Maintenance disclosures
DURING FLIGHT, THE AIRCRAFT WAS LEVEL AT 8000 FEET WITH CRUISE CHECKS COMPLETED AND THE SCHEDULED POWER WAS SET. NO ABNORMAL INDICATIONS WERE OBSERVED BY THE CREW DURING CRUISE AND NO MAJOR POWER ADJUSTMENTS WERE MADE ENROUTE. WHILE STILL AT CRUISE POWER JUST PRIOR TO INITIATING THE DECENT, THE FO BRIEFLY NOTICED THE RIGHT GENERATOR LIGHT APPEARING FOR FEW SECONDS ON THE CWP FOLLOWED BY COMPLETE FAILURE OF THE RIGHT ENGINE. A VISUAL CHECK OF THE RIGHT ENGINE WAS MADE CONFIRMING THAT THERE WAS NO FLAME PRESENT AND THE AUTOFEATHER MECHANISM WAS ENGAGED. THE CREW PERFORMED THE VITAL AND QRH ACTIONS TO SECURE ENGINE, AND ATLANTA CENTER WAS NOTIFIED OF THE FAILED ENGINE. AN EMERGENCY WAS DECLARED. A ONE ENGINE APPROACH AND LANDING WAS COMPLETED WITH NO FURTHER INCIDENT.
AFTER CALLING V1, PILOT NOTICED BOTTOM PANEL RADIO LIGHTS GO OUT. UPON COMPLETING CLIMB CHECKLIST, ENTIRE RT SIDE PANEL LIGHTS WENT OUT & AIR CREW SMELLED SMOKE. AN EMER WAS DECLARED. ACFT WAS CLEARED TO LAND, WHICH IT DID WITHOUT INCIDENT & MET BY AIRPORT FIRE DEPARTMENT. EMERGENCY CREW SCANNED ACFT WITH A THERMAL SCANNER & FOUND NO ISSUES OR CONCERNS. UPON TROUBLESHOOTING FOUND INSTRUMENT PANEL LIGHTING TRANSFORMER TO HAVE FAILED INTERNALLY. THE AFFECTED PART WAS REPLACED & FUNCTION CHECKED. SYS WAS FOUND TO BE IN A SATISFACTORY CONDITION. AWAITING MX WRITE UP & SIGNOFF FOR FAILED PART INFORMATION FOR SECTION 5 BLOCK B C & D.
ON APPROACH, CREW SELECTED LANDING GEAR DOWN. RT MLG SHOWED UNSAFE. CREW ABORTED APPROACH AND PERFORMED QRH PROCEDURES TO ATTEMPT TO REMEDY THE PROBLEM. CREW WERE UNABLE TO GET A GREEN INDICATION FOR THE RIGHT MAIN. CREW DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. IT WAS DETERMINED TO BE AN INDICATION PROBLEM.
ON APPROACH, CREW SELECTED LANDING GEAR DOWN. RIGHT MAIN SHOWED UNSAFE. CREW ABORTED APPROACH AND PERFORMED QRH PROCEDURES TO ATTEMPT TO REMEDY THE PROBLEM. CREW WERE UNABLE TO GET A GREEN INDICATION FOR THE RIGHT MAIN. CREW DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. IT WAS DETERMINED TO BE AN INDICATION PROBLEM.
AFTER TAKEOFF PILOT NOTICED SMELL OF SMOKE. PILOT ASKED COPILOT TO CONFIRM WHICH HE DID. NO SMOKE INDICATION OCCURRED. COPILOT CHECKED CARGO COMPARTMENT AND NOTICED WHITE SMOKE. SMELL OF SMOKE WAS STILL PRESENT. FLIGHT CREW DECLARED AND EMERGENCY AND RETURNED TO DHN. DURING DESCENT THE FLIGHT CREW NOTICED THE SMOKE HAD DISSIPATED. AFTER LANDING THERE WAS NO EVIDENCE OF SMOKE PRESENT. MAINTENANCE FOUND A WEATHER RADAR TRANSCEIVER INOP AND FAINT BURNT SMELL. APPARENTLY SMOKE WAS CONDENSATION, NOT SMOKE.
DURING INSPECTION OF AD 2006-22-12 TWO PROPELLER BOLTS WERE FOUND LOOSE. REPLACED PROPELLER BOLTS AND PROPELLER ASSEMBLY.
THE LT MAIN WHEEL ASSY DEFLATED DURING TAXI AFTER LANDING. A MECHANIC WAS DISPATCHED AND REMOVED THE DEFLATED LT MAIN WHEEL AND REPLACED WITH A SERVICEABLE MAIN WHEEL ASSY IAW MM. THE AIRCRAFT WAS RETURNED TO SERVICE. (K)
THE LT MAIN WHEEL ASSY DEFLATED DURING TAXI AFTER LANDING. MECHANIC WAS DISPATCHED AND REMOVED THE DEFLATED LT MAIN WHEEL AND REPLACED WITH A SERVICEABLE MAIN WHEEL ASSY IAW MM. THE AIRCRAFT WAS RETURNED TO SERVICE. (K)
THE BRAKES WENT OUT ON THE GROUND. THE FLIGHT CREW LOST CONTROL AND HIT ANOTHER AIRCRAFT. THE RIGHT WING TIP HIT THE RT VERTICAL STABILIZER ON OTHER AC. MECHANICS WERE DISPATCHED, THE RT WING TIP WAS REMOVED AND REPLACED. THE BRAKE CONTROL VALVE WAS ALSO REMOVED AND REPLACED AND OPS CHECKED GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
UPON REACHING CRUISING ALTITUDE FROM PLB, THE THROTTLES ON THE RIGHT SIDE WOULD NOT ADVANCE OR RETARD IN-FLIGHT. THE CAPTAIN SHUT DOWN THE NR 2 ENGINE AND LANDED UNEVENTFULLY IN PLB.
