ATA 2436

ON CLIMB OUT, THE PILOT REPORTED THAT THE LT ALTERNATOR AMPS WENT TO ZERO AND THE RT ALTERNATOR ILLUMINATED. THE PILOT RESET AND THE RT ALTERNATOR THE AMP INDICATION WENT TO 70 AMPS AND FAILED AGAIN. THEN THE LT ALTERNATOR ILLUMINATED AS BOTH ALTERNATORS WENT OFF LINE. THE PILOT THEN WAS ABLE TO RESET RT ALTERNATOR AND IT REMAINED ON LINE. THE LT ALTERNATOR WOULD NOT RESET. THE PILOT DECLARED AN EMERGENCY AND RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. MAINTENANCE TROUBLESHOOTING DETERMINED THAT THE LT VOLTAGE REGULATOR WAS THE SOURCE OF THE FAULT. R & R THE LT VOLTAGE REGULATOR, PN B-00286-1, SN AR-14586. ADJUSTED UNIT VOLTAGE PARALLELED TO RT VOLTAGE REGULATOR. GROUND RUN OPS CHECK WAS NORMAL.

AFTER TAKEOFF, THE PILOT REPORTED THE RT ENG WAS VIBRATING, RUNNING ROUGH & LOW ON POWER. AFTER MONITORING THE ENG GAUGE’S THE PILOT OBSERVED THE RT ENG OIL PRESS WAS AT THE BOTTOM OF GREEN ARC & TOP OF YELLOW ARC. THE PILOT DECIDED TO PERFORM A PRECAUTIONARY IN FLIGHT SHUTDOWN OF THE RT ENG, RETURNED & LANDED WITHOUT INCIDENT. TROUBLESHOOTING REVEALED A FAULTY RT ENG RT MAGNETO. REPLACED THE RT ENG, RT MAGNETO. PERFORMED ENG RUN & LEAK CHECKS. ALL CHECKS WERE NORMAL, OBSERVED OIL PRESS ON THE RT ENG WAS NORMAL.

NOTIFIED CONTROL TOWER OF THE LG MALFUNCTION, PERFORMED MANUAL EXTENSION OF LG. AFTER EXTENSION PILOT RECEIVED THREE GREEN DOWN & LOCKED INDICATOR LIGHTS & LANDED WITHOUT FURTHER INCIDENT. INSPECTION REVEALS A HOLE IN HYD PRESS LINE, PN 40818-00 FOR HYD ENG PUMP PRESS LINE, BEHIND FIREWALL IN RT NACELLE. R & R HYD LINE, ALSO CHANGED BOTH LT & RT HYD FILTERS & BOTH HYD PUMPS. OPERATIONAL CHECKS OF HYD SYS WERE NORMAL.

SHORTLY AFTER RECEIVING CLEARANCE FOR LANDING, THE PILOT NOTICED THAT THERE APPEARED TO BE FLAMES COMING FROM INSIDE THE RT ENGINE, NEAR THE INBD EXHAUST MANIFOLD. THE PILOT IMMEDIATELY DECLARED AN EMERGENCY AND REQUESTED THE FIRE TRUCKS BE DISPATCHED. IMMEDIATELY AFTER TOUCHDOWN THE PILOT SHUT DOWN THE NR 2 ENGINE. AFTER EXITING THE RUNWAY ONTO THE TAXI WAY THE PILOT PERFORMED A PRECAUTIONARY SHUT DOWN THE OF LT ENGINE AND EVACUATED THE ACFT. MX INSPECTED THE RT ENGINE AND FOUND THE HYDRAULIC FILTER BOWL THAT MOUNTS TO THE INBD SIDE OF THE FIREWALL CRACKED AT THE THREADED PORTION OF THE BOWL CAUSING THE HYDRAULIC FLUID TO LEAK ONTO THE EXHAUST MANIFOLD. FILTER BOWL WAS REPLACED.
