Maintenance disclosures
DURING CLIMB, THE FLIGHT CREW NOTED THAT THE AIRCRAFT ROLLED TO THE LEFT AND REQUIRED FULL INPUT TO LEVEL OFF THE WINGS. THE CREW CALLED IT IN, AND AIR RETURNED WITHOUT FURTHER INCIDENT. MAINTENANCE TROUBLESHOT THE SYSTEM, CHECKED ALL FLIGHT CONTROL CABLES FOR WEAR AND PROPER TENSION AND FOUND THE ELEVATOR TRIM SLIGHTLY OUT OF THE ZERO-DEGREE SETTING. THEY THEN ADJUSTED THE TRIM PER THE MANUAL REQUIREMENTS, REPLACED THE AILERON DUE TO A PREVIOUS UNRELATED SQUAWK FOR ASSURANCE THAT IT WOULD NOT BE AN ISSUE TO OVERCOME, AND THE AIRCRAFT WAS CHECK FLOWN SUCCESSFULLY. NO REPEAT DISCREPANCIES HAVE BEEN NOTED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
AFTER DEPARTING KMFR ENROUTE TO KPDX THE FLIGHT CREW REPORTED YAW DAMPER FAILURE FOLLOWED BY AN ELECTRICAL BURNING SMELL. PILOT DECLARED AN EMERGENCY. THE AIRCRAFT DIVERTED TO KSLE AND LANDED WITHOUT INCIDENT. POST FLIGHT INSPECTION NOTED THE YAW DAMPER CIRCUIT BREAKER TRIPPED. MAINTENANCE TROUBLESHOT ISSUE AND REMOVED AND REPLACED YAW DAMP COMPUTER. OPS CHECKS NORMAL. AIRCRAFT
AMF1908 â ENROUTE TO KMFR PILOT DIVERTED TO KEUG DUE TO RT PROPELLER RPM FAILURE. PILOT REPORTED THAT AT CRUISE, THE RIGHT PROPELLER RPM FAILED AND DROPPED TO AROUND 700 RPM. PILOT REDUCED TORQUE AND RPMS INDICATED NORMAL, AFTER RAISING TORQUE AGAIN RPM FAILED BACK TO AROUND 700 RPM. EMERGENCY WAS DECLARED. LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED RH ENGINE CONTROLS FOR ANY OBSTRUCTIONS, DEFECTS OR DAMAGE AND NOTHING WAS FOUND WAS FOUND AT ANY OF THE ENGINE CONTROLS FOR THE RH ENGINE. NO LEAKS OR ENGINE COMPONENTS WERE FOUND LOSE. ALL WIRING WAS FOUND TO BE IN SERVICEABLE CONDITION AND ALL ELECTRICAL CONNECTIONS WERE CONNECTED. PERFORMED GROUND RUN WITH AUTO FEATHER CHECKS, PROP OVERSPEED CHECKS AND PRIMARY GOVERNOR CHECKS. ALL ENGINE INDICATIONS WERE NORMAL. OPS CHECK GOOD FOR RH ENGINE WITH NO DEFECTS FOUND AT THIS TIME. MAINTENANCE DETERMINED THAT THE ISSUE WAS THE RH PROP TACH INDICATOR. ALL FOLLOW ON OPERATIONAL CHECKS OF THE GAUGE WERE GOOD. AIRCRAFT HAS BEEN RETURNED TO SERVICE.
THE PILOT REPORTED THAT THE LEFT ENGINE OIL PRESSURE BEGAN FLUCTUATING NEAR THE LOW REDLINE. THE PILOT PERFORMED A PRECAUTIONARY INFLIGHT SHUT DOWN OF THE LEFT ENGINE AND DIVERTED. LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE LEFT ENGINE AND FOUND OIL LEAKING FROM THE INLET AREA. FURTHER INVESTIGATION FOUND THAT THE OIL BREATHER SKEET TUBING INNER WALL COLLAPSED NOT ALLOWING THE OIL SYSTEM TO VENT PROPERLY. REPLACED THE SKEET TUBING, PERFORMED ENGINE GROUND RUNS NO OIL LEAKS WERE PRESENT. NO OIL CONSUMPTION WAS NOTED ON THE GROUND RUN.
ON LANDING THE PILOT REPORTED THERE WAS NO PRESS ON THE RT BRAKE. TAXIED TO PARKING WITHOUT INCIDENT. INSPECTED THE RT BRAKE SYS & FOUND THE FLEXIBLE BRAKE LINE PN MS28741-4-0226 LEAKING AT THE FITTING. REPLACED BRAKE LINE, BLEED BRAKE SYS. RT BRAKE OPERATIONS CHECKED OK.
