CENTER ELEVATOR HINGE ASSEMBLY P/N 101-620013-1 FOUND WITH EXCESSIVE CORROSION / MISSING MATERIAL CREATING A 1.5 X 0.5 INCH HOLE. OUTBOARD ELEVATOR HINGE ASSEMBLY P/N 101-620011-1 ALSO FOUND WITH EXCESSIVE CORROSION / MISSING MATERIAL.
THE FLIGHT CREW REPORTED THAT THE LANDING GEAR DOWN AND LOCKED GREEN INDICATION LIGHT DID NOT ILLUMINATE WHEN THE GEAR WAS SELECTED TO THE DOWN POSITION. OUT OF PRECAUTION, THE CREW DECIDED TO REPORT TO ATC THE DISCREPANT ITEM AND OPTED TO MANUALLY EXTEND THE LANDING GEAR BY ACTUATING THE EMERGENCY LANDING GEAR HANDLE. THE AIRCRAFT LANDED SAFELY AND WAS FERRIED TO DFW FOR THE REPAIR. DFW MAINTENANCE INSPECTED THE LANDING GEAR SYSTEM AND FOUND THAT THE MOLEX CONNECTOR TO THE INDICATOR HAD A BROKEN WIRE CONNECTION. THE WIRE WAS REPAIRED, AND MAINTENANCE PERFORMED TEN EXTENSION AND RETRACTION CHECKS ON AIRCRAFT JACKS. ALL LANDING GEAR SWINGS WERE SUCCESSFUL. THE PILOT SQUAWK WAS CLOSED OUT AND MAINTENANCE IS WORKING OTHER SCHEDULED ITEMS AT THIS TIME.
THE FLIGHT CREW REPORTED EXHAUST ODOR GETTING PUMPED INTO THE COCKPIT WHEN THE LEFT BLEED IS OPENED AT 400 FEET AGL. MAINTENANCE PERFORMED GROUND RUN CHECK OF THE LEFT BLEED AIR SYSTEM AND COULD NOT DUPLICATE THE DISCREPANCY. MAINTENANCE SUSPECTED THE INNER WALL OF THE BREATHER HOSE WAS COLLAPSING WITH THE ENGINE RUNNING. REMOVED AND REPLACED LH ENGINE BREATHER SYSTEM HOSE WITH NEW. MAINTENANCE REMOVED AND CLEANED THE FUEL SYSTEM PURGE TANK AND TESTED THE CHECK VALVES, REINSTALLED PURGE TANK. AIRCRAFT WAS CHECK FLOWN WITH THE BLEED AIR SYSTEM ON NO EXHAUST ODOR WAS PRESENT, THE CHECK FLIGHT WAS SATISFACTORY.
SMOKE IN COCKPIT. TURNED OFF BOTH BLEEDS. SMOKE FILTERED OUT. RIGHT BLEED CAUSED NO MORE SMOKE. LEFT BLEED BROUGHT SMOKE AGAIN. FINISHED WITH RIGHT BLEED ONLY. THE AIRCRAFT LANDED WITHOUT INCIDENT IN KOMA. OMA MX- CHECKED GAS GEN. BREATHER TUBE FOR OBSTRUCTIONS. NO DEFECTS NOTED. CHECKED START GEN. CARBON SEAL FOR LEAKS & NONE NOTED. CHECKED SCAVENGE OIL STRAINER PER P&W PT6A-65B FOUND TO BE CLEAN. CHECKED OIL FILTER & CHECK VALVE PER P&W PT6A-65B NO DEFECTS NOTED. MX DID NOTE OIL LEAKING FROM PROP GOVERNOR GASKET & OIL FROM LOW PRESSURE FUEL PUMP DRIVE LEAKING FROM BROKEN DRAIN LINE HOSE INTO BYPASS & PRECOOL VALVES AND ASSOCIATED DUCTS IN LOWER NACELLE AREA. ORDERED REQ. PARTS PER REQ. #180767.
AT 400 FEET AFTER TAKE OFF - EXHAUST SMELL ENTERED CABIN THROUGH FLOOR VENTS. TURNED BLEEDS OFF SMOKE STOPPED. AFTER ONE MORE ATTEMPT AND OFF SMOKE STOPPED. SMOKE DID NOT CONTINUE ON THE THIRD TIME TO TURN ON BLEEDS. COMPLETED THE RUN WITH NO DELAY. MAINTENANCE OPERATED ENGINES AT MULTIPLE POWER SETTINGS AND WITH LEFT BLEED ONLY, RIGHT BLEED ONLY, AND BOTH BLEEDS TOGETHER IN BOTH MANUAL AND AUTO TEMP CONTROL SETTINGS. RAN ENGINES FOR 20 MINUTES. NO DEFECTS NOTED. OAT -20*C
DURING ILS APPROACH THE LEFT RMI WENT INOPERATIVE FOLLOWED BY THE NUMBER TWO HSI TURNING 90 DEGREES AND PILOT SIDE ATTITUDE INDICATOR SHOWING A 15-30 DEGREE ROLL. EXECUTED A MISSED APPROACH AND INFORMED ATC OF THE ISSUE. ATC DIRECTED HIM TO HIS ALTERNATE KSJT. DECLARED AN EMERGENCY AND WAS GIVEN AN AIRPORT SURVEILLANCE RADAR APPROACH INTO KSJT WHERE HE LANDED WITHOUT INCIDENT. MX T/S AND RESET THE NR 1 BUSS CIRCUIT BREAKER AND THE PILOTS HSI SYSTEM WAS OPERATIONAL, DEFERRED THE NUMBER TWO HSI SYSTEM. MX DISCOVERED THE SERVO AMPLIFIER WAS SHORTED INTERNALLY. REMOVED AND INSTALLED THE SERVO AMPLIFIER P\N 522-1400-000 WITH SERVICEABLE UNIT. OPS CHECKS ON GROUND WERE SATISFACTORY WITH NO SUBSEQUENT ISSUES REPORTED.
