DURING HEAVY CHECK INSPECTION FOUND O/B RIB L/H AILERON WITH CORROSION BEYOND LIMITS
WHEN TAXIING TO THE GATE AND BEFORE ENGINE SHUTDOWN, THE PILOTS EXPERIENCED A STRONG ELECTRICAL ODOR IN THE COCKPIT. FOUND THE PROCESSING UNIT.
WHILE MOVING THROTTLES FORWARD FOR TAKEOFF AT ABOUT 30 PERCENT, LT ENGINE ITT RAISED TO 940 DEGREES FOLLOWED BY THE LT CHIP DETECTOR WARNING LIGHT.
RIGHT HOT PIPE LIGHT CAME ON AFTER TAKEOFF 2,000 FT. R & R WARNING ELECTRONIC COMPUTER.
LEFT ENGINE STAGE 1 ROTOR BLADE MISSING MATERIAL. DAMAGE DISCOVERED VIA BORESCOPE INSPECTION. PICTURE TAKEN AND FORWARDED TO TECH SUPPORT AND THEY DETERMINE THAT ENGINE NEED TO BE REPLACE.
FLAPS LIGHT ILLUMINATED DURING FLIGHT, NO FLAP LANDING PERFORMED AFTER THE QRH COMPLETED. EMERGENCY DECLARED. PERFORMED A DETAIL VISUAL INSPECTION OF FLAPS AND FLAP SYSTEM. FOUND CIRCUIT BREAKER, FLAP CONTROL, TRIPPED OUT. PERFORMED OPT CHECK OF FLAP SYSTEM IAW AMM 27-51-00-05 AND FOUND WORKING PROPERLY.
STRONG ODOR OF BURNED CABLE IN THE COCKPIT DURING CRUISE FLIGHT. DIVERTED ACFT. PERFORMED VISUAL INSPECTION OF THE AVIONICS RACK, AVIONICS FAN, COCKPIT GLARE SHIELD, COCKPIT INSTRUMENTS, OPENED INTERIOR PANELS. NO SIGN OF BURNED CABLE OR WIRES NOTED IAW AMM 20-50-00. ACFT RETURNED TO SERVICE.

NOSE LANDING GEAR LIGHTS INOPERATIVE, GREEN, DOWN AND LOCKED. HAD TOWER VERIFY GEAR WAS DOWN, EMERGENCY WAS DECLARED. R & R BOTH LAMPS ON THE LANDING GEAR DOWN LOCK INDICATOR.
LT AND RT COCKPIT FLOORS HAVE DEEP PITTING CORROSION. REF CONTROL # 350-43 AND MAL MM409 # 404548 FOR REPAIRS.
DENT IN RT HORIZONTAL STAB LOWER SKIN AT HS49.92 THAT`S DISBONDED. REPAIRED IAW TD 340-55-11-04. LT ELEVATOR O/B HINGE DAMAGED BY LOOSE SWAGED BUSHINGS. REQUIRED HINGE FITTING REPLACEMENT. REPLACED IAW TD 340-51-11-15. TWO DISBONDED DENTS ON RT HORZ STAB UPPER SKIN. REPAIRED IAW TD 340-55-11-04.
LT FLAP REAR SPAR CRACKED AT WS 142. REPAIRED IAW TD 340-57-11-09. TWO DISBONDED PREVIOUS BONDED DOUBLERS ON LT FLAP. REPAIRED IAW TD 340-57-11-03.
REAR FUSELAGE FRAME AT STA 710 IN CARGO II BELLY IS KINKED AT TOP LBL 18 TO RBL 3.5. REPAIRED IAW TD 340-53-11-45. LT COCKPIT FLOOR HAS SOME CORROSION PITTING. REPAIRED IAW TD 340-53-11-27. PREVIOUSLY INSTALLED FRAME REPAIR NOT IAW SAAB SRM 53-80-30-201. REMOVED EXISTING REPAIR AND COMPLETED REPAIR IAW TD 340-53-11-44. FOUND TOOLING MARKS TO LT UPPER TAIL CONE FORWARD OF FS 803 AND BELOW SL 1504. INSTALLED DOUBLER REPAIR IAW TD 340-51-11-16 REF CONTROL NUMBER 341-62.
DURING THE APPROACH PHASE THE CREW DID NOT GET ANY INDICATION THAT THE FLAPS ARE EXTENDING. RETURNED TO IAD FOR MAINTENANCE. MX FOUND C/B POPPED, RESET AND WENT THROUGH OPERATIONAL CHECK OF THE FLAP SYSTEM AND ALL OPS CHECKED GOOD.
RETURNED TO FLIGHT DUE TO RIGHT ENGINE TORQUE FLUX. FOUND FLUX TO BE ONLY WITH PROP SYNC ON. PLACED PROP SYNC ON MEL.
LEFT TAIL PIPE HOT LIGHT ILLUMINATED. CLEANED AND DRIED CONNECTORS DUE TO HEAVY RAIN.
DURING CRUISE CREW NOTICED THAT DEICE BOOT SYSTEM STOPPED WORKING. AS FLIGHT WAS ENTERING INTO ICING CONDITION, CREW CONTACTED DISPATCH AND IT WAS AGREED TO DIVERT BACK TO IAD. THE FLIGHT LANDED WITHOUT FURTHER INCIDENT. DRAINED WATER TRAPS & LT MID WING APPLIED 3 PATCHES.
