RT MAIN GEAR INDICATOR LIGHTS ARE OUT, REPLACED THE LAMPS AND OPS CHECKED GOOD.
DURING CHECK, EMERGENCY LIGHTING POWER SUPPLY FAILED AT LOCATION 24LN. R & R POWER SUPPLY.
LEFT SIDE EMERGENCY EXIT SIGN IS LOOSE. ATTACHED EMERGENCY EXIT SIGN IAW AMM 20-20-00.
LAV SMOKE LIGHT ILLUMINATED. FLIGHT ATTENDANT VERIFIED SMOKE SMELL. RETURNED TO BASE.
WHILE AIRCRAFT WAS IN FOR INSPECTION, MECHANIC FOUND A MELTED CABIN LIGHT FITTING END FOR THE CABIN FLUORESCENT LIGHTING SYSTEM. THE FIXTURE WAS MELTED AND BURNT AND NOT WORKING AT THE TIME OF INSPECTION. THERE WERE NO PILOT REPORTS OF ANY SMOKE OR ODOR IN THE CABIN PRIOR TO INSPECTION. THIS FIXTURE IS MADE OF PLASTIC AND SHOWED SIGNS OF AN OVER HEAT CONDITION. IT IS NOT KNOWN IF THIS FITTING END WAS EVER REPLACED AFTER THE AIRCRAFT WAS MANUFACTURED. THE PART WAS REPLACED WITH A NEW UNIT.
WHILE IN CRUISE THE ELT TURNED ON, FLIGHT CREW ABLE TO RESET AND CONTINUED WITH FLIGHT. TWO DAY LATER THE SAME THING, ABLE TO RESET, WHEN FLIGHT ARRIVED BACK TO MAINTENANCE BASE ELT WAS CHANGED AND TESTED OK. TEAR DOWN REPORT INDICATED THAT G-SWITCH ACTIVATED DURING TEST AT -20 DEGREES CELCIUS. SWITCH WAS CHANGED AND FUNCTIONED PROPERLY.
ON 7-29-08 SAAB 340B N364PX - ON TAKEOFF ROLL WHEN PILOT HEARD A POP AND THE AIRCRAFT STARTED TO YAW TO THE LT, THE PILOT ABORTED THE TAKEOFF AND DETERMINED THAT THE LT ENGINE WAS CAUSING THE PROBLEM. THE LT ENGINE WAS SHUT DOWN AND AIRCRAFT RETURNED TO THE HANGER, UPON EXAMINATION WITH BORESCOPE AND VISUAL THROUGH THE EXHAUST WE FOUND ALL THE 3 1 TURBINE BLADE MISSING DAMAGE TO THE NR 2, 3, 4, AND 5 TURBINE NOZZLE AND DAMAGE TO THE TURBINE WHEELS. THE ENGINE HAS A TTSN 32562,4 AND TOTAL CYCLES 27799 AND A TTSOH OF 4945. (A)
DURING INSPECTION AND TESTING OF EMERGENCY POWER SUPPLY BATTERIES P/N 61-0478-9, FOUND ONE BATTERY WITH A SPLICE IN WIRE NOT ALLOWED IN CMM, FOUND ANOTHER THAT FAILED VOLTAGE CHECK. REMOVED BOTH BATTERIES FROM SERVICE AND SCRAPED.
ON CLIMBOUT FROM ANC LANDING GEAR WOULD NOT RETRACT. AIRCRAFT RETURNED TO ANC AND MX CLEANED WEIGHT ON WHEEL SWITCHES. THE NEXT DAY AIRCRAFT HAD A SIMILAR PROBLEM AND MX REPLACED THE LT WEIGHT ON WHEEL SWITCHES. NO FURTHER PROBLEMS HAVE BEEN NOTED SINCE.
AFTER TAKEOFF THE RT INTAKE HEAT CAUTION LIGHT ILLUMINATED. AIR RETURN TO MSP. MAINTENANCE REPLACED LOWER DUCT P/N 6124-100, S/N ON 292, S/N OFF 366.
A/C WOULD NOT GO INTO BOTTOM GOVERNING. HISTORY SHOWS LQC WAS THE LAST STATION WHERE ENGINE RIGGING WAS PERFORMED. A/C HAD NEITHER ENGINE CAPABLE OF GOING INTO REVERSE WHEN IT LANDED IN MEM ON A REPOSITION FLIGHT. MAINTENANCE PERFORMED RIGGING OF BOTH LT AND RT ENGINE CONDITION LEVERS FROM WING TO HMU CABLE IAW SF 340 AMM 76-10-00-05. PERFORMED ENGINE RUNS AS REQUIRED IAW 71-00-00-5. ADJUSTED CONDITION LEVERS AND REMOVED AND REINSTALLED DC GENERATORS ON BOTH SIDES.
