DURING INSPECTION IT WAS FOUND THE NR 3 CYLINDER ON THE LEFT ENGINE HAD SCORING ON THE CYLINDER WALLS CAUSED BY A BROKEN OIL CONTROL RING. THE CYLINDER WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED DUAL ALTERNATOR FAILURE IN FLIGHT. USED EMERGENCY: BACK UP ESSENTIAL PATH AND BATTERY DID NOT PROVIDE POWER. BOTH ALTERNATORS, LH/RH POPPED CIRCUIT BREAKERS. MAINTENANCE INSPECTED AND NOTED THE LEFT ALTERNATOR PULLEY WAS LOOSE AND THE ALTERNATOR BELT DAMAGED. THE BELT WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED RIGHT ALTERNATOR CIRCUIT BREAKER POPPED WHICH RESULTED IN COMPLETE ELECTRICAL FAILURE, EMERGENCY BATTER WAS USED AND THE AIRCRAFT DIVERTED AND LANDED SAFELY. MAINTENANCE INSPECTED AND FOUND THE LEFT ALTERNATOR BELT WAS LOOSE. TIGHTENED THE BELT AND PERFORMED MULTIPLE OPS CHECKS WITH NO FURTHER DEFECTS NOTED.
DURING ROUTINE INSPECTION THE NUMBER ONE CYLINDER THROUGH STUD WAS FOUND TO BE SHEARED. THE ENGINE IS BEING REPLACED.
PILOT REPORTED LEFT ENGINE FIRE CASS MESSAGE IN FLIGHT. THE PROPELLER WAS FEATHERS & ENGINE WAS SHUT DOWN AS A PRECAUTION. MAINTENANCE INSPECTED AND FOUND NO DAMAGE. THE LT ENGINE #2 FIRE DETECTOR WAS DETERMINED TO BE FAULTY. DETECTOR WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PWM - FLT 4204 - AFTER TAKEOFF WHEN GEAR WAS SELECTED UP, THE HYDRAULIC QUANTITY STARTED TO DROP. CREW SELECTED GEAR AND FLAPS DOWN AND RETURNED TO THE AIRPORT. LANDING WAS ACCOMPLISHED WITHOUT INCIDENT. MAINTENANCE FOUND HYDRAULIC LINE GOING TO NOSE GEAR RETRACT ACTUATOR LEAKING. LINE WAS REPLACED AND SYSTEM CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE. (X)

LANDING GEAR INTRANSIT LIGHT REMAINED ON AFTER GEAR RETRACTION. COULD NOT DUPLICATE DISCREPANCY. INSPECTION AND OPERATIONAL CHECKS OF LANDING GEAR SATISFACTORY.
STRONG FUEL SMELL IN AIRCRAFT AFTER TAKEOFF. REPLACED THE RT ENGINE FUEL CONTROL UNIT. PART TOTAL CYCLES, 23,980.
MAIN PITCH TRIM STUCK IN FULL DOWN POSITION. APPLIED WARM GLYCOL TO TRIM ACTUATOR, OPS CHECK GOOD.
LEFT PROPELLER RPM INDICATED 1380 DURING TAKEOFF ROLL. COULD NOT DUPLICATE DISCREPANCY. 1380 RPM IS IN NORMAL OPERATING RANGE.
HYDRAULIC FLUID LEAKING FROM NOSE GEAR. REPLACED NOSE GEAR RETRACT ACTUATOR.
CAPTAINS EADI WENT BLACK. REPLACED NR 1 DPU.
DURING TAKEOFF AND LANDING THE MAX PROPELLER RPM IS 1380. REPLACED THE RT PROPELLER CONTROL UNIT.
RT ENGINE ITT REACHED 945 DEGREES DURING TAKEOFF. REPLACED THE RT ENGINE.
FORWARD SECTION CABIN EMERGENCY LIGHTS ARE INOP. REPLACED EMERGENCY LIGHTS BATTERY PACKS.
MECHANIC REMOVED WHEEL ASSY DUE TO TIRE WORN TO LIMITS. DURING VISUAL INSPECTION FOUND INNER RACE SPUN. SUBASSY HAS BEEN REMOVED FROM SERVICE AND SENT TO ABS FOR REPAIR.
RIGHT ENGINE ITT REACHED 931 DEGREES DURING TAKEOFF. REPLACED THE RT ENGINE.
LEFT AIR VALVE LIGHT STAYS ON WITH SWITCH IN OFF POSITION. REPLACED THE WARNING ELECTRONIC UNIT.
RT PROPELLER BLADE, ERROSION GUARD, IS DEBONDING ON BLADE NR 4. REPLACED THE PROPELLER ASSY.
RT BLEED AIR FAULT CAUTION LIGHT ILLUMINATED DURING CLIMB. REPLACED THE BLEED AIR PRESSURE SWITCH AND THE BLEED AIR OVERTEMP SWITCH FOR TROUBLESHOOTING. PROBLEM DID NOT DUPLICATE IN MAINTENANCE.
NR 1 COMM RADIO WILL RECEIVE, BUT WILL NOT TRANSMIT. COULD NOT DUPLICATE - NR 1 COMM RADIO SYSTEM TEST OK.
CON FIG WARNING, DURING TAKEOFF ROLL. REPLACED THE WARNING ELECTRONIC UNIT AND ACFT RETURNED TO SERVICE.
FLAP FAULT ON CAUTION WARNING PANEL. PERFORMED FLAP CONTROL SYSTEM RIGGING AND ADJUSTMENT.
CONDITION LEVERS FRICTION LOCK DOES NOT HOLD SUFFICIENTLY. REPLACED THE CONTROL QUADRANT.
DURING FLIGHT, VERY NOTICEABLE VIBRATION IN CONTROL YOKE. AILERONS SEEM TO HAVE DELATED RESPONSE TO RT ROLL INPUT COMMAND. INSPECTED AILERON CONTROL SYSTEM AND FOUND NO DEFECTS. REPLACED WORN BEARINGS ON RT ELEVATOR TRIM TAB.
RIGHT PROPELLER COMES OUT OF FEATHER VERY SLOW. ACTION: FERRIED AIRCRAFT TO CVG, FOUND RIGHT PROPELLER OVERSPEED GOVERNOR BAD. REMOVED AND REPLACED GOVERNOR WITH SERVICEABLE UNIT.
RIGHT PROPELLER VERY SLOW COMING OUT OF FEATHER. THE OVERSPEED TEST DID NOT FUNCTION NORMAL. REPLACED THE RIGHT PROPELLER OVERSPEED GOVERNOR.
AUTO COARSEN WARNING ON CWP. FLIGHT RETURNED TO EVV. REPLACED THE AUTO COARSEN P3 PRESSURE SWITCHES.
CRACK INDICATION FOUND DURING SCHEDULED AD 93-19-01 INSPECTION. PROPELLER WAS DISASSEMBLED AND HUB S/N CW1024 REMOVED FROM SERVICE.