ON APPROACH TO BQN PILOT SELECTED LG DOWN, RECEIVED THE RED IN TRANSIT WARNING LIGHT FOR 10 SECONDS THEN THE LIGHT WENT OUT BUT LG DID NOT EXTEND & THREE GREEN DOWN & LOCK INDICATOR LIGHTS DID NOT ILLUMINATE. TRIED TO CYCLE THE LG, THE LG WOULD NOT EXTEND. PERFORMED A MANUAL EXTENSION OF THE LG, RECEIVED THREE DOWN & LOCKED LG INDICATOR LIGHTS & LANDED AT BQN WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED LOOSE PINS IN THE PC BOARD CONNECTOR FOR LG TIME DELAY. THE LOCKING FEATURE FOR THE PINS WAS BROKEN. R & R CONNECTOR IAW MANUAL 32-31-01. PERFORMED GND OPERATIONAL CHECK OF LG SYS IAW MM 32-30-01 & ACFT PERFORMED AN OPERATIONAL CHECK FLIGHT, BOTH CHECKS WERE NORMAL.
DURING APPROACH INTO BGI LG FAILED TO EXTEND WITH NO INDICATION, IN TRANSIT LIGHT, PILOT ATTEMPTED TO CYCLE LG WITH SAME RESULTS. THE PILOT MANUALLY EXTENDED LG, GEAR WAS EXTENDED WITH THREE GREEN DOWN & LOCK INDICATION LIGHTS ILLUMINATED. LANDED AT BGI WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED LOOSE GND CABLE, SECURED CABLE & PERFORMED OPERATION CHECK OF LG SYS IAW MM 32-60-00 SYS CHECKED NORMAL.
ON APPROACH AT TLPC PILOT SELECTED LG DOWN, LG FAILED TO DEPLOY & THREE GREEN LIGHT LG POSITION INDICATORS FAIL TO ILLUMINATE. PILOT TRIED SEVERAL TIMES TO CYCLE GEAR WITH SAME RESULTS, AT THAT TIME PILOT DECIDED TO MANUALLY EXTEND THE LG. AFTER SUCCESSFULLY EXTENDING LG & RECEIVING THREE GREEN DOWN & LOCK INDICATOR LIGHTS, PILOT LANDED AT TPLC WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED DAMAGED WIRE ON LT MLG ACTUATOR SWITCH. R & R LT MLG ACTUATOR IAW MM 32-30-10-201. OPERATIONAL CHECKS OF LG SYS WERE NORMAL.
ON VISUAL APPROACH, PILOT SELECTED LG HANDLE DOWN, THERE WAS NO AUDIBLE REACTION TO MOVING THE HANDLE & THE GEAR HANDLE REMAINED RED WITH NO GREEN INDICATOR LIGHTS, VISUALLY VERIFIED GEAR WAS NOT EXTENDED, TRIED CYCLING GEAR SEVERAL TIMES WITH SAME RESULTS, DECIDED TO PERFORM MANUAL LG EXTENSION IAW EMERGENCY PROCEDURES. AFTER EXTENSION PILOT PERFORMED A FLY BY THE TOWER TO VISUALLY VERIFY GEAR WAS IN FACT EXTENDED, WITH CONFIRMATION FROM TOWER THAT GEAR WAS EXTENDED PILOT LANDED WITHOUT FURTHER INCIDENT. MX INITIAL TROUBLESHOOTING FOUND LG CONTROL 2 AMP CB POPPED. MX RESET LG CONTROL CB & PLACED AIRCRAFT ON JACKS, PERFORMED LG OPERATIONAL CHECK NO DEFECT FOUND AFTER FOUR CYCLES. PERFORMED A VISUAL INSPECTION OF THE LG SYS COMPONENTS, FOUND THE CB FOR THE LG TIME DELAY TERMINALS DIRTY, CLEANED THE TERMINALS. FOUND LG TIME DELAY CONTROL BOX CANNON PLUG LOOSE & VERY DIRTY. CLEANED CONTACTS SECURED CANNON PLUG. PERFORMED OPERATIONAL CHECK OF LG SYS BY COMPLETING 8 CYCLES IAW MM 32-30-00 NO DEFECTS NOTED.
ELT BATTERY, WITH 2 MOS REMAINING UNTIL CALENDAR EXPIRATION, BEGAN LEAKING, CAUSING PLASTIC CASE TO CRACK AND DISINTEGRATE. ACID LEAKED INTO TRANSMITTER RENDERING IT INOPERATIVE. LEAKING ACID ALSO CAUSED THE PLASTIC TRAY (ORIGINALLY SUPPLIED WITH THE ELT AS PART OF THE INSTALLATION KIT) TO CRACK AND DISINTEGRATE AS WELL, REDUCING OR ELIMINATING THE 9G RESTRAINT CAPABILITY. ACID ALSO SPILLED ON AIRFRAME MEMBERS. FOUND DURING A SCHEDULED INSPECT. SUBMITTER STATES, THIS IS THE 2ND UNIT TO HAVE BEEN FOUND FAILED IN THIS MANNER. S/N 55215 WAS ALSO FOUND TO HAVE CRACKED CASE AND TRAY DUE TO LEAKING ACID.
