DURING PERIODIC INSPECTION, A 0.125" CRACK WAS DISCOVERED BY DYE PENETRANT IN THE LOWER INBD FLANGE RADIUS OF THE LT TRUNNION ASSY, LANDING GEAR AFT.
DURING THE PERIODIC INSPECTION, A 0.125 INCH CRACK WAS DISCOVERED DURING DYE PENTRANT IN LOWER INBD FLANGE RADIUS OF THE LT TRUNNION ASSY -LANDING GEAR AFT.
PILOT COMPLAINED OF WING DE-ICE BOOTS STAYING INFLATED. REMOVED VALVE AND FOUND IT STUCK IN INFLATION MODE DUE EXCESSIVE SPACE BETWEEN THE GASKETS. THIS ALLOWS AIR PRESSURE TO VENT WHICH IS THE ACTUATION MODE FOR INFLATION. IT APPEARS THAT THE SQUEEZE ON THE RIVETS MAY BE TOO LIGHT. PART WAS REPLACED.
DURING INSPECTION OF THE HYDRAULIC POWER PACK IT WAS DISCOVERED THAT THE CHANNEL THAT REINFORCES THE NOSE WHEEL TUNNEL WHERE THE NOSE GEAR ACTUATOR ATTACHES HAD CRACKED ON THE FORWARD CUTOUT FOR THE HYDRAULIC POWER PACK RELIEF VALVE THE CHANNEL WAS REMOVED AND INSPECTED WHERE IT WAS DISCOVERED THAT THE OUTBOARD BOLT HOLE FOR THE NOSE GEAR ACTUATOR BRACKET WAS ELONGATED AND CRACKED ON THE BOTTOM SIDE THE PART WAS REMOVED AND REPLACED. IT IS RECOMMENDED THAT THE NOSE GEAR ACTUATOR ATTACH BRACKET BOLTS BE CHECKED PERIODICALLY FOR SECURITY DURING LANDING GEAR INSPECTIONS AS IT IS SUSPECTED THAT THE LOOSE BOLT CAUSED THE PART TO STRESS AND CRACK DURING GEAR OPERATIONS.
PILOT TAXIED OUT TO RUNWAY AND REPORTED NOSE TIRE WENT FLAT AIRCRAFT WAS RECOVERED. AFTER INSTALLING NEW WHEEL ASSEMBLY IT WAS FOUND THAT THE TUBE HAD A HOLE IN THE CENTER. THIS IS THE THIRD INCIDENT WITH THIS TUBE MANUFACTURER. SUSPECT THE MATERIAL, A BUTYL RUBBER, A FACTOR WHEN COMBINED WITH LOW WINTER TEMPERATURES.(K)
DURING INSPECTION IT WAS NOTICED THAT THE RIGHT FUEL PUMP WAS LEAKING AT THE BODY GASKET THE PUMP WAS REMOVED FROM SERVICE. THIS IS A M STYLE PUMP.(K)
PILOT REPORTED THAT IN CRUISE THE RIGHT ENGINE MANIFOLD PRESSURE DROPPED 2 INCHES, THE OIL TEMPERATURE STARTED TO RISE WITH THE OIL PRESSURE REMAINING IN THE GREEN. CHART WAS WITHIN LIMITS, AT THIS POINT HE DECIDE TO DO A PRECAUTIONARY SHUT DOWN OF THE ENGINE. HE ATTEMPTED TO FEATHER, THE ENGINE APPARENTLY SEIZED AND THE PROP FEATHERED. THE A/C LANDED WITHOUT INCIDENT. IT APPEARS THAT EITHER NR 5 CYLINDER CONNECT ROD OR POSSIBLY THE CRANKSHAFT BROKE CAUSING THE ENGINE TO SEIZE. THE CYLINDER HAD STARTED TO SEPARATE FROM THE CASE, CRACKING IT THROUGH THE INTAKE/OIL SUMP ASSEMBLY AND THE ACCESSORY SECTION CASE WAS ALSO CRACKED . CAUSE OF THE FAILURE: TO BE DETERMINED AT BREAKDOWN BY THE MANUFACTURER.
PILOT TAXIED OUT TO RUNWAY AND REPORTED NOSE TIRE WENT FLAT. AIRCRAFT WAS RECOVERED AFTER INSTALLING NEW WHEEL ASSEMBLY. IT WAS FOUND THAT THE TUBE HAD A HOLE IN THE SIDE WALL OPPOSITE THE VALVE STEM. THIS IS THE SECOND FLAT RELATED TO A TUBE.
DURING VALVE CLEARANCE CHECK, INTAKE VALVE 3 AND 4 EXHIBITED EXCESSIVE LIFT. NR 4 CYLINDER WAS REMOVED AND DAMAGE OBSERVED ON 3 AND 4 INTAKE LOBES OF CAMSHAFT AND TAPET BODIES. ENGINE WAS REMOVED FROM SERVICE.
DURING ROUTINE INSPECTION OBSERVED FUEL LEAK FROM PRESSURE REGULATOR CONTAINMENT VESSEL. REMOVED CONTAINMENT VESSEL AND FOUND FUEL LEAKING FROM PARTING SURFACE AT THE TOP OF THE REGULATOR.
ON POST-FLIGHT INSPECTION, PILOT NOTICED OIL ON UNDER SIDE OF ENGINE COWLING AND NOTIFIED MAINTENANCE PERSONNEL. FURTHER INVESTIGATION REVEALED OIL DRIPPING FROM INDUCTION HOUSING DRAIN AND WAS DETERMINED TO BE COMING FROM THE SENSE LINE OUT OF THE DIFFERENTIAL PRESSURE CONTROLLER. (X)
DURING DESCENT TO LANDING, PILOT NOTICED MANIFOLD PRESSURE STARTING TO DECREASE, FUEL FLOW INCREASE AND CYLINDER HEAD TEMP DECREASE AND THE EXHAUST GAS TEMP DECREASE. AFTER LANDING AND ON THE GROUND, THE ENGINE WOULD ONLY PRODUCE 2,200 RPM AND 28 INCHES OF MANIFOLD PRESSURE. THE OIL SCREENS WERE CHECKED WITH AN EXCESIVE AMOUNT OF STEEL AND ALUMINUM SHAVINGS FOUND. THE ENGINE WAS REMOVED FROM SERVICE FOR A TEAR DOWN INSPECTION.
LANDING GEAR WOULD NOT EXTEND WITHOUT USING EMERGENCY HAND PUMP. FOUND HYD LEAK AT LT HYD FILTER ASSY. O-RING WAS BLOWN AND UPON REMOVING FILTER BOWL, THREADS ON FILTER BOWL WERE FOUND BROKEN. SUSPECT CAUSE THREADS SEPARATED FROM METAL FATIGUE OR OVERTORQUING.
