ENGINE SURGED 2ND TIME CAUSING ACFT TO YAW. ELECTED TO SHUTDOWN ENGINE. MAX NP WAS 106 PERCENT. PROP WAS IN A PITCH LOCK SITUATION AND THEN FEATHERED NORMALLY. MAINTENANCE REMOVED AND REPLACED LT ENGINE.

DEN - FLT 7131 - CABIN FILLED WITH SMOKE AT ENGINE SHUT-DOWN. INSPECTED PACKS, FOUND NO DISCREPANCIES. GROUND RAN AIRCRAFT 15 MINUTES, COULD NOT DUPLICATE. NO CAS MESSAGE, SYSTEM OPS CHECK NORMAL. (X)
SHORTLY AFTER TAKEOFF, THE FLIGHT CREW REPORTED A PROBLEM WITH THE NR 1 ALTERNATOR. THEY WERE ALERTED BY AC VOLT FAIL MESSAGE ON THE EICAS SCREEN. AIRCRAFT RETURNED TO DEN, LANDED WITHOUT FURTHER INCIDENT. DENVER MAINTENANCE REPLACED THE NR 1 DAU IAW AMM 31-42-00, OPERATIONAL CHECK WAS CONDUCTED WITH SATISFACTORY RESULTS.
DURING CLIMBOUT, THE CREW REPORTED A PROBLEM WITH AIR LEAKING AROUND THE DOCUMENT DOOR. CREW THEN ELECTED TO RETURN TO DEN. AIRCRAFT LANDED IN DEN WITH NO FURTHER INCIDENT. MAINTENANCE INSPECTED THE DOCUMENT DOOR AND FOUND NO DISCREPANCIES. ALSO, CONDUCTED A GROUND OPERATIONAL CHECK OF THE PRESSURIZATION SYSTEM IAW AMM 21-31-00 COULD NOT DUPLICATE THE PROBLEM. THE AIRCRAFT WAS THEN RETURNED TO SERVICE. IT SHOULD BE NOTED, THE DOCUMENT DOOR WAS REPLACED THE FOLLOWING DAY, BECAUSE OF A SEAL FAILURE.
AFTER TAKEOFF ON CLIMBOUT, THE LANDING GEAR LEVER WILL NOT SELECT UP. LEVER MOVED TO THE UP POSITION WHEN THE DOWNLOCK RELEASE BUTTON WAS DEPRESSED. REPLACED THE PROXIMITY SWITCH ELECTRONIC UNIT, OPERATIONAL CHECKS GOOD.
SEA- ACRID SMELL IN COCKPIT AND CABIN. ISOLATED TO AFT AVIONICS FAN, PLACED ON MEL, REF 21-6. AIRCRAFT RETURNED TO SERVICE. MEL CLEARED BY REPLACING REAR AVIONICS FAN AND RECIRIC FILTER, OPERATIONAL CHECKS GOOD.
DURING TEST OF THE FIRE EXTINGUISHING SYSTEM, THE NR 1 LT SQUIB LIGHT WAS INOP. CLEANED THE SQUIB CANNON PLUG CONNECTION ON THE NR 1 FIRE BOTTLE IN THE ENGINE COMPARTMENT, OPERATIONAL CHECKS GOOD.
LEFT REAR EMERGENCY EXIT DOOR WARNING CAS MESSAGE INFLIGHT AT 17,000 FT. LIGHT WENT OUT AT 4.50 CABIN PSID. PROXIMITY SENSOR REPLACED AND ADJUSTED IAW MAINT MANUAL, WARNING SYSTEM OPS CHECKED GOOD.
DOOR WARNING CAS MESSAGE IN FLIGHT, SYSTEM INDICATED LEFT REAR UNDERWING EXIT. NO AIR LEAKS OR SOUND COMING FROM EXIT, NO LOSS OF CABIN PRESSURE. AIRCRAFT ALTITUDE WAS 14,000 FT, LIGHT EXTINGUISHED AT 5,000 FT. EXIT PROXIMITY SENSOR ADJUSTED IAW MAINT MANUAL AND INDICATION OPS CHECKED GOOD.
INTERMITTENT DOOR WARNING CAS MESSAGE AFTER TAKEOFF, LEFT SIDE UNDER WING EMERGENCY EXIT. EMERGENCY EXIT PROXIMITY SENSOR ADJUSTED IAW MAINT MANUAL, DOOR INDICATION OPS CHECKED GOOD.
LEFT WING LEADING EDGE SECTION 511AL .5 INCH CRACK NOTED IN OUTBOARD RIB UPPER FLANGE AT THE LAST FASTENER LOCATION. CRACK FOUND DURING C-CHECK. CRACK STOP DRILLED AND DOUBLER INSTALLED PER EA 7-57-49.
NR 1 ENGINE BEGAN SURGING 20 TO 80 PERCENT ON APPROCH TO SEA. ALL ENGINE INSTRUMENTS INDICATED THE SURGING ALONG WITH AIRCRAFT YAW. NR 1 ENGINE BROUGHT TO IDLE AND PROP WAS FEATHERED. AIRCRAFT LANDED IN SEA WITHOUT FURTHER PROBLEM. NR 1 NP PROBE REPLACED AND ENGINE OPS CHECKED GOOD ON GROUND RUN.
PDX - AFT EXIT SIGN INOP 9 AND 10, FLOOR PROXIMITY LIGHTS INOP AFT RIGHT HAND EXIT LIGHT INOP. BATTERY AND POWER SUPPLY 7LK REPLACED. DAMAGED PIN FOUND ON REMOVED UNIT AND AIRCRAFT CONNECTOR. LIGHTS OPS CHECKED GOOD AND AIRCRAFT RETURNED TO SERVICE.
PDX - EXIT SIGN ABOVE SEAT 11A AND RIGHT SIDE AFT EXTERIOR INOP. REPLACED AND LIGHT OPS CHECKED GOOD.
PDX - GALLEY DOOR EXTERIOR EMERGENCY LIGHT IS INOP. BATTERY PACK REPLACED AND LIGHT OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
PDX - AT FL 260 ON DESCENT, MESSAGE ELEVATOR MISTRIM NOSE ON. AFTER AUTOPILOT DISCONNECT THE ELEVATOR TRIM WOULD NOT FUNCTION, THE TRIM WAS STUCK. FLIGHT CREW SELECTED STANDBY TRIM, BUT TRIM DIDN'T FUNCTION WITH CONTINUED FORWARD PRESSURE TO MAINTAIN FLIGHT PATH. AT LOWER ALTITUDE WITH AIRSPEED BELOW 200 KTS TRIM SPEED FAST ACTIVATED AND TRIM BEGAN TO FUNCTION NORMALLY FROM LEFT AND RIGHT SIDE. AIRCRAFT ACCELERATED TO 210 KTS AND TRIM FUNCTIONED NORMALLY, LEFT, RIGHT AND STANDBY. AIRCRAFT LANDED WITHOUT FURTHER PROBLEM. LEFT AND RIGHT TRIM ACTUATORS REPLACED IAW MAINT MANUAL. TRIM SYS OPS CHECKED GOOD.
