FLT 248, PILOT REPORTED THAT WHILE DESCENDING FROM 8000 TO 3000 FT APPROX 30 NM OF DESTINATION, THE ELEVATOR TRIM FAILED. DECLARED AN EMERGENCY, PILOT REPORTED THAT THE CREW "MUSCLED" THE ACFT WHERE THEY LANDED WITHOUT FURTHER INCIDENT. FOUND TRIM CABLE BROKEN.
PILOT REPORTED LT ENGINE WOULD ONLY PRODUCE 1600 LBS OF TORQUE AT MOST. THE TAKEOFF WAS ABORTED AND ACFT RETURNED TO THE PARKING RAMP WITHOUT FURTHER INCIDENT AND MX WAS NOTIFIED. MX TROUBLESHOT ACFT AND PERFORMED THE FOLLOWING MX. TROUBLESHOOT PY LINES AND FILTER - CHECKS GOOD. CAPPED FUEL CONTROL UNIT AND TESTED LINES - CHECKED GOOD. MX FOUND A FAULTY NR1 PROP GOVERNOR TO BE THE CAUSE. MX R & R FAULTY PROP GOVERNOR IAW MM 61-20-00 AND SD3-30 MM 71-00-02. MX PERFORMED ENGINE RUN FUNCTIONAL AND LEAK CHECKS AND ALL WERE GOOD. ACFT RETURNED TO SERVICE.
DURING MX, CORROSION INSPECTION, FOUND CORROSION ON LT WING OTBD SIDE OF NR 1 NACELLE INBD T/E BETWEEN WS 160 & 175. MX REMOVED LT WING UPPER SKIN DOUBLERS AT WS 160 AT APPROX 7.5" FROM L/E SPAR AND APPROX 7.5" FROM THE REAR EDGE SPAR. ON REMOVAL AND FURTHER INSPECTION OF WING BAY AREA, FOUND CORROSION ON INTERNAL WING SUPPORT U CHANNELS, TEE SUPPORT FITTINGS AND SUPPORT BRACKETS. FABRICATED WING SKIN DOUBLERS IAW SD3-30 SRM 57-09-5, FIG 1. FABRICATED SUPPORT U CHANNELS IAW SRM 57-09-5, FIG 1. REMOVED THE INTERNAL SUPPORT LT BRACKETS AT WS 160. FABRICATED SUPPORT L BRACKETS AT WS 160. FABRICATED SUPPORT LT BRACKETS IAW SRM 51-30-00. REMOVED BUTT STRAP AT WS 160 FROM LEADING EDGE SPAR TO T/E SPAR, FABRICATED BUTT STRAP IAW SRM 51-30-00 AND SHORTS DWG SD360-578-11C. REMOVED BUTT STRAP FILLERS AT WS160. FABRICATED BUTT STRAP FILLERS IAW SRM 51-30-00. ALL MATERIAL FABRICATED WERE CLEANED, TREATED AND PRIMED AND PAINTED IAW SRM 51-23-00. INSTALLED FASTENERS AND WING SKIN DOUBLERS AND BUTT STRAP ON LT WING UPPER SURFACE AT WS 160 IAW DWG SD3-30-6535 / SD360-57-11/C.
AFTER LEVELING OFF AT 9,000 FT, CREW OBSERVED A FLAME COME OUT OF NR 1 ENGINE, FOLLOWED BY THE GENERATOR LIGHT ILLUMINATING, AND ENGINE SPOOLING DOWN. FOLLOWED EMERGENCY CHECKLIST AND SHUTDOWN NR 1 ENGINE IN-FLIGHT. ATTEMPTED TO RESTART NR 1 ENGINE PER EMERGENCY CHECKLIST, WHICH WAS UNSUCCESSFUL. DECLARED AND EMERGENCY AND DIVERTED. LANDED WITHOUT ANY FURTHER INCIDENT. DURING POSTFLIGHT, OBSERVED OIL LEAKING OUT THE BOTTOM OF COWLING. FOUND FUEL OIL HEATER HOUSING CRACKED, OIL LEVEL AT APPROXIMATELY 4, FOUND COMPRESSOR SEIZED. POWER SECTION CHIP DETECTOR INSPECTED AND FOUND VERY FINE DEBRIS ON CHIP DETECTOR.
