PILOT REPORTED SMOKE SMELL IN CABIN 2 MIN AFTER DEPARTING HUNTSVILLE HOSPITAL WHILE RETURNING TO BASE. AFTER LANDING DISCUSSION WITH PILOT, THEY THOUGHT IT WAS RELATED TO AIR CONDITIONING SYSTEM.
MAIN ROTOR MAST FOUND TO HAVE CORROSION INTERNALLY DURING 12 MONTH INSPECTION. REMOVED MAIN ROTOR MAST AND REPLACED WITH SERVICEABLE.
UPON INSPECTING AIR CONDITIONING DRIVE PULLEY AND SPLINE IN ACCORDANCE WITH AIR COMM SB206EC-18-07 AND AIR EVAC MIL 356. IT WAS DISCOVERED A/C DRIVE PULLEY TO BE WORN BEYOND LIMITS. DRIVE PULLEY WAS REPLACED AND INSTALLED IN ACCORDANCE WITH MIL 356 AND SB206EC-18-07. NO DEFECTS NOTED.
TAIL BOOM RING/BULKHEAD ASSY PN: 206-032-400-027 HAS APPROXIMATELY ONE 1/2 - 3/4 INCH CRACK AROUND THE UPPER LEFT SIDE OF THE UPPER LEFT INTERCOSTAL/MOUNT POINT.
DFW - FLT 3754 - CARGO SMOKE LIGHT CAME ON IN FLIGHT. CREW DISCHARGED NR 1 CARGO EXIT SWITCH AND THE CARGO SMOKE LIGHT WENT OFF AFTER APROX 8 MINUTES. DFW, MAINTENANCE RAI BOTH CARGO SMOKE DETECTORS AND THE RT CARGO C02 BOTTLE. NO EVIDENCE OF SMOKE OR FIRE REPORTED FROM MTX. A/C WAS RETURNED TO SERVICE. (M)
DFW - FLT 3706 - ADC C/B F13 POPPED AND POPPED AGAIN. A STRONG SCENT OF SMOKE IN COCKPIT WAS NOTICED. RAI ADC, OPS CHECK GOOD. A/C RETURNED TO SERVICE. (M)

FSM - FLT 3948 - DURING CRUISE FLT, RIGHT ENGINE PROP RPM FLUCTUATED AND RIGHT TQ FLUCTUATED. CREW SHUT DOWN ENGINE AND DIVERTED TO FSM. RAI'D RIGHT ENGINE NP SENSOR. OPS CHECK GOOD. AIRCRAFT WAS RETURNED TO SERVICE. (M)
LAW - DURING INSPECTION OF RT ENGINE, POWER GEARBOX (PGB) PROPELLER SHAFT FOLLOWING PROPELLER REMOVAL, THE SHAFT FLANGE SHOWED EVIDENCE OF FRETTING IN AREA INBOARD OF MOUNTING BOLT HOLES. THIS DAMAGE TRAVELED 360 DEGREES AROUND FLANGE AREA AND APPEAR TO BE APPROX .035 INCH DEEP (MAX). THE PROPELLER HUB WAS INSPECTED, REVEALING EVIDENCE OF DAMAGE. THE PGB, PN 775900-10, SN UDAG071S, WAS REMOVED FROM SERVICE AND SENT TO VENDOR FOR TEAR DOWN AND REPAIRS. PART TOTAL CYCLES, 12,493. (X)
DFW - FLT 3944 - AFTER TAKEOFF, CREW REPORTED THEY COULD NOT CONTROL COCKPIT AIR TEMP WHICH WAS BLOWING FULL HOT. REMOVED AND REPLACED LEFT ACM. AIRCRAFT RETURNED TO SERVICE. (X)
