ON THE MORNING OF MARCH 9TH 2022, LEAR 55 N277AL DEPARTED KMAF. ON CLIMB APPROACHING 10,000 FT, THE LH ENGINE FIRE LIGHT FLASHED. THE CAPTAIN SCANNED THE ALL OF THE ENGINE INSTRUMENTS AND NOTICED THE LH ENGINE EGT WAS APPROACHING 930 DEGREES. THE CAPTAIN REDUCED POWER AND CONTACTED ATC TO REQUEST A RETURN TO MAF. AFTER ANOTHER SCAN OF ENGINE INSTRUMENTS, THE EGT HAD DROPPED BUT THEN THE FIRE LIGHT FLASHED AGAIN. HE THEN PULLED THE FUEL SHUT OFF AND SHUT THE LH ENGINE DOWN THEN DECLARED AN EMERGENCY. THE AIRCRAFT LANDED SAFELY AT MAF WITH NO OTHER ISSUES. THE CREW INSPECTED THE LH ENGINE COWLING AND PYLON THERE WAS NO SIGN OF FIRE OR OVER TEMP. THE AIRCRAFT WILL REMAIN AT MAF UNTIL AN ATI MAINTENANCE CREW CAN BE DISPATCHED FOR THE R&R OF THE LH ENGINE.
IN CRUISE FLIGHT IN ROUTE TO KSGR THE CREW ON FLIGHT CYO277 NOTICED THE RT STALL WARNING INDICATOR FLUCTUATING. NO CROSSWINDS NOTICED AT TIME. THE LT INDICATOR WAS STABLE. THE CAPTAIN ELECTED TO RETURN TO ELP. HE TURNED THE RT STALL WARNING OFF FOR THE RETURN. THE AIRCRAFT RETURNED TO ELP WITH NO OTHER ISSUES. A COMPLETE STALL WARNING FUNCTIONAL CHECK WAS COMPLETED IAW THE MM. NO DISCREPANCIES WERE NOTED. THE RT STALL WARNING INDICATOR WAS REPLACED WITH A SERVICEABLE UNIT FOR TROUBLE SHOOTING PURPOSES AND THE FUNCTIONAL TEST WAS COMPLETED AGAIN WITH NO DISCREPANCIES NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON 4-4-2018, DEPARTED KADS HEADING TO KSAV. APPROXIMATELY 15 MINUTES INTO THE FLIGHT, THE LEFT ESSENTIAL A BUS CIRCUIT BREAKER POPPED. THE CAPTAIN ATTEMPTED TO RESET THE BREAKER ONE TIME. IT WOULD NOT RESET. THE CAPTAIN ELECTED TO DIVERT INTO KGGG. AFTER SHUTTING THE AIRCRAFT DOWN TO CONTACT MAINTENANCE, ALL BREAKERS ON THE ESS A BUS WERE PULLED TO ISOLATE ANY ELECTRICAL ISSUES THAT MAY HAVE CAUSED THE ESS A BREAKER TO POP. THE ESS A BREAKER WAS RESET AND STAYED IN. ALL PULLED BREAKERS WERE RESET ONE AT A TIME TO TROUBLE SHOOT THE A BUS. ALL RESET NORMALLY. THE CAPTAIN RAN SYSTEMS CHECKS WITH THE AIRCRAFT GENERATORS LOADED TO TRY TO DUPLICATE THE DISCREPANCY. NO ELECTRICAL ISSUES WERE ENCOUNTERED. THE AIRCRAFT WAS FLOWN BACK TO ITS HOME BASE (KELP)FOR FURTHER EVALUATION. ALL INDICATIONS WERE FUNCTIONING NORMALLY WITH NO DISCREPANCIES NOTED DURING THE FLIGHT INTO ELP. PRIOR TO THE FLIGHT THE BREAKER POPPED, THE AIRCRAFT HAD FLOWN TWO LEGS TOTALING 2.5 HRS WITH NO ISSUES REPORTED. ALL ESS A CIRCUIT BREAKERS WILL BE REPLACED AS A PRECAUTION BEFORE FURTHER FLIGHT OF THE AIRCRAFT.
DEPARTING, A DUCT OVERHEAT LIGHT CAME ON & NOTICED A SLIGHT BURNT SMELL. THE CAPTAIN DECIDED TO RETURN. THE LIGHT STAYED ON DURING THE RETURN, LANDING & TAXI INTO THE RAMP. THE PRESSURIZATION & AIR CONDITIONING SYS WERE INSPECTED. A CONNECTOR TO THE CLIMATE CONTROL BOX WAS FOUND TO BE LOOSE & MAKING THE DUCT OVERHEAT LIGHT COME ON & OFF WHEN MOVED. THE CONNECTOR WAS TIGHTENED & THE ACFT WAS RUN UP ON THE GND, NO FURTHER ISSUES WERE NOTICED. A TEST FLIGHT WAS ALSO COMPLETED WITH NO FURTHER DISCREPANCIES.