DURING 4K INSPECTION IN LCQ, FUSELAGE SKIN RIGHT SIDE HAS A GROUP OF DENTS FROM BS 307.5 TO 319.0 UNDER PAX WINDOW 1.5 FT TO FLOOR LEVEL APPROXIMATELY. MAINTENANCE REMOVED AND REPLACED NR 8 ICE GUARD, FABRICATED AND INSTALLED DOUBLER REPAIR IAW SAAB SRM 51-10-10 & 53-70-10-2 AND IAW MESABA EO S53005. A/C RELEASE DATE 6/24/04.
DURING 4K INSPECTION IN LCQ, EVIDENCE OF HEAVY CORROSION UNDER EXTERNAL DOUBLER STA 248 SL1527-SL1627. DURING 4K CHECK, CORROSION WAS FOUND OF THE TCAS ANTENNA DOUBLER AND LOWER FUSELAGE SKIN. REPAIRED IN IAW MESABA EO S53232. A/C RELEASE DATE 6/24/04.
DURING 4K INSPECTION IN LCQ, FWD PAX CABIN FLOORBOARD SUPPORT UPPER SIDE HAS CORROSION AT PNL 121AZ FWD SIDE. MAINTENANCE REMOVED CORROSION FOUND OUT OF LIMITS IAW SAAB 340 SRM 51-10-10. CUT OUT CORROSION AND FABRICATED REPAIR AND INSTALLED REPAIR IAW SAAB 340 SRM 51-40-21 AND EO 53108. A/C RELEASE DATE 6/24/04.
BURING WIRE ODOR WAS PRESENT DURING DESCENT. SUPP A/C ON, BOTH BLEEDS WERE ON, NO CAUTION OR WARNING INDICATIONS PRESENT. ODOR WAS STRONGEST IN COCKPIT AND DISSIPATED AFT OF F/A JUMPSEAT. MAINTENANCE DETERMINED ODOR TO BE DUE TO RT ACM BEING CEASED. REMOVED AND REPLACED ACM P/N 776910-6, S/N OFF 880342, S/N ON 8614121, IAW JOB CARD 215181, OPS CHECK GOOD.
AT DEPARTURE CREW REPORTED SMOKE COMING FROM SIDEWALL AREA AROUND ROW 9. MAINTENANCE FOUND BAD LIGHT SOCKET. REMOVED SOCKET AND CAP AND STOWED WIRING. DEFERRED SIDEWALL LIGHT.
DURING FLIGHT 2741, DURING FLIGHT THE LEFT I/B DEICE BOOT WOULD NOT INFLATE WITH THE AUTO CYCLE SWITCH SELECTED. THE TIME LIGHT WOULD COME ON, THE TIMER LIGHT WOULD ALSO ILLUMINATE PRESSING THE MANUAL BOTTON AFTER SEVERAL ATTEMPTS. AIR RETURN TO MSP. MAINTENANCE REMOVED AND REPLACED LEFT I/B LOWER EJECTOR VALVE IAW AMM 30-10-31-4. P/N 3D2381-62, S/N OFF 161, S/N ON N1432. OPS CHECK GOOD OF I/B BOOTS CHECKS GOOD.
DURING FLIGHT 2741, THE LEFT I/B DEICE BOOT WOULD NOT INFLATE, WITH THE AUTO CYCLE SWITCH SELECTED THE TIMER LIGHT WOULD COME ON. THE TIMER LIGHT ALSO WOULD ILLUMINATED PRESSING THE MANUAL BUTTON. AFTER SEVERAL ATTEMPTS, THE LEFT I/B BOOT SECTION DID INFLATE NORMALLY. AIR RETURN TO MSP. MAINTENANCE REPLACED LEFT I/B (LOWER) EJECTOR VALVE IAW AMM 30-10-31-4. P/N OFF 3D2381-62, S/N OFF 161. P/N ON 3D2381-62, S/N ON N1432, OPS CHECK OF I/B BOOT CHECK GOOD.
DURING C-CHECK INSPECTION IN LCQ, TIMCO FOUND CORROSION AT LT STA 294, AT BUTTLINE 19 LT SIDE SEAT TRACK ON FLOORBOARD FLANGE. TIMCO MAINTENANCE REPAIRED SEAT TRACK IAW MESABA AIRLINES EO A53060 BY REPLACING SEAT TRACK AT STA 294 LBL 19. REFER TO CONTINUATION CARD 048531 FOR FURTHER DETAIL.

