FLIGHT RETURNING TO BASE AFTER FREIGHT CHARTER FLIGHT. UPON APPROACH PILOT SELECTED GEAR DOWN BUT DID NOT RECEIVE A GREEN LIGHT INDICATION FOR THE NOSE GEAR. PILOT RECYCLED GEAR AND REQUESTED TO TERMINATE LANDING. PILOT MANUAL EXTENDED THE LANDING GEAR IAW AFM PROCEDURES. NO INDICATION OF HYDRAULIC OR ELECTRICAL FAILURE. RELEASING MANUAL GEAR RELEASE EXTENDED AND LOCKED NLG. PILOT LANDED AND TAXIED TO BASE WITHOUT ANY ADDITIONAL INDICATIONS OF PROBLEMS. AIRCRAFT HAS NOT BEEN RETURNED TO SERVICE BUT SUSPECT PART IS THE LANDING GEAR SERVO IN THE HYDRAULIC POWER PACK ASSEMBLY . MAINTENANCE BELIEVES THAT THE SERVO IS NOT CLOSING COMPLETELY AND THEREFORE DIDN'T DEVELOP ENOUGH INITIAL PRESSURE TO RELEASE THE MECHANICAL UP LATCH DURING THE INITIAL GEAR DOWN OPERATION. MAINTENANCE DETERMINED THAT THE VALVE ASSY GEAR WHICH IS PART OF THE POWER PACK MAY HAVE BEEN NOT FULLY CLOSING CAUSING A BYPASS CONDITION. THIS PREVENTED THE GEAR DOWN SYSTEM PRESSURE TO BE LESS THAN NORMAL.

INSPECTED LT AND RT ACMS, WITH NO DEFECTS NOTED. SERVICED BOTH ACMS AND REMOVED, CLEANED, REINSTALLED WATER SOCKS. RAN ENG SECOND TIME WITH NO DEFECTS NOTED. COULD NOT DUPL ORIGINAL WRITE-UP. MRR-M04-18, DURING FLIGHT THE LT FIRE EXTINGUISHER (E) LIGHT CAME ON BRIEFLY SEVERAL TIMES. THE CREW MONITORED THE LIGHT AND THE FLIGHT WAS CONTINUED TO THE DESTINATION WITH NO INCIDENT. FIRE EXTINGUISHER (E) LIGHT WAS FOUND ON, AND THE FIRE EXTINGUISHER BOTTLE WAS FOUND EMPTY DURING TROUBLESHOOTING BY MAINTENANCE. THE SQUIB HAD BEEN FIRED. REPLACED FIRE EXTINGUISHER BOTTLE AND SQUIB HAD BEEN FIRED. REPLACED FIRE EXTINGUISHER BOTTLE AND SQUIB. FIRE BOTTLE PN: 3030020, SN 05782B1 OH 7-14-04, INSTALLED 7-19-04. NO DEFECTS WERE NOTED DURING TROUBLESHOOTING OR PARTS CHANGES. PRESS TO TEST FUNCTION AND ALL OTHER FUNCTION CHECKS WERE GOOD. (K)

UPON LANDING ROLLOUT AT SLU, A/C BEGAN DRIFTING TO THE RT, WITH THE NOSE WHEEL STEERING ARMED. THE PILOT MAINTAINED THE RUNWAY CENTERLINE WITH THE RUDDER BUT AS THE A/C SLOWED, THE RUDDER BECAME LESS EFFECTIVE AND NOSEWHEEL STEERING STILL DID NOT RESPOND. PILOT APPLIED DIFFERENTIAL POWER AND BRAKES BUT THEIR EFFECTIVENESS WAS REDUCED DUE TO WET RUNWAY. PILOT APPLIED FULL REVERSE TO AVOID IMPACTING THE RUNWAY LIGHTS AND FENCE. THE A/C CAME TO A FULL STOP APPROX 4 FT OFF RUNWAY. MX DID VISUAL INSPECT OF L/G, WHEELS, BRAKES, AND PROPELLERS AND NO DAMAGE TO A/C FOUND. MX DEFERRED NOSE WHEEL STEERING AND A/C WAS FERRIED TO SJU FOR FURTHER MX. MX AT SJU PERFORMED GEAR SWING RETRACTION AND THE OPS CHECK WAS GOO

WHILE EN ROUTE FROM SJU TO BGI, THE AIRCRAFT EXPERIENCED COMPLETE HYDRAULIC FAILURE ON THE LEFT ENGINE. BOTH HYDRAULIC LIGHTS ILLUMINATED AND THE HYDRAULIC GAUGE INDICATED ZERO PRESSURE. AFTER ACCOMPLISHING EMERGENCY GEAR EXTENSION PROCEDURE, THE AIRCRAFT LANDED AT BGI WITH NO FLAPS, AND NO NWS. MX FOUND A BROKEN HYDRAULIC LINE AND REPLACED IT WITH A TEMPORARY FLEX LINE. OPERATIONAL CHECK AND LEAK CHECK WERE SATISFACTORY. AIRCRAFT WAS RETURNED TO SERVICE AND A MX ACTION REQUEST WAS INITIATED TO TRACK TIME FOR THE TEMPORARY FLEX LINE. UPON RETURN TO THE MX BASE AT SJU, MX REPLACED THE FLEX LINE WITH A HARD LINE. OPERATIONS CHECK AND LEAK CHECK WERE GOOD AND AIRCRAFT WAS RETURNED TO SERVICE. (M)

THE AIRCRAFT LANDED AT SJU WITHOUT FURTHER INCIDENT. MAINTENANCE REMOVED AND REPLACED BOTH THE LEFT AND RIGHT RETRACTION ACTUATORS WITH OVERHAULED UNITS. ON TAKEOFF ROLL OUT OF SJU, THE LEFT ENGINE EGT AND TORQUE INDICATIONS BECAME ERRATIC. THE PILOT ABORTED THE TAKEOFF AND ASKED FOR CLEARANCE TO A SECLUDED AREA TO PERFORM AN ENG RUNUP CHECK. DURING THE RUNUP CHECK THE PILOT TURNED OFF THE LT SRL AND THE LT ENGINE EGT AND TQ INDICATIONS STABILIZED. THE PILOT RETURNED TO MX WITH THE A/C. MX DEFERRED THE LT SRL SYS PER MEL 77-2 AND PERFORMED A GROUND RUNUP CHECK TO VERIFY THE SYSTEM OPERATED NORM WITH THE SRL DEFERRED. THE A/C WAS RELEASED FOR SERV. MX CLEARED THE DMI AFTER PERFORMING 6 TAKEOFF POWER CHECKS WITHOUT ANY PROBLEMS. THE PROBLEM HAS NOT RETURNED. (M)