DURING TAXI, RT POWER LEVER HAD SEVERE VIBRATION. RETURNED TO RAMP. INSPECTED NR 2 ENG & PROP. FOUND 3 PROP BOLTS FRACTURED AND/OR SHEARED. ACFT GROUNDED. PERFORMED ENGINE VIBRATION INSPECTION CHECK IAW MM 5-51-13. NR 2 ENG REDUCTION GEARBOX PROP MOUNTING FLANGE NDT INSPECTED & RUN-OUT INSP COMPLETED. NR 2 ENGINE TUBULAR MOUNT, RUBBER ISO MOUNTS & MOUNT BOLTS INSP. INSTALLED NEW ISO MOUNTS BOLTS IAW MM 71-21-01. INSTALLED NEW NR 2 PROP GOV IAW MM 61-20-00. INSTALLED NR 2 PROP IAW MM 61-10-01. INSTALLED NEW CARBON BLOCK IAW MM 61-21-00. OPS CHECKED & DYNAMIC PROP BALANCE COMPLETED. NR 2 ENG OIL FILTER INSPECTED & PACKINGS REPLACED IAW MM 79-00-00. INSTALLED NEW CAM BOX IAW MM 76-10-00 DUE TO WORN LINKAGE & ENG RIGGED IAW MM 71-00-02. ENG RUNS & OS CHECK COMPLETED, NO DEFECTS NOTED. ACFT RETURNED TO SERVICE.
ON APPROACH, THE PILOT PUT THE LANDING GEAR HANDLE DOWN FOR LANDING AND THE RT MAIN GEAR INDICATOR FAILED TO ILLUMINATE. THE CREW EXECUTED A GO AROUND AND REQUESTED VECTORS IN ORDER TO ACCOMPLISH ABNORMAL CHECKLIST (DID NOT DECLARE AN EMERGENCY). AFTER A SUCCESSFUL COMPLETION OF THE ABNORMAL GEAR DOWN CHECKLIST, THE FLIGHT RETURNED TO THE AIRPORT AND LANDED WITHOUT ANY FURTHER INCIDENT. MX TROUBLESHOT ACFT AND FOUND BURNED OUT / FAILED RT MAIN GEAR INDICATOR LAMP. MX R & R RT MAIN GEAR INDICATOR LAMP IAW MM 33-13-00. OPS CHECKS WERE GOOD.
DURING 4800/9600 HOUR STRUCTURAL INSP THE FOLLOWING ITEM WAS FOUND. RT LOWER WINDOW FRAME RAIL-STA 30 HAS CORROSION DAMAGE. REMOVED CORROSION, TREATED AREA AND INSTALLED DOUBLER FABRICATED FROM 2024 T3 .063-SIZE (7" X 4"). REPAIRED IAW ENGINEERING REPORT NR 8434-1 REV IR.
LT AND RT CEILING STA 584.93 HAS SKIN CORROSION, APPROX SIZE (5" X 9"). LT UPPER SIDE WALL STA 514 HAS SKIN CORROSION. REMOVED CORROSION, TREATED AREA AND INSTALLED 5.25" X 7.25" FABRICATED DOUBLER FROM 2024T3 .032. CLEANED ALODINED AND PRIMED, INSTALLED IAW SRM 53-00-00.
DURING 4800/9600 HOUR STRUCTURAL INSP, FOUND CORROSION ON FUSELAGE SKIN FOUND AT STA 146.2-FUEL TANK RT OF CENTERLINE. REMOVED CORROSION, TREATED AREA AND INSTALLED DOUBLER FABRICATED FROM 2024T3 .032 -10" X 6" IN SIZE AT STA 146.2 - (7) INCHES RT OF CENTERLINE IAW SRM 53-00-00. CORROSION DAMAGE FOUND AT STA 200.2 - FUEL TANK LT OF CENTERLINE. REMOVED CORROSION, TREATED AREA AND INSTALLED DOUBLER FABRICATED FROM 2024 T3 .032 - 9" X 6" IN SIZE AT STA 200.2 - 18 INCHES LT OF CENTERLINE IAW SRM 53-00-00.

