THE FUSELAGE SKIN WAS FOUND WITH 2 CRACKS ALONG THE BONDED DOUBLER EDGES AT STRINGERS 22R & 23R. AN EXISTING REPAIR FOR A SIMILAR CRACK WAS LOCATED AT STRINGER 24R. THE EXISTING REPAIR WAS REMOVED, ALL CRACKS REMOVED WITH CUTOUTS, AND THE CUTOUTS REPAIRED WITH A SINGLE ALUMINUM DOUBLER/TRIPLER REPAIR. THE REPAIR WAS ACCOMPLISHED PER ER/A 089558-14, REV A.
EMERGENCY LIGHTS INOP AT SEAT 2B OVERHEAD, AT 10D AND 14D. REPLACED OVERHEAD LIGHT ASSY AT 2B AND RELAMPED SEAT LIGHTS AT 10D & 14D CHECKS GOOD.
ON L/O FOUND EMERGENCY OVERHEAD PATH LIGHTING AT SEATS 23ABC AND 17D INOP. RELAMPED SAME, OPS CHECK NORMAL.
WHEN CATERING ATTEMPTED TO OPEN THE LEFT REAR SERVICE DOOR . IT WOULD NOT OPEN. REPLACED BROKEN ARM ASSEMBLY
CO-PILOTS FRONT WINDSHIELD POWER WIRE IN UPPER RT CORNER ARCED. C/BS PULLED TO REMOVE POWER. REPLACED TERMINAL END AND HARDWARE. REPAIRED SAME.
CLIMBING THROUGH FL240, NR 1 ROLLED BACK TO 0 INDICATIONS AFTER LOUD BANG. ENG FAIL CK LIST WAS COMPLETED. METAL IN TAIL PIPE, ENG CHANGED.
AT CRUISE ALTITUDE L-1 DOOR HANDLE POPPED UP APPROX .5 INCH. FOUND DOOR STOP PINS AND GATE UPPER AND LOWER RODS OUT OF ADJUSTMENT. RIGGED BOTH PER MM, PRESSURIZED ACFT TO 7.0 PSID, WITH CABIN PRESSURE ON UNABLE TO MOVE HANDLE.
AT FS 795 AT THE LWR FWD CORNER OF THE C-2 DOOR, A 1 INCH CRACK WAS FOUND IN THE BEARSTRAP. IN ADDITION AN EXISTING SKIN REPAIR NEAR STR 23R. THE EXISTING REPAIR IS FOR A CUTOUT IN THE SKIN AND BEARSTRAP. THE BEARSTRAP CRACK WAS GROWING FROM EXISTING EDGE DAMAGE AND WAS TRIMMED OUT. REPAIRED PER ER/A 412245-14 REV ORIGINAL.
FORWARD SPAR DAMAGED AT THE CUTOUT FOR THE EXHAUST GATE ACTUATOR ROD. DAMAGE CAUSED BY INSTALLATION OF INCORRECT ARM THAT MADE THE ROD TOO HIGH AND BOLT ATTACHING ROD TO ARM CONTACT THE SPAR.
THE NR 3 MID FLAP FRONT SPAR UPPER CHORD AND WEB WERE FOUND DAMAGED. REPAIRED PER ER/A 410644-14.
INSPECTION OF THE BL 17.1 AND BL 5.7 VERTICAL BEAMS OF THE AFT SIDE OF THE FS 178 BULKHEAD PER BAC S/B 737-53A1225. REVEALED A CRACK IN THE RBL 5.7 BEAM WEB AT WL 185 AND IN THE LBL 17.1 BEAM WEB AT WL 230. THE WEBS OF ALL 4 BEAMS WERE REMOVED AND NEW WEBS INSTALLED TO REPAIR/MODIFY THE BEAMS PER ER/A 381260-14, REV A.
FOUND SEAT EMERGENCY LIGHT MISSING AT 16D. REPLACED MISSING LIGHT AND CABLE ASSY.
