Maintenance disclosures
ONCE WERE ON VISUAL APPROACH IN ANCHORAGE, I SELECTED THE GEAR LEVER DOWN. THE PLANE'S NOSEWHEEL FAILED TO ILLUMINATE GREEN. AFTER VERIFYING THAT IT WASN'T A BULB BURNT OUT, WE PERFORMED A FLYBY, AND TOWER STATED THAT THE GEAR APPEARED TO BE DOWN. WE CYCLED THE GEAR TWICE MORE WITH SAME RESULTS. AFTER THIS, I MANUALLY PUMPED DOWN THE GEAR, AT WHICH TIME THE NOSEWHEEL LIGHT INTERMITTENTLY BLINKED, BUT FAILED TO STAY ON. AN EMERGENCY WAS DELCARED, AND WE LANDED ON RWY 15 WITHOUT INCIDENT. CONSIDERING THAT THE NOSEWHEEL WAS POTENTIALLY UNLOCKED, WE ELECTED TO STOP ON TAXIWAY MIKE AND REQUESTED A TOW TO THE HANGAR.
THE NOSEWHEEL GEAR DOWN INDICATOR LIGHT FAILED TO ILLUMINATE. THE ACTUAL NOSEWHEEL HAD COME DOWN AND LOCKED, BUT WITH NO INDICATION TO THE PILOT OF ITS STATUS, AND EMERGENCY WAS DECLARED. THE PLANE LANDED WITHOUT INCIDENT.
AIRCRAFT DEPRESSURIZED AT 23,000 FEET. WOULD NOT PRESSURIZE AFTER THAT. MADE A "PAN PAN" CALL TO ATC AS I PERFORMED AN EMERGENCY DECENT AND DONNING MY OXYGEN MASK. AIRFRAME FWD UPPER LUG WELD ASSEMBLY BROKEN.
THE LANDING GEAR FAILED TO EXTEND ON APPROACH. I WENT MISSED AND EXERCISED THE GEAR HANDLE. I DECIDED TO PROCEED TO KOTZEBUE AND COMMENCE THE ABNORMAL OPERATIONS MANUAL GEAR EXTENSION. WITH A SUCCESSFUL DOWN/LOCK INDICATION I DID THE ILS APPROACH TO LANDING. WE WERE UNABLE TO PICK UP THE PATIENT. ANC CENTER DECLARED EMERGENCY AND FIRE TRUCK WAS BROUGHT OUT IN KOTZEBUE. MAINTENANCE INSPECTED LANDING GEAR SYSTEM, RESET CIRCUIT BREAKER, PERFORMED MULTIPLE GEAR SWINGS WITH PROPER INDICATIONS. NO DEFECT NOTED. SERVICED HYDRAULIC RESERVOIR IAW KING AIR 200 MM CH. 32.
PILOT REPORTS "LOSS OF ELETRICAL POWER AFTER TAKEOFF - I NOTICED HSI HEADING WAS OFF 30 DEGRESS GIVING ERRONEOUS INDICATION. SHORTLY THEREAFTER I NOTICED MY AVIDYNE MOVING MAP HAD GONE BLACK THEN MOST MY INSTRUMENTS GAVE FLAGS AND RADIOS WENT DARK AS WELL. I THEN REALIZED I HAD A COMPLETE LOSS OF ELECTRICAL POWER AND TRIED TO RESET GENERATORS UNSUCCESSFULLY BOTH SHOWING 0/0 VOLTS ON AMMETER. I ALSO TRIED TO RESET BATTERY MASTER AND WAS STILL UNABLE TO REGAIN ANY ELECTRICAL POWER TO AIRCRAFT." PRECAUTIONARY MEASURES TAKE: UPON REALIZING LOSS OF ELECTRICAL POWER, TRIED TO RESET AND PUT GENERATORS BACK ONLINE UNSUCCESSFULLY. ONCE DECISION WAS MADE TO RETURN TO BASE, I RAN MANUAL EXTENSION OF GEAR CHECKLIST AND DID A PRECAUTIONARY LOW PASS OVER OTZ AND HAD FSS VERIFY GEAR DOWN BEFORE I PROCEEDED TO LAND. MADE WIDE STABILIZED APPROACH AND MADE SOFTEST LANDING POSSIBLE TO INSURE LESS STRESS ON LANDING GEAR.
FAR 1135.415 REPORT. PILOT REPORTS "LH GENERATOR DROPPED OFFLINE CAUSING A MASTER CAUTION. TRIED ONE RESET, BOTH VOLTMETERS STARTED FLUCTUATING. TURNED LH GEN BACK OFF AS PER CHECKLIST". MAINTENANCE INSPECTED LH STARTER/GENERATOR AND CONTROL SYSTEM. WHEN CENTER FLOORBOARD WAS REMOVED FOR ACCESS, FOUND LIQUID ON THE LH GCU AND SURROUNDING AREA OF THE BELLY FROM SUSPECTED SPILL IN THE CABIN. REMOVED LH GCU, CLEAN LOCAL AREA AND GCU CONNECTOR. REPLACED LH GCU, OPERATIONAL CHECKS SATISFACTORY.
