AIR RETURN BACK TO LAX DUE TO ENGINE 4 SURGE. ENGINE 4 SURGED MULTIPLE TIMES DURING CRUISE, SURGE WAS INTERMITTENT. PERFORMED SURGE CHECKLIST. REMOVED AND REPLACED VSV ACTUATOR LEVER PIVOT BRACKET & BOLT/STUD AS PER AMM 72-36-00. OPS CHECK GOOD.
ENGINE 4 FUEL FILTER EICAS INTERMITTENT. ENGINE 4 SURGES INTERMITTENTLY.
NOSE LANDING GEAR WOULD NOT RETRACT, GEAR DISAGREE AND GEAR DOOR EICAS ON DEPARTURE. AIR RETURN.
"GEAR DOOR" EICAS MESSAGE ON DEPARTURE - WITH VIBRATION CYCLED GEAR - GOT "GEAR DISAGREE" MESSAGE. NOSE GEAR NOT DOWN. LOCKED ALL LANDING GEAR IN DOWN POSITION AND TRANSFERRED TO DMI #11558 IAW MEL 32-30-01 PLACARDS INSTALLED. REPOSITIONED TO KOSC FOR REPAIRS.
AFTER TWO HOURS OF FLIGHT, CREW OXYGEN LOW MESSAGE OXYGEN CONTINUOUSLY DECREASED DURING FLIGHT AS LOW AS 370 PSI. REPLACED OXYGEN LINE AT THE BOTTLE. LEAK CHECK PERFORMED IAW AMM 35-11-00.
TENSION TIES FRAME STA 1160&1200 BETWEEN L/RBL48-L/RBL70 STA 1200-THE DWG REQUIRES 47 FASTENERS HOLES DIA 1/4" ON EACH SIDE BUT 55 HOLES WERE DRILLED. STA 1160-THE DWG REQUIRES 53 FASTENERS HOLES DIA 1/4" ON EACH SIDE BUT 59 HOLES WERE DRILLED
UPPER DECK RT ESCAPE SLIDE INADVERTENTLY DEPLOYED. SLIDE DID NOT FULLY DEPLOY. DOOR ARMED, NO MECHANICAL DEFECT. REMOVED SLIDE AND DEFERRED SYSTEM.
THE FLIGHT CREW REPORTED, THROUGHOUT FLIGHT AN INDICATION OF ANTI-SKID FAULT AND NR 4 HYDRAULIC SYSTEM LOW QUANTITY, DECLARED EMERGENCY LANDING INTO ANC.
