STRONG BURNING ODOR IN COCKPIT AND CABIN. MAINTENANCE FOUND AVIONICS FAN PREVIOUSLY MEL EMITTING STRONG BURNING ODOR. REMOVED AND REPLACED HX FAN.
HYD AUX QTY FAIL AND PROX SYS CAS DURING DESCENT GEAR WILL NOT COME DOWN AND HAD TO BE MANUALLY LOWERED. MAINTENANCE REMOVED AND REPLACED PSEU.
DEN - FLT 7062 - DURING TAXI IN, HAD NUMEROUS ELECTRICAL FAILURES AND SMOKE IN COCKPIT PRIOR TO NR 1 ENGINE SHUT-DOWN. GROUND RAN A/C USING BOTH ENGINES, OPERATING BOTH PACKS. CHECKED ELECTRICAL SYSTEM LOADS ON MOST MAJOR SYSTEMS, ALL CHECKS WERE OK. NO DEFECTS NOTED. (X)
EN ROUTE, THE CREW RECEIVED A DOOR WARNING INDICATION. ACFT RETURNED, LANDED WITHOUT FURTHER INCIDENT. MX DISCOVERED THE PAX DOOR PROXIMITY SENSOR TARGET WAS MISRIGGED. ALSO, THE PROXIMITY SENSOR BRACKET WAS DAMAGED SEVERAL ATTACH RIVETS WERE LOOSE. ACFT WAS THEN FLOWN TO COS, WHERE MX REPLACED THE DOOR HANDLE BELLCRANK ASSEMBLY COMPLETE WITH NEW PROX TARGET BRACKET. ALL WORK ACCOMPLISHED IAW AMM 52-71-00. ALL GROUND OPERATIONAL CHECKS WERE SATISFACTORY. ACFT WAS RELEASED FOR SERVICE. THIS IS THE SECOND OCCURANCE OF THIS DISCREPANCY IN THE LAST THREE DAYS.
EN ROUTE, THE CREW WAS ALERTED BY A DOOR WARNING CAS MESSAGE. CREW ELECTED TO RETURN. MX INSPECTED THE DOOR WARNING SYSTEM IAW AMM 52-71-00 WITH NO DISCREPANCIES NOTED. THE ACFT WAS RETURNED TO SERVICE. NOTE, TWO DAYS PRIOR TO THIS INCIDENT, MAINTENANCE REPLACED THE PAX DOOR BECAUSE OF A HINGE FAILURE ON THE ORIGINAL UNIT. THE REPLACEMENT DOOR WAS NEW, THE LOCKING MECHANISM WAS VERY STIFF IN OPERATION. SUSPECT F/A MAY NOT HAVE HAD THE DOOR FULLY LOCKED PRIOR TO DEPARTURE, AND ANY BUFFETING, NO MATTER HOW SLIGHT, MAY HAVE CAUSED THE ACTIVATION OF THE DOOR WARNING SYSTEM. NO FURTHER DIFFICULTIES.
SHORTLY AFTER THE A/C ARRIVED IN (DIA) MX WAS REQUESTED TO INSPECT THE PAX DOOR AS A RESULT OF DOOR BEING DIFFICULT TO CLOSE FROM TUL. MX INSPECT DOOR AND DISCOVERED AFT MAIN DOOR SUPPORT HINGE CRACKED ALMOST INTO, CRACK WAS COMPLETELY THROUGH THE UPPER FLANGE AND WEB MEMBER AND CONTINUED THROUGH LOWER FLANGE. THE A/C WAS FERRIEDTO COS, WHERE THE DOOR COULD BE REPAIRED. MX ELECTED TO REPLACE THE COMPLETE DOOR ASSY. THE DOOR ASSY WAS REPLACED IAW AMM 52-12-00. UPON REMOVAL OF DOOR ASSY MX NOTICED DAMAGE TO THE EXT SKIN OF THE DOOR. SUBMITTER STATES, SUSPECT DAMAGE WAS RESULT OF SOMEONE PLACING SUPPORT UNDER THE DOOR TO PREVENT FURTHER DAMAGE TO THE HINGE SUPPORT.
DURING THE TAKEOFF ROLL, THE FLT WAS ALERTED BY A ELPMP AUTO FAIL CAS MESSAGE. THE FLT CREW ABORTED THE TAKEOFF AND RETURNED TO THE GATE. DENVER MX INSPECTED THE A/C AND CONDUCTED GROUND RUNS IAW AMM JIC. 28-00-00. ALL CHECKS WERE SATISFACTORY. MX UNABLE TO DUPLICATE THE PROBLEM. A/C RETURNED TO SERVICE. A/C EXPERIENCED THE IDENTICAL AS THE DAY PRIOR, ELPMP AUTO FAIL. COS, MX REPLACED BOTH THE JET AND ELECTRIC BOOST PUMPS. GROUND CHECKS WERE SATISFACTORY. A/C RETURNED TO SERVICE.
NR 1 ENGINE INTAKE FAIL CAS MESSAGE AFTER TAKEOFF. AIRCRAFT RETURNED TO THE FIELD. REPLACED INTAKE ASSEMBLY, OPERATIONAL CHECKS GOOD.
NOSE GEAR DID NOT INDICATE DOWN AND LOCKED ON GEAR EXTENSION. CYCLED GEAR AND NOSE GEAR AGAIN FAILED TO INDICATE DOWN AND LOCKED. ALTERNATE GEAR EXTENSION PERFORMED AND NOSE GEAR INDICATED DOWN AND LOCKED. AIRCRAFT LANDED WITHOUT FURTHER PROBLEM IN RDD. AIRCRAFT FERRIED TO PDX FOR REPAIR. LANDING GEAR CONTROL LEVER REPLACED AND RESTRICTOR VALVES CLEANED. GEAR OPS CHECKED GOOD ON GEAR SWING.