DURING FLIGHT NR 2990, RT MAIN GEAR GREEN LIGHT NOT ILLUMINATING DURING EXTENSION. AFTER SELECTING GEAR HANDLE DOWN RIGHT MAIN GEAR LIGHT DID NOT ILLUMINATE. COMPLETED QRH WITH GEAR HANDLE DOWN TRANSIT LIGHT WAS OUT. NO FLIGHT IDLE LIGHT, PULLED OVERRIDE. MAINTENANCE FOUND RT DOWNLOCK CONNECTOR DISCONNECTED. CLEANED AND INSPECTED CONNECTOR PINS AND FOUND TO BE SERVICEABLE. REINSTALLED CONNECTOR AND PERFORMED GEAR SWINGS IAW SF3 AMM 32-30-00-5. RESET FLIGHT IDLE STOP AS REQUIRED. ALL OPS CHECKS WERE GOOD.
DURING FLIGHT NR 3117, F1 STOOP OPEN LIGHT ILLUMINATED JUST AFTER TAKEOFF FROM MEM. AIR RETURN TO MEM. NO OTHER ABNORMALITIES. MAINTENANCE REMOVED AND REPLACED RELAY AT RDI POSITION 82KA PN M83726-28-2001P NSN IAW AMM 76-10-60-1 FIG 101 OPS CHECK DONE AMM 76-10-60 AND IAW JOB CARD #76-10-60. OPS CHECK GOOD.
AFTER TAKEOFF ON FLIGHT 2789, GEAR WOULD NOT LOCK IN UP POSITION. HYDRAULIC PUMP CONTINUED TO RUN, NO HYDRAULIC LEAK OBSERVED. REYECLED GEAR TWICE, GEAR FINALLY CAME UP. ALL SPEEDS WERE COMPLETED WITH GEAR EXTENSION AND RETRACTION. AIR RETURN TO MSP. MAINTENANCE DUPLICATED PROBLEM DURING INITIAL GEAR SWINGS. FOUND LT UPLOCK NOT BEING DEPRESSED BY HOOK ENGAGEMENT. SUSPECT MOISTURE IN THE HOOK/ROLLER ASSEMBLY. LUBED UPLOCK, ROLLERS AND ALL PIVOT POINTS. COMPLETED 15 GEAR CYCLES AND PROBLEM DID NOT RETURN IAW SAAB MM 32-30-00.
DURING FLIGHT NR 3113, LANDING GEAR FAILED TO RETRACT. AIR RETURN TO YOW. A/C FERRIED TO DTW. MAINTENANCE JACKED A/C AND PERFORMED FUNCTIONAL TEST OF EXTENSION AND RETRACTION OF LANDING GEAR IAW CHAP 32-30. COULD NOT DUPLICATE DISCREPANCY. INSPECTED GEAR FOUND NO MECHANICAL DEFECTS. CLEANED AND LUBRICATED LT AND RT MLG UPLOCK SWITCHES. ALL CHECKS GOOD IAW CHAP 32.
DURING FLIGHT 3151, CREW REPORTED LT PROP OIL PRESSURE FLUCTUATING. PROP OIL PRESSURE GAUGE GIVES ERRATIC READING FROM 0-200 PSI FOR A MINUTE OR SO, THEN CONTINUES TO GIVE ERRATIC READING. QRH COMPLETE AND MX HAS US TRIP AND RESET C/B J11. CONDITION CONTINUES. AIR RETURN TO MSP. MAINTENANCE FOUND LOOSE WIRE IN SPLICE. REPLACED SPLICE, OPS CHECK GOOD.
ON APPROACH WHEN LANDING GEAR WAS SELECTED DOWN, THE CIRCUIT BREAKER FOR THE LANDING GEAR CONTROL VALVE ''POPPED'' AND THE GEAR DID NOT EXTEND. THE CREW ATTEMPTED ONE RESET OF THE CIRCUIT BREAKER BUT IT WOULD NOT STAY SET. ABNORMAL AND EMERGENCY CHECKLISTS WRE COMPLETED AND THE GEAR WAS EXTENDED UTILIZING THE EMERGENCY EXTENSION SYSTEM. THE GEAR EXTENDED AND INDICATED DOWN AND LOCKED AND THE CREW DECLARED AN EMERGENCY AS A PRECAUTION AND A SUCCESSFUL LANDING WAS MADE. LANDING GEAR CONTROL VALVE WAS REMOVED AND REPLACED AND A GEAR SWING PERFORMED AFTER REPLACEMENT WITH NO DEFECTS NOTED. (M)

UPON REMOVING THE CONTROL QUADRANT FOR A MAINTENANCE ACTION IT WAS DISCOVERED THAT THE OPERATING CABLE FOR THE FI STOP OVERRIDE HAD SEPARATED FROM THE OVERRIDE CONTROL ACTUATOR SWITCH. RETURNED TO VENDOR. (M)
