERENCE HANDBOOK AGAIN AND THE MESSAGE CLEARED. THE CREW RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS FOUND SAFE FOR A FERRY FLIGHT TO CMH. CMH MAINTENANCE REPLACED BOTH THE RUDDER 1 AND RUDDER 2 PRESSURE SWITCHES. OPERATIONAL AND LEAK CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.

CREW REPORTED THAT DURING TAKEOFF THERE WAS SMOKE IN THE CABIN AND COCKPIT. THE CREW ABORTED TAKE-OFF AND REPORTED THAT THEY RECEIVED NO FIRE INDICATIONS AND ALL ENGINE INDICATIONS WERE GOOD. AS SOON AS THE TAKEOFF WAS ABORTED THE SMOKE BEGAN TO CLEAR. MAINTENANCE ISOLATED THE SMOKE TO THE ENGINE ONE BLEED SYSTEM AND DEFERRED THE BLEED SYSTEM. THE AIRCRAFT WAS REMOVED FROM SERVICE IN KSDF FOR TROUBLESHOOTING. KSDF MAINTENANCE FOUND THE ENGINE ONE NUMBER 4 CARBON SEAL IN THE COMPRESSOR SECTION LEAKING. MAINTENANCE REPLACED ENGINE ONE, OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION THE LEFT OVERWING EMERGENCY LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REPLACED THE LEFT OVERWING EMERGENCY LIGHT BULB. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
AFTER TAKEOFF, CREW ATTEMPTED TO RETRACT LANDING GEAR, RECEIVED A LDG AIR/GND FAIL MESSAGE ON EICAS AND THE GEAR WOULD NOT RETRACT. THE AIRCRAFT RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. MAINTENANCE PLACED THE AIRCRAFT ON JACKS AND CYCLED THE LANDING GEAR BUT WERE UNABLE TO DUPLICATE THE FAULT. THE LANDING GEAR ELECTRONIC UNIT WAS REPLACED AS A PRECAUTION.
CREW REPORTED THAT DURING CLIMBOUT A VIBRATION AND LOW RUMBLE COULD BE HEARD AND FELT. AFTER LEVELING OFF, THE AIRCRAFT WOULD NOT ACCELERATE PAST 0.70 MACH IN CLIMB MODE. THE AIRCRAFT DIVERTED TO SDF AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE NOSE LANDING GEAR DOOR SOLENOID AND FLAP ELECTRONIC CONTROL UNIT AND GEAR RETRACTION TESTS CHECKED GOOD.
DURING INSPECTION THE OVERWING EMERGENCY LIGHTS WERE FOUND TO BE INOP. MAINTENANCE REPLACED THE LEFT OVERWING EMERGENCY LIGHT AND THE NR 4 BATTERY PACK WITH NO FURTHER DEFECTS NOTED.
DURING INSP, THE LT AND RT OVER-WING EMERGENCY EXTERIOR LIGHTS WERE FOUND TO BE INOP. MX REPLACED THE NR 4 EMERGENCY LIGHT BATTERY AND THE EXTERIOR LIGHTS OPERATED NORMALLY.
FLIGHT 7822, IAD TO IND - SHORTLY AFTER TAKEOFF, AT APPROXIMATELY 400 FT THE CREW RECEIVED A SERVICE DOOR OPEN EICAS MESSAGE. THE QRH CHECKLIST WAS COMPLETED AND IT WAS DECIDED TO RETURN TO IAD WHERE THE AIRCRAFT LANDED WITHOUT INCIDENT. THE SERVICE DOOR INDICATION WAS DEFERRED AND THE AIRCRAFT WAS RELEASED FOR SERVICE. THE MEL WAS CLEARED ON 4/25/2009 IN CMH WHEN A BROKEN WIRE WAS REPAIRED ON THE SERVICE DOOR IAW AMM 20-21-00.