SMOKE IN COCKPIT DURING DESCENT. NO ABNORMAL INDICATIONS AND FLIGHT ATTENDANT DID NOT SMELL THE SMOKE. CHECKED RT & LT ENG FOR OBVIOUS DAMAGE, NO FAULT FOUND. CHECKED BOTH RT & LT ACM OILS, FOUND FULL. CHECKED FOR POPPED C/B, NONE FOUND. RAN RT ENG FOR APPROX 20 MIN WITH BLEED AIR ON, ACM RUNNING WAS A SLIGHT SMOKE/WARM AIR SMELL. RESIDUAL FROM DISCREPANCY NR 8 LOG PAGE 168096. (ENG CHANGE) RAN LT ENG, NO FAULTS NOTED. RETURNED AIRCRAFT TO SERVICE.
FOUND LT ENG IGNITION ILLUMINATED WITH TQ FLUX FULL SCALE AND ENGINE SURGING. REPLACED LEFT TORQUE SPEED SENSOR IAW GE MM 74-00-00, PN: 5066T92G05SN ON: 1253B, SN OFF: 51M0190B. REPLACED LEFT ENG DECU IAW GE MM CH 74-00-00. OPS CHECK GOOD. NO PROBLEMS NOTED, PN: 6071T20P16, SN ON: ECOT1599 SN OFF: ECOT1613.

DURING TAKEOFF ROLL, NO INDICATION ON CAPT'S AIRSPEED INDICATOR, F/O'S READING 70 KTS. RESEATED AIRSPEED INDICATOR CONNECTOR, OPS CHECK GOOD IAW 34-12-00-05.
LANDING GEAR CONTROL CIRCUIT BREAKER (F7) WAS FOUND POPPED AFTER GEAR WAS SELECTED DOWN. RESET C/B AND GEAR CYCLED NORMALLY. INSPECTED LT AND RT MAIN LANDING GEAR HARNESS, INSPECTED FOR LEAKS, SWITCH UPLOCK, ALSO INSPECTED NOSE LANDING GEAR HARNESS AND SWITCH FOR ANY DEFECTS. VERIFIED THREE GREEN LIGHT INDICATION IN COCKPIT, INSPECTED C/B F7 NO FAULTS FOUND.
LANDING GEAR CONTROL C/B (F7) WAS FOUND POPPED AFTER GEAR WAS SELECTED DOWN. CB WAS RESET AND GEAR CYCLED NORMALLY. CB WAS DISCOVERED OPEN AFTER MANUAL HIT C/B PANEL. INSPECTED MAIN LANDING GEARS LT AND RT FOR ANY LEAKS, WIRE HARNESS, SWITCH UPLOCK. ALSO INSPECTED NOSE LANDING HARNESS AND SWITCH FOR ANY DEFECTS. VERIFIED THREE GREEN LIGHTS INDICATION IN COCKPIT, AND INSPECTED C/B F7 NO FAULTS FOUND.
INFLIGHT RT TORQUE INDICATOR DROPPED TO ZERO AND STAYED AT ZERO FOR THE REMAINDER OF THE FLIGHT. ALL OTHER INDICATIONS NORMAL. R&R RT TORQUE INDICATOR, PN OFF 9302472006, PN ON 10225N01W00, SN OFF 09880445, SN OFF 09880445. OPS CHECK GOOD.
AFTER TAKEOFF TAIL PIPE HOT LIGHT ILLUMINATED. FOUND MOISTURE IN TWO CONNECTORS ON TAIL PIPE AND CLEANED CONNECTORS. OPS CHECKED GOOD IAW 26-12-00
DURING T/O ROLL, BOTH TORQUE GAUGES LEAPED TO ABOUT 90 PERCENT AS POWER WAS APPLIED. CTOT SELECTED TO APR, ONLY THE L APR LIGHT ILLUMINATED. PERFORMED ENGINE OPS CHECK, AND HIGH POWER RUNS WITH CREW, APR AND CTOT OPS CHECK GOOD AT THIS TIME. NO DEFECTS NOTED IAW 76-14-01.
AUTOCOARSEN CAUTION LIGHT ILLUMINATED ON TAKEOFF ROLL. REMOVED AND REPLACED AUTOCOARSEN COMPUTER PN ON/OFF 8542M2, SN ON 538, SN OFF 435 IAW 61-22-05 AND DID NOT FIX IT. ON 01/18/2006 MEGGED OUT WIRING BETWEEN AUTOCOARSEN COMPUTER AND P3 SENSORS, ENGINE TORQUE SENSORS, ALL WIRING CHECKED GOOD. REMOVED AND REPLACED THE AUTOCOARSEN SWITCH CONTROL UNIT IAW 61-22-15-2 PN ON/OFF 8581, SN ON 213, SN OFF 223. SYSTEM OPS CHECK GOOD.
LEFT PROP RPM DROPPED 300 RPM IN FLIGHT, UNABLE TO SYNC, AFTER LANDING. LT PROP RPM DROPPED TO 0. REMOVED AND REPLACED PROP RPM GAUGE PN ON/OFF 10223N0, SN ON 237, SN OFF 165 OPS CHECK GOOD.
ON TAKEOFF ROLL, LEFT ENGINE WENT INTO AUTOFEATHER. PERFORMED CWC AUTOCOARSEN TEST, OPS CHECK GOOD.
LT PROPELLER FEATHERED DURING TAKEOFF ROLL.
NO TORQUE INDICATION ON LEFT ENGINE DURING TAKEOFF ROLL.
