BOLT, P/N NAS464P4, SHEARED CAUSING SIDE LOAD ON THE BELLCRANK BREAKING THE ATTACH POINT AT THE GEAR LIGHT SAFETY SWITCH. (X)
DURING A ROUTINE INSPECTION, THE LT WING TIP FUEL TANK ATTACH BULKHEAD WAS FOUND CRACKED. THE CRACK WAS LOCATED TWO INCHES OUTBOARD OF THE WING ATTACH BOLT. SUBMITTER STATED THIS CONDITION GOING UNREPAIRED, WOULD RESULT IN LOSS OF WING TIP FUEL TANK AND LOSS OF CONTROL OF AIRCRAFT.
DURING A ROUTINE INSPECTION, THE LEFT LANDING GEAR RETRACT LINKAGE HAD EXCESSIVE PLAY. INVESTIGATION REVEALED THE LEFT MAIN GEAR TORQUE TUBE FORK BOLT HOLE WAS SEVERELY ELONGATED. FORK BOLT IS COVERED UNDER AD 76-13-07 WHICH REQUIRES REPLACEMENT AT 5,000 HOUR INTERVALS. THIS PART HAD BEEN REPLACED 1,650 HOURS PREVIOUSLY. SUSPECT ATTACH HOLE DRILLED OVERSIZE DURING MANUFACTURE. IMPROPER HEAT TREATMENT. SUBMITTER SUGGESTS MORE FREQUENT REPLACEMENT AND INSPECTION INTERVALS.
DURING A ROUTINE INSPECTION, NOTICED THE RIGHT MAIN WING TIP FUEL TANK WAS LEAKING. FURTHER INVESTIGATION REVEALED THE AFT BULKHEAD AND AFT WING ATTACH BULKHEAD WERE CRACKED. SUBMITTER STATED IF CONDITION WOULD HAVE GONE UNNOTICED, IT WOULD POSSIBLY RESULT IN WING TIP FUEL TANK DEPARTING THE AIRCRAFT. SUBMITTER SUSPECTS CAUSE AS FATIGUE, VIBRATION. SUBMITTER SUGGESTS PREVENTION - INSPECT AREA CLOSE DURING INSPECTION INTERVALS AND IMMEDIATELY IF FUEL STAINS NOTICED.