PILOTS WINDSHIELD STARTED ARCING AND CRACKING UP THE CENTER OF THE WINDSHIELD. WINDSHIELD HEAT SWITCHED OFF AND AIRCRAFT LANDED IN BTM WITHOUT FURTHER PROBLEMS. PILOTS WINDSHIELD REPLACED IAW MAINTENANCE MANUAL. WINDSHIELD HEAT CONTROLLER REPLACED AS A PRECAUTIONAY MEASURE. WINDSHIELD HEAT OPS CHECKED GOOD.
FLIGHT ATTENDENT REPORTED SMOKE IN CABIN ON CLIMBOUT. LEFT AIR PACK SWITCHED OFF AND AIRCRAFT RETURNED TO FIELD. AIRCRAFT LANDED WITHOUT FUTHTER PROBLEM. LEFT HX FAN CIRCUIT BREAKER TRIPPED CAUSING PACK TO OVERHEAT. ALSO, TAXI LIGHT BREAKER TRIPPED. CIRCUIT BREAKERS RESET AND LEFT AIR PACK OPS CHECKED GOOD.
GEG - DOOR WARNING LIGHT ILLUMINATED, SYSTEM PAGE INDICATED EXIT AT 9A. CABIN DIFFERENTIAL PRESSURE LOWERED TO 3.45 PSID AND LIGHT WENT OUT. AIRCRAFT LANDED WITHOUT FURTHER PROBLEMS. REMOVED AND REINSTALLED EMERGENCY EXIT DOOR AT 9A. AIRCRAFT PRESSURIZED TO 5.5 PSID AND WARNING LIGHT REMAINED OUT. AIRCRAFT RETURNED TO SERVICE.
BIL - DOOR WARNING LIGHT ILLUMINATED DURING DESCENT THROUGH 20,000 FEET AT 5.1 PSID, SYSTEM PAGE INDICATED EXIT AT 9A. AIRCRAFT LANDED IN BIL WITHOUT FURTHER PROBLEMS. AIRCRAFT FERRIED TO GEG FOR REPAIR. PROXIMITY SENSOR ADJUSTED AND EXIT AT 9A IAW MM. SENSOR OPS CHECKED GOOD IAW MM. TEST FLIGHT CONDUCTED AND FLIGHT WAS SATISFACTORY. AIRCRAFT RETURNED TO SERVICE.
EN ROUTE, SMOKE WITH ASSOCIATED SMELL COMING FROM NOSE UP/ NOSE DOWN THUMB WHEEL ON FGC. AIRCRAFT DIVERTED TO EUG AND LANDED WITHOUT FURTHER PROBLEMS. FGC AND NR 1 IAC WERE REPLACED AND AUTOPILOT OPS CHECKED GOOD.
NR 1 ENGINE OIL PRESS DROPPED TO 30 PSI, LEFT ENGINE LIGHT ILLUMINATED. NR 1 ENGINE SHUTDOWN IN FLIGHT. AIRCRAFT LANDED WITHOUT FURTHER PROBLEM. NR 1 ENGINE CENTRIFUGAL CARBON SEAL FOUND CRACKED. CARBON SEAL REPLACED AND ENGINE OPS CHECKED GOOD. NO LEAKS OR OIL LOSS NOTED.
PDX - RIGHT MAIN GEAR INDICATION LIGHT FLICKERS AND PROXIMITY HYD CAS MESSAGE ON GEAR EXTENSION, ONLY RIGHT MAIN GEAR LIGHT FLICKERS. S12 DOWNLOCK SWITCH CANNON PLUG CLEANED AND RESEATED. GEAR INDICATION OPS CHECKED GOOD ON GEAR SWING. AIRCRAFT RETURNED TO SERVICE.
SEA - SMELL OF SMOKE IN AIRCRAFT FOLLOWED BY AVIONIC FAN FAIL CAS MESSAGE. AFT AVIONIC FAN REPLACED I/A/W MAINT. MANUAL. FAN OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
DURING ROUTINE MAINTENANCE, FOUND COCKPIT PBE CONTAMINATED. REPLACED PBE AND RETURNED AIRCRAFT TO SERVICE.
RDM - APPROCH INTO RDM, NR 2 ENG TORQUE DROPPED FROM APPROX 40 PERCENT TO 0 PERCENT, FOLLOWED BY YAWING OF A/C. ADVANCED NR 2 POWER LEVER, TORQUE WAS RESTORED. ALMOST IMMEDIATLY NR 1 TQ DROPPED FROM APPROX 40 PERCENT TO 0 PERCEN. ADVANCED PWR LEVER AND TQ WAS RESTORED. WEATHER WAS SNOWING, ALL ENG DEICE WAS ON. NO EVIDENCE OF ICE WAS ON THE A/C. A/C LANDED IN RDM W/O FURTHER INCIDENT. OPS CHECK OF NR 1/2 AIR INTAKE AND DEICE SYS PERFORMED, AIR INTAKE INSP PERFORMED, POWER ASSURANCE RUN ACCOMPLISHED ON NR 1/2 ENG, ALL CHECKS WERE NORMAL. REPLACED NR 1/2 FUEL FILTERS. FUEL SAMPLES TAKEN, WATER AMOUNT IN FUEL WAS ABOVE NORMAL BUT NOT ENOUGH TO CAUSE ENG POWER LOSS. FDR REMOVED FOR FURTHER ANALYSIS.
