Part: PUMP — FAILED (ENGINE FUEL) · airframe 4,609 hrs
DURING TAKEOFF, THE ENGINE MOMENTARILY LOST POWER WHEN FUEL BOOST PUMP WAS SHUT OFF. FUEL FLOW REDUCED BY 8 GPH. AFTER A COUPLE OF SECONDS, THE FUEL FLOW RETURNED TO NORMAL AND THE ENGINE RECOVERED POWER. THE AIRCRAFT LANDED WITHOUT INCIDENT. GROUND TESTING REVEALED THE ENGINE DRIVEN PUMP WAS SUPPLYING PROPER PRESSURE DURING ALL OPERATIONS EXCEPT FOR FULL POWER AFTER ELECTRIC PUMP SHUT DOWN. INTERUPTION OF FUEL FLOW WAS ONLY MOMENTARY AND DID NOT OCCUR AT LOWER POWER SETTINGS. ENGINE DRIVEN PUMP WAS REPLACED AND ENGINE OPERATES NORMALLY. (X)
Part: CONTROL CABLE — FRAYED (ELEVATOR TRIM)
DURING ROUTINE INSPECTION, THE FORWARD TRIM CABLE WAS FOUND FRAYED IN THE AREA THAT PASSES THROUGH THE ELECTRIC TRIM SERVO. PIPER SB NR 669 HAD BEEN C/W BY INSTALLATION OF PIPER ELECTRIC TRIM PULLEY ALIGNMENT KIT P/N 763-971 DURING THE LIFE OF THE CABLE. A CLOSE VISUAL INSPECTION OF THE TRIM SERVO REVEALED THAT INSTALLATION OF THE KIT DOES NOT PROPERLY ALIGN THE CABLE. THE SERVO MOUNT IS INCORRECTLY DESIGNED CAUSING THE ENTIRE SERVO TO BE OUT OF ALIGNMENT WITH THE CABLE. A NEW MOUNT SHOULD BE DESIGNED THAT ALIGNS THE SERVO.
Part: BULKHEAD — CRACKED (SPINNER)
DURING ROUTINE INSPECTION, NOTED A CRACK EXTENDING FROM THE TRAILING EDGE OF THE FORWARD SPINNER BULKHEAD TO ONE OF THE NUT PLATES AND CONTINUING ANOTHER .50 INCH FORWARD. THIS IS THE SECOND BULKHEAD INSTALLED ON THIS AIRCRAFT IN LESS THAN 8 HOURS DUE TO THIS SAME PROBLEM. THE MOUNTING OF THE BULKHEAD AND SPINNER HAS BEEN CHECKED SEVERAL TIMES FOR PROPER FIT AND ALIGNMENT AND CORRECT TORQUE ON THE SPINNER MOUNTING SCREWS. TWO OTHER AIRCRAFT IN THE SAME FLEET HAVE ENCOUNTERED SIMILAR FAILURE TIMES.
Part: TRIM PULLEY — SPLIT (TAIL CONE)
DURING 100-HOUR INSPECTION, FOUND TRIM CABLE TENSION AT ZERO. RECORDS REVEALED TRIM CABLES HAD BEEN ADJUSTED TO PROPER TENSION AT THE PREVIOUS 100-HOUR INSPECTION. FOUND THE 1.25 INCH DIAMETER PULLEY ON THE RIGHT SIDE IN THE TOP OF THE TAIL CONE JUST BELOW THE DORSEL FIN, WAS SPLIT IN TWO. SUBMITTER SUGGESTS THERE APPEARS TO BE SIGNS OF DELAMINATIONS SOAKED WITH LUBRICANTS.
