DURING FLIGHT, THE CREW REPORTED LT DC GEN AND FLAP CONTROL FAIL. FLAPS STUCK EXTENDED TO 3 DEGREES. THE CREW PERFORMED AN AIR-TURN-BACK. BRAKES USED HEAVILY ON LANDING AND BRAKE TEMP HIGH MESSAGE RESULTED. MAINTENANCE INSPECTED THE AC. THE LT DC GEN COULD NOT DUPLICATE THE PROBLEM. THE FLAP SYTEM AFTER RESETTING THE FLAP CONTROL UNIT COULD NOT DUPLICATE THE PROBLEM. WHILE INSPECTING THE BRAKES IT WAS DETERMINED THAT THE NR 1 BRAKE WAS WORN TO LIMITS. DURING A FLIGHT THE LT DC GEN FAILED AFTER CYCLING THE GEN THE GENERATOR OPS CHECKED GOOD. MAINTENANCE LATER ON, MAINTENANCE REPLACED THE LT HALL EFFECT DEVICE (HED) FOR TROUBLESHOOTING. (K)
DURING FLIGHT, CREW REPORTED AN AMBER CAS MESSAGE FOR BOTH LT AND RT FUEL QUANTITY INDICATORS. THE CREW CONSULTED WITH MECHANIC AND MADE AN AIR-TURN-BACK. MECHANIC REPLACED THE FUEL CONTROL PANEL AND ALL OPERATIONAL CHECKS WHERE GOOD. (K)
DURING TAKEOFF OF FLIGHT, THE CREW REPORTED LOW PRESSURE BLEED SOV FAIL CAS MESSAGE AFTER POWER UP FOR TAKE OFF. THE CREW ABORTED THE TAKEOFF AND RETURNED TOT HTE GATE. MECHANIC INSPECTED THE SYSTEM AND REPLACED LT PACK FLOW CONTROL VALVE BUT THE SYSTEM STILL OPS CHECKED BAD. MECHANIC THEN SWAPPED THE LT AND RT LOW PRESSURE VALVES (PRSOV) AND OPS CHECKED GOOD. MECHANIC RETURNED THE AC TO SERVICE. AS OF 05/11/05, PROBLEM HAS NOT RETURNED. (K)
APPROXIMATELY 80 KNOTS RECEIVED AN ENGINE ANTI-ICE OVERHEAT AND A SINGLE WARNING CHIME. CREW ABORTED TAKEOFF AND MESSAGE CLEARED DURING ROLLOUT. CREW UNABLE TO DUPLICATE THE PROBLEM AND THE A/C WAS CLEAR TO LAUNCH. DURING FLT 2265, MKE-BNA, THE SAME PROBLEM OCCURRED. THE CREW MADE AN AIR TURN BACK TO MKE. MAINTENANCE REPLACED THE BLEED AIR SHUTOFF VALVE AND WITH ALL OPERATIONAL CHECKS WERE GOOD THE A/C WAS RETURNED TO SERVICE. LATER THAT DAY BEFORE FLIGHT 2127 - CLE-MKE, THE SAME PROBLEM OCCURRED. THE CREW ABORTED THE TAKEOFF AND AGAIN THE MESSAGE CLEARED AND WAS UNABLE TO BE DUPLICATED. ON RETURN TO MKE, MAINTENANCE INSPECTED THE RIGHT ANTI-ICE VALVE AND CONNECTOR, ALL OPERATIONAL CHECK WERE GOOD. (M)
FLT 2380 - MKE-PIT - DURING FLIGHT, THE CREW NOTICED A LEFT GENERATOR OVERHEAT CAS MESSAGE, CONSULTED WITH MX BUT WAS UNABLE TO GET THE MESSAGE TO CLEAR. THE CREW MADE AN AIR TURN BACK TO MKE. MAINTENANCE INSPECTED THE A/C AND REPLACED THE LEFT GENERATOR, AFTER OPERATIONAL CHECKS WERE GOOD THE A/C WAS RETURNED TO SERVICE WITH NO OTHER DISCREPANCIES NOTED. (M)
DURING FLIGHT, CREW REPORTED A LOAD BANG FROM AFT OF THE FLIGHT DECK FOLLOWED BY A FAINT SMELL OF SMOKE. THEY DONNED OXYGEN MASKS AND REQUESTED PRIORITY, BUT DID NOT SHUTDOWN THE ENGINE OR DECLARE AN EMERGENCY. THE CREW PERFORMED AN UNEVENTFUL LANDING. MX INSPECTED THE ENGINE AND FOUND THE NR 2 COMPRESSOR FAN HAD ONE BLADE THAT APPEARED TO HAVE LOST AND SECTION OF ITS LEADING EDGE AND 3 OTHER BLADES WERE DAMAGED. FANS AFT OF THE NR 2 COMPRESSOR HAD SEVERAL DAMAGED BLADES MOSTLY LIKELY FROM THOSE PIECES GOING THROUGH THE ENGINE. THE ENGINE WAS REMOVED AND SENT TO MFG FOR SERVICE.(K)
BEFORE FLIGHT, THE CREW NOTED A CAS MESSAGE FOR LT BLEED SOV FAIL AND LT PACK FAIL. THE CREW ABORTED THE TAKEOFF AND RETURNED TO THE GATE. MX INSPECTED AND OPERATED THE SYSTEM BUT COULD NOT DUPLICATE THE DISCREPANCIES. MX REPLACED THE LT PRESSURE REDUCING VALVE (BOV) FOR TROUBLESHOOTING AND RETURNE THE AIRCRAFT TO SERVICE. (K)
WHILE TAXIING OUT FOR FLIGHT 2419, ATW-MKE, THE CREW NOTED A LOUD NOISE SOUNDING LIKES BRAKES. DETERMINED THT THE RIGHT ENGINES COMPRESSOR WAS STALLING WHEN THE POWER WAS ADVANCED. MAINTENANCE TROUBLESHOT THE SYSTEM AND REPLACE MOST OF THE BLEED SYSTEM COMPONENTS BUT THE COMPRESS STILL STALLED WHEN ADVANCING POWER. MAINTENANCE BORESCOPED THE ENGINE AND DEEMED IT SAFE FOR A FERRY FLIGHT BACK TO MKE. THE ENGINE WAS CHANGED TO CORRECT THE PROBLEM. (M)
DURING APPROACH OF FLIGHT 2288 MKE-IAD THE CREW SELECTED GEAR DOWN BUT NOSE GEAR INDICATOR DID NOT ILLUMINATE. THE CREW HAD THE FIRE TRUCK ROLL AS A PRECAUTION. THE CREW MADE AN UNEVENTFUL LANDING. MECHANIC REPLACED THE BULBS AND RELEASED THE AIRCRAFT.