CORROSION DAMAGE FOUND AT STA 187.2-FUEL TANK RT OF CENTERLINE. REMOVED CORROSION, TREATED AREA AND INSTALLED DOUBLER FABRICATED FROM 2024 T3 .032 -7" X 6" IN SIZE AT STA 187.2 - (7) INCHES RT OF CENTERLINE IAW SRM 53-00-00.

LT AFT TENSION STRAP AT STA 264.77 HAS CORROSION DAMAGE. REMOVED CORROSION, TREATED AREA AND INSTALLED DOUBLER FABRICATED FROM 2024 T351 .190-SIZE (29" X 5"). LT FWD SEAT TRACK CORRODED AT STA 187.2 AND STATION 213.2 - REQUIRES A 36 INCH SPLICE REPAIR- (REF SRM 53-00-00 PAGES 62 & 64). LEFT AFT SEAT TRACK CORRODED AT STATION 264.77 AND STATION 289.92-REQUIRES A 48 INCH SPLICE REPAIR-(REF SRM 53-00-00 PAGES 62 & 64). LT AFT SEAT TRACK CORRODED STATION 422-REQUIRES A 6 INCH SPLICE REPAIR- REF SRM 53-00-00 PAGES 62 & 64. REMOVED 11" OF CORRODED SEAT TRACK AT STATION 187.2 AND INSTALLED NEW SPLICE FABRICATED IAW SRM 53-00-00. RIGHT AFT SEAT TRACK CORRODED AT STATION 264.77THROUGH STATION 422. REMOVED RIGHT AFT SEAT TRACK AND INSTALLED NEW TRACK FAVRICATED IAW SRM 53-00-00.
JUST AFTER TAKEOFF, THE CREW REPORTED THAT THE NOSE LANDING GEAR WOULD NOT LOCK IN THE UP POSITION. SHORTLY AFTERWARDS BOTH OF THE HYDRAULIC SYSTEM LOW PRESSURE LIGHTS ILLUMINATED. THE CREW DECLARED AN EMERGENCY AND PERFORMED THE ABNORMAL PROCEDURES LIST FOR HYDRAULIC FAILURE. THE AIRCRAFT RETURNED TO DEPARTURE AND LANDED WITH OUT FURTHER INCIDENT. MX FOUND THE NOSE LANDING GEAR UP HYDRAULIC ACTUATOR HYDRAULIC FLEX LINE BROKEN. MX REMOVED AND REPLACED THE HYDRAULIC LINE. AS A PRECAUTION MX REMOVED AND REPLACED BOTH LT AND RT ENGINE DRIVEN HYDRAULIC PUMPS BECAUSE OF POSSIBLE CAVITATIONS AND SERVICED THE HYDRAULIC SYSTEM. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLT 120-DURING APPROACH, CREW REPORTED RT MLG UNLOCK INDICATION AFTER SELECTING GEAR HANDLE TO THE DOWN POSITION. THE AIRCRAFT WAS DIVERTED, ATC DECLARED AN EMERGENCY AND THE AIRCRAFT LANDED WITHOUT INCIDENT. MX FOUND THE RT MLG DOWNLOCK PROXIMITY SWITCH OUT OF ADJUSMENT. MX PERFORMED ADJUSTMENT AND OPERATIONAL CHECK OF THE SWITCH AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
HNL - LOWER FUSELAGE SKIN CORRODED THROUGH CORRUGATED SKIN AT STA 350 - STA 364 AT CENTER LINE ZERO TO 4 LEFT. HIGHLY CORROSIVE OPERATING ENVIRONMENT, AGING AIRCRAFT. REPAIRED WITH INSERTION REPAIR PER THE SD3-30 SRM AND 43.13-1B. DOCUMENTED ON FORM 337. (X)
