Part: LATCH — FAILED (ESCAPE HATCH) · airframe 2,891 hrs
SHORTLY AFTER LIFT OFF, HATCH ASSY SEPARATED FROM ACFT. RETURNED TO AIRPORT AND LANDED WITHOUT INCIDENT. FOUND LATCH PARTS; PIVOT, SPRING, AND ROD WERE ALL DISCONNECTED FROM CLEVIS AND LOCK NUT MISSING. FINDINGS INDICATE THE ROD VIBRATED LOOSE AND THE ASSY CAME APART.

DURING ROUTINE INSPECTION, THE UPPER LEFT STATION 300 TAIBOOM FITTING WAS FOUND TO BE CRACKED. THE CRACK APPEARED TO START AT THE BOLT HOLE AND EXTEND TO LOWER EDGE. THE FITTING WAS REPLACED IAW SIKORSKY STRUCTURAL REPAIR MANUAL AND THE AIRCRAFT WAS RETURNED TO SERVICE.