DURING INSPECTION THE OVERWING EMERGENCY LIGHTS WERE INOP ON THE LEFT AND RIGHT SIDES. THE NR 3 EMERGENCY LIGHT BATTERY WAS REPLACED AND THE EMERGENCY LIGHT SYSTEM OPERATED NORMALLY.
FLIGHT 3438 - ATL-CLE - ON DESCENT WHEN FLAPS SELECTED TO 9, EICAS MESSAGES SPS ADV AND FLAP FAIL APPEARED. PERFORMED THE QRH AND DECLARED AN EMERGENCY. LANDED WITHOUT INCIDENT. MAINTENANCE RESET FLAPS AND CYCLED THROUGH, OPS CHECK GOOD.
FLOOR PROXIMITY LIGHT NR 2 AND NR 2 INOPERATIVE. REMOVED AND REPLACED EMERGENCY LIGHT INVERTER IAW MM 33-50-00. OPS CHECK GOOD.
NR 1 EMERGENCY FLOOR LIGHT INOPERATIVE. REMOVED AND REPLACED FLOOR EMERGENCY LIGHT IAW MM 33-26-06.
EMERGENCY LIGHTING PROXIMITY STRIPS 2 AND 3 INOPERATIVE. MAINTENANCE REMOVED AND REPLACED NR 2 EMERGENCY BATTERY IAW EMB 145 MM 33-50-06. OPS CHECK GOOD.
DURING DESCENT THROUGH FL200, THERE WAS A THIN CLOUD LAYER WITH ICING CONDITIONS. HEARD WHAT SOUNDED LIKE WATER DROPLETS SIZZLING. THEN SMELLED ELECTRICAL SMOKE AND AFTER A FEW SECONDS, SAW SMOKE COMING FROM THE FRONT OF THE GLARESHIELD WHERE THE HEATING ELEMENT FOR THE WINDSHIELD IS. TURNED OFF WINDSHIELD HEAT AND SMOKE CEASED. DECLARED AN EMERGENCY. MAINTENANCE REMOVED AND REPLACED NR 1 WINDSHIELD.
FLIGHT 900 - LAN-OKC - DURING INITIAL CLIMBOUT, RETRACTED GEAR DURING NORMAL SCHEDULE. RIGHT MAIN SHOWED IN TRANSITION (AMBER BOX) FOR APPROXIMATELY 10 SECONDS, THEN WENT TO RED. RAN QRH, RECYCLED GEAR PER QRH. WHILE PUTTING GEAR IN THE UP POSITION AGAIN, SAME PROBLEM. SELECTED GEAR TO DOWN POSITION, THREE GREEN, RETURNED TO LAN. DECLARED AN EMERGENCY. UPON POST FLIGHT, FOUND INBOARD RIGHT AXLE HUB CAP MISSING, AND WHEEL SPEED TRANSDUCER HANGING OUT OF THE AXLE. MAINTENANCE REMOVED AND REPLACED HUB CAP. OPS CHECK GOOD.
FLIGHT 6320 - FLL-MCO - ON FINAL APPROACH, NOTICED SMOKE AND AN ODOR OF SMOKE. THE ELECTRIC PAGE INDICATED GEN 3 AMP 105. PERFORMED GENERAL VISUAL INSPECTION OF COMPONENTS IN THE FORWARD AVIONICS BAY. NO BURNS OR ODOR OF SMOKE. RAN BOTH ENGINES FOR 20 MINUTES, AND NO SMOKE OR ODOR APPEARED IN THE COCKPIT OR CABIN. GEN NR 3 STABLE AT 50 AMPS. OPS CHECK GOOD IAW EMB 145 SMP.

RT ENGINE ITT 50 DEGREES HOTTER THAN LT ITT. INSPECTED AIRCRAFT NR 2 ENG, PERFORMED BORESCOPE OF COMPRESSOR TURBINE SECTION HOT SECTION WITHIN LIMITS. PERFORMED LEAK CHECK OF BLEED LINE, FOUND LEAKS AND REPLACED GASKETS. ALL CHECK GOOD AT THIS TIME.