EN ROUTE, THE CAPTAIN EXPERIENCED THE RT & LT ADIS FAILING WITH FLAGS & NO USE FULL INFORMATION. AN EMER WAS DECLARED & THE CAPTAIN STARTED DESCENDING. DESCENDING THROUGH 4000FT, ALL NORMAL ADI INDICATIONS & FUNCTIONS RETURNED. THE ACFT SAFELY LANDED. AVIONICS SYS WERE CHECKED, NO DISCREPANCIES WERE NOTED. R & R THE PRIMARY & SECONDARY A/C INVERTERS AS WELL AS THE PARALLELING CONTROL BOX WITH SERVICEABLE UNITS. AGAIN ALL AVIONICS & ACFT SYSTEMS WERE TESTED ON THE GND WITH NO DISCREPANCIES NOTED.
AFTER TAKEOFF FROM KELP, CLIMBING THROUGH FL 250, THE CAPTAIN NOTICED THE RT ENG INSTRUMENTS FLUCTUATING. HE SLIGHTLY REDUCED POWER TO SEE IF THE FLUCTUATIONS FOLLOWED & THEN WENT TO IDLE. HE THEN NOTICED THE OIL LIGHT CAME ON & THERE WAS NO RESPONSE TO POWER LEVER ADVANCE. DECLARED AN EMERGENCY & REQUESTED TO RETURN TO ELP, THE ACFT RETURNED SAFELY. FOLLOWING TROUBLE SHOOTING PROCEDURES IN THE ENG MANUAL, FUEL PRESS AT THE ENG DRIVEN FUEL PUMP WAS FOUND TO BE FLUCTUATING FROM 70-240 PSI. THE FUEL PUMP WAS REPLACED & PRESS CHECKED AGAIN. ALL INDICATIONS WERE AS THE MANUAL STATED FOR NORMAL OPERATION. A TEST FLIGHT WAS COMPLETED WITH NO ISSUES REPORTED.
AT LEVEL FLIGHT 380, THE CABIN ALTITUDE STARTED TO RISE AND THE EMERGENCY PRESSURIZATION WAS ACTIVATED, CREW ASKED FOR DESCENT TO A LOWER ALTITUDE. DESCENDING UNDER 30,000 FT CABIN PRESSURE STABILIZED, THE ACFT DIVERTED AND LANDED WITHOUT INCIDENT. THE ACFT WAS FLOWN BACK TO DEPARTURE AT 25,000 FT FOR MX, THE CABIN ALTITUDE LIMITER WAS FOUND TO HAVE DUST ACCUMULATION AROUND THE PRESSURE INLET SCREEN CAUSING IT TO SENSE A FALSE ALTITUDE. THE CABIN ALTITUDE LIMITER SCREEN WAS CLEANED AND THE PRESSURIZATION SYS TESTED ON THE GROUND, NO DISCEREPANCIES NOTED.
AFTER LANDING, MIDLAND TX, APPROX 13:49 PM TO DROP OFF PAX`S, SCHEDULED TO PROCEED TO DALLAS TO DROP OFF REMAINING PAX`S, THE PILOT EXPERIENCED A VIBRATION DURING TAXI. INSPECTION OF A/C REVEALED RT OUTBOARD MAIN TIRE HAD A BLOW OUT DUE TO A POSSIBLE LOCKUP OF THE WHEEL. A SERVICEABLE WHEEL AND TIRE WAS INSTALLED ON THE A/C AND A/C WAS FLOWN TO EL PASO. ANOTHER A/C WAS DISPATCHED TO MIDLAND TO TRANSPORT PAX`S TO FINAL DESTINATION TO ADDISON AIRPORT IN DALLAS TX. UPON ARRIVAL AT ELP, A/C WAS PLACED ON JACKS AND RT WHEEL ANTI-SKID SYSTEM WAS NOT FUNCTIONING PROPERLY. A BROKEN WIRE WAS DISCOVERED ON ANTI-SKID WIRING HARNESS LOCATED IN THE WHEEL WELL. THE WIRE WAS REPAIRED AND SYSTEM CHECKED OK.
SHORTLY AFTER TAKEOFF THE RIGHT ENGINE FIRE WARNING ILLUMINATED. POWER WAS REDUCED TO THE RIGHT ENGINE AND THE WARNING LIGHT EXTINGUISHED. A PRECAUTIONARY LANDING WAS MADE AT THE DEPARTURE AIRPORT. INVESTIGATION REVEALED THE RIGHT GENERATOR EXPERIENCED A CATASTROPHIC FAILURE. THE FIRE SENSING ELEMENT IS ROUTED OVER THE GENERATOR AND WAS THE CAUSE IF THE FIRE INDICATION. PRELIMINARY INSPECTION REVEALED POSSIBLE BEARING FAILURE WITHIN THE GENERATOR.
DURING TROUBLESHOOTING, A TRANSPONDER IDENTS ALL THE TIME. AVIONICS TECH DETERMINED THERE WAS A SHORT IN CANON PLUG RP738. WHEN PLUG WAS DISASSEMBLED, HE FOUND THE WIRES AND PINS WERE IMPROPERLY INSTALLED WITH WIRE INSULATION CUT BACK TOO FAR LEAVING BARE WIRES INSIDE THE PLUG. SOME WIRES WERE TOUCHING EACH OTHER. A BRIEF LOG SERCH WAS CONDUCTED, BUT NO WORK IN THIS AREA IN THE RECENT PAST WAS DISCOVERED.