ELECTRIC STAB TRIM INOP BOTH MAIN AND AUTOPILOT. TRIM INITIALLY WORKED. PASSING 13,000 FEET TRIM QUIT WORKING. A/C AIR TURNBACK. REPLACED STAB TRIM ACTUATOR PER NR CARD NR 1 REF M/M.
ON GEAR EXTENSION NOSE GEAR RED UNSAFE LIGHT REMAINED ILLUMINATED AND GREEN GEAR DOWN LIGHT DIDN''T ILLUMINATE. C/W POM AND VISUAL INSPECTION. RED LIGHT REMAINED ILLUMINATED. FOUND NOSE GEAR TRUNNION TARGET LOOSE, BENT AND OUT OF ADJUSTMENT. REPLACED TARGET. JACKED AIRCRAFT AND SWUNG GEAR. OPERATION NORMAL.
AFTER T/O FROM SLC, WITH THE FLAPS INTRANSIT FROM 5 DEG TO 1 DEG, THE FLAP GAUGE SHOWED A SPLIT WITH RT FLAP SHOWING 2 DEG AND THE LT FLAP SHOWING 1.5 DEG. THIS CAUSED AN ASYMMETRICAL TRIP. AFTER TALKING WITH ATL MTC AND HAVING THE GAUGE FAIRED WE LANDED NML IN SLC. REPLACED INDICATOR. CHECKED FLAPS FOR CONDITION AND OPERATION. FOUND OK. OPERATED FLAPS SEVERAL TIMES.
PASSENGERS AT ROW 17 AND 18 ABC REPORTED A BREIF PUFF OF SMOKE AROUND WINDOW AREA AND ASSOCIATED SMELL. ODOR WAS A MILD ELECTRIC SMELL AND DISSIPATED. WINDOW LIGHTS TURNED OFF AS PRECAUTION. REPLACED SIDEWALL LIGHT BALLAST.
NOSE GEAR WOULD NOT EXTEND WITH NORMAL GEAR EXTENSION. MANUAL GEAR EXTENSION WAS REQUIRED TO GET A SAFE NOSE GEAR INDICATION. FOUND SHIM MISSING ON OVER CTR LINKAGE. INSTALLED SAME, ALL OPS CKS OK. RETRACTION CK NORMAL.
DURING INSPECTION, FOUR CRACKS WERE FOUND IN THE BLKHD WEB AT BS 178. CRACKS WERE LOCATED BETWEEN WL 180 AND WL 210 AND BETWEEN RBL 5 AND LBL 5. REPAIRED PER ERA 331749-14.
AT FLT LEVEL 230 WITH 'B' A/P ENGAGED AN UNCOMMANDED RUDDER INPUT OCCURED, JUST PRIOR THE MASTER CAUTION LIGHT WAS ILLUMINATED BY THE MACH TRIM FAIL LIGHT. YAW DAMPER INDICATOR ON FWD PANEL SHOWED FULL RT DEFLECTION. YAW DAMPER WAS TURNED OFF, MACH TRIM FAIL LIGHT EXTINGUISHED, THEN CAME BACK ON MOMENTARLY THEN REMAINED EXTINGUISHED. RECOVERY FROM UNCOMMANDED RUDDER INPUT REQUIRED ABOUT .5 TO .75 LT RUDDER DEFLECTION. RETURNED TO DFW WITH NO FUTHER UNCOMMANED RUDDER INPUT. INVESTIGATION FOUND MECHANICAL FAILURE OF RATE GYRO IN THE YAW DAMPER COUPLER.
DURING CRUISE LOST B-SYS HYDRAULICS, COULD NOT LOWER T/E FLAPS. FOUND NR 3 SLAT ACTUATOR LEAKING, HOUSING WAS CRACKED AT PARTING SURFACE. REPLACED ACTUATOR, OPERATION NORMAL.
