AFTER TAKEOFF APU OIL PRESS INDICATION FOLLOWED BY AUTO SHUTDOWN. INVESTIGATION OF APU OIL FILTER AND OIL FOUND 6 BALL BEARINGS CAME OUT DURING DRAINING OF APU OIL. AN APU LOANER WAS INSTALLED, AND THE DEFECTIVE APU SENT OUT FOR REPAIR. FACILITY PERFORMING REPAIR FOUND DEFECTIVE STARTER CLUTCH SHAFT BEARING FAILED. IT WAS REPLACED USING AN UPGRADED PART PER S/B 49-6114.
PILOTS DISPLAY LOST ALL VSPEEDS DURING TAKEOF ROLL, TAKEOF WAS ABORTED. V-SPEEDS REAPPEARED UPON TAXI BACK TO RAMP. TROUBLESHOT SYSTEM USING PITOT/STATIC CART AT ALL AIRSPEEDS AND ALTITUDES, NOT DEFECTS NOTED. CREW PERFORMED 2 HIGH SPEED TAXI WITH NO DEFECTS NOTED. SWAPPED J#1 AND #2 ADC AND DISCREPANCY REAPPEARED ON CO-PILOT DISPLAY. REPLACED THE SUSPECT #2 ADC UNIT, AND OPS TESTED GOOD.
ON TAKEOFR ROLL, AT 50 KNOTS, PILOT STATED "#2 GEN FAIL" CAS MESSAGE DISPLAYED, AND TAKEOFF ABORTED. FINAL INVESTIGATION REVEALED #2 ENG INTEGRATED DRIVE GENERATOR (IDG) DELTA P INDICATOR WAS SET. REMOVAL AND INSPECTION OF FILTER REVEALED NUMEROUS METAL PARTICLES INSIDE FILTER. REPLACEMENT OF IDG WAS PERFORMED AND AIRCRAFT RETURNED TO SERVICE.
DURING LINE MAINTENANCE INSPECTION, FOUND THE TAIL COMPARTMENT OVERHEAD EMERGENCY LIGHTS INOPERATIVE. REPLACED BATTERIES, OPERATIONAL CHECK NORMAL.
AFTER TAKEOFF, THE RIGHT MAIN LANDING GEAR UNSAFE LIGHT, RIGHT MAIN GEAR DOOR OPEN LIGHT, AND AUTO SPOILER DO NOT USE LIGHT ALL REMAINED ILLUMINATED. THE FLIGHT CREW CYCLED THE LANDING GEAR, RETURNED TO MSP, AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED A BROKEN LINK ON THE RETRACT ACTUATOR OF THE RIGHT MAIN LANDING GEAR. OPERATIONAL CHECK NORMAL.
DURING LINE CHECK, FOUND BOTH AFT OVERWING EMERGENCY EXIT SIGNS INOPERATIVE. MAINTENANCE REPLACED BATTERIES, OPERATIONAL CHECK NORMAL.
DURING LINE CHECK, FOUND OVERHEAD EMERGENCY LIGHTS AT ROWS 20-24 WOULD NOT EXTINGUISH WITH AIRCRAFT BATTERY IN OFF POSITION. REPLACED POWER SUPPLY AND BATTERIES. OPERATIONAL CHECK GOOD.
DURING LINE CHECK, CEILING MOUNTED EMERGENCY LIGHTS AT ROWS 20-23 WOULD NOT EXTINGUISH WHEN SHIP'S BATTERY IS SELECTED OFF AND POWER IS REMOVED. REPLACED BATTERIES. OPERATIONAL CHECK NORMAL.
SLAT DISAGREE LIGHT ILLUMINATED ABOUT 200 KNOTS IN DESCENT WITH FLAP AND SLATS RETRACTED. AFTER EXTENDING SLATS IT CONTINUED TO FLICKER FOR A COUPLE OF MINUTES. ALSO, RECEIVED SLAT OVERSPEED CAWS WARNING. CHECKED PROXIMITY AND WARNING COMPUTER. TESTED ALL SLAT SENSORS AND FOUND OK. SYSTEM OPERATIONALLY CHECKED OK.
AFTER TAKEOFF FROM DEN, LANDING GEAR WOULD NOT RETRACT. FLIGHT RETURNED TO AIRPORT AND LANDED WITHOUT INCIDENT. SERVICED NOSE STRUT. OPERATIONAL CHECK OK.
DURING LINE CHECK, FOUND CEILING EMERGENCY LIGHTS IN FIRST CLASS INOPERATIVE. REPLACED POWER SUPPLY B5-24. OPERATIONAL CHECK OK.
ON DEPARTING, UNABLE TO PRESSURIZE CABIN IN AUTO OR MANUAL. FLIGHT RETURNED TO GEG AND LANDED WITHOUT INCIDENT. FOUND DUCT HOSE OFF BETWEEN AIR CYCLE MACHINE AND HEAT EXCHANGER. REINSTALLED DUCT, LEAK AND OPERATIONAL CHECK OK.
DURING LINE CHECK, FOUND EMERGENCY CEILING LIGHTS FROM ROW 1 TO ROW 15 INOPERATIVE. REPLACED BATTERIES AT CHARGER B5-32, OPERATIONAL CHECK GOOD.
DURING LINE CHECK, FOUND TAIL COMPARTMENT EMERGENCY LIGHTS INOPERATIVE. REPLACED BATTERY PACK AND OPERATIONAL CHECK GOOD.
DURING LINE MAINTENANCE INSPECTION, FOUND AFT TWO ROWS OF OVERHEAD EMERGENCY LIGHTS INOPERATIVE. REPLACED BATTERY PACK AND OPERATIONAL CHECK OK.
NR 2 ENGINE LOST POWER ON TAKEOFF. THE CREW CONTINUED THE TAKEOFF DUE TO HIGH AIRSPEED AND POOR RUNWAY CONDITIONS. AFTER LIFT OFF A COMPRESSOR STALL OCCURRED AND THE ENGINE RESUMED NORMAL OPERATION. THE AIRCRAFT COMPLETED ITS SCHEDULED FLIGHT WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED BOTH ENGINES UPON ARRIVAL. NO DISCREPANCIES WERE NOTED.

RIGHT HYDRAULIC SYSTEM QUANTITY WENT TO ZERO ON APPROACH, LANDED WITH MAIN GEAR DOORS OPEN. FOUND AUXILIARY PUMP PRESSURE LINE LEAKING INTERNALLY. REPLACED LINE AND SERVICED TO PROPER LEVEL. ALSO, REPLACED MAIN GEAR DOORS DUE TO DAMAGE INCURRED. AIRCRAFT RETURNED TO SERVICE.
DURING TAKEOFF ROLL DEN-MSP (BELOW 60 KNOTS), FORWARD CARGO DOOR WARNING LIGHT ILLUMINATED. ABORTED TAKEOFF WITHOUT FURTHER INCIDENT. NO TROUBLE FOUND WITH DOOR, TARGET, OR SENSOR.
CAPTAIN'S WINDSHIELD ANTI-ICE SYSTEM FAILED AFTER TAKEOFF FROM PHL. FLIGHT DIVERTED TO MEM AND LANDED WITHOUT INCIDENT. REPLACED CAPTAIN'S WINDSHIELD TEMPERATURE CONTROLLER. OPERATIONAL CHECK OK. AIRCRAFT RETURNED TO SERVICE.
