LASTLY, GEAR INDICATION SYSTEM SHOWED DISAGREE FOR THE LEFT MAIN. PULLED LANDING GEAR EMERGENCY HANDLE. CORRECTIVE ACTION; REPLACED GSD41 PER MCC REQUEST. UPLOADED A SOFTWARE AND CONFIGURATION AS REQUIRED PER PHENOM 300 AMM 1-41-01-470-801-A. PERFORMED OPS CHECK AND THE OPS CHECK WAS SATISFACTORY. WORK WAS DONE IAW PHENOM 300 AMM 31-41-01.
CABIN EMERGENCY LIGHTS FLASHING IN FLIGHT WITH THE EMER LT SWITCH IN ARM. THE LIGHTS STOPPED FLASHING WHEN WE TURNED THE SWITCH OFF. REPLACED BATTERY. OPS CHECK GOOD. IAW AMM 33-35-01
RIGHT SEAT, RIGHT RUDDER PEDAL REQUIRES EXCESSIVE PRESSURE FOR A NORMAL CONTROL RESPONSE. MORE THAN NORMAL PRESSURE IS REQUIRED AT TAXI SPEEDS, EXCESSIVE PRESSURE IS REQUIRED AT TAKEOFF AND LANDING SPEEDS. THE PLANE PULLS TO THE LEFT ON TAKEOFF AND LANDING WITHOUT RIGHT RUDDER INPUT. CORRECTIVE ACTION: PER EMBRAER'S RECOMMENDATION, REMOVED AND REPLACED SUSPECT FAULTY SHIMMY DAMPER IAW PHENOM 300 MM 32-31-06. PERFORMED SEVERAL STRAIGHT-LINE TAXI CHECKS. NO ABNORMAL PULLING WAS NOTED AT SLOW OR HIGH TAXI SPEEDS.
EMERGENCY LIGHTS IN CABIN WERE FLICKERING WHILE TAXIING INTO FBO RAMP. AFTER SHUTDOWN, WE MOVED THE EMERGENCY-LIGHTS SWITCH TO ARM AND ALSO ON POSITIONS AND THE CABIN EMERGENCY LIGHTS FLICKERED IN BOTH POSITIONS. POWERED UP ACFT, MOVED EMERGENCY LIGHTS SWITCH TO ARM , EMERGENCY LIGHT CAME ON STEADY , DISCONNECTED NR 1 BATTERY & RECHECKED, LIGHTS ON STEADY RECONNECTED NR 1 & DISCONNECTED NR 2 & CHECKED AGAIN , SAME RESULTS , INSTALLED NEW BATTERIES IN NR 1 & 2 POSITIONS AND VERIFIED PROPER OPERATION, OPS CHECK GOOD IAW AMM 33-52-01.
ON POSTFLIGHT, DISCOVERED THE LOWER RIGHT MAIN CABIN DOOR LOCK INDICATOR GREEN FLAG IS NO LONGER VISIBLE THROUGH THE LOCK VISOR. THERE IS A SILVER BLANKET/TAPE MATERIAL INSIDE THE DOOR THAT HAS BEEN BEGINNING TO PARTIALLY OBSTRUCT VIEW OF THE LOCK FLAG. IT IS NOW 100% OBSTRUCTED FROM VIEW. GAINED ACCESS TO DOOR LOCK INDICATORS AND REMOVED DEBRIS COVERING INDICATOR.. CLOSED ACCESS. OPS CHECK GOOD. WORK DONE REF EMB-5055 MM CHAPTER 52·14-00
COMM 1 FAILED WITH VERY DISTINCTIVE ELECTRICAL ODOR IN FLIGHT. EVERY TIME YOU PUSHED TO TALK A YELLOW X WOULD APPEAR OVER COMM 1 IN BOTH GTCS ALONG WITH ELECTRICAL BURN SMELL. IT APPEARED THE SMELL ONLY HAPPENED WHEN YOU PRESSED PTT COMM 1. COMM 2 WORKED NORMALLY. WE WERE ON APPROACH WHEN FAILURE OCCURRED SO WE LANDED IMMEDIATELY. C0MPUE) WIT1T OPS CHECK AND VERIFIED YELLOW X ON COM 1. REMOVED MFD AND GIA1 ANO COULD SMELL THE BURNT SMELL. ON GIA. REMOVED AND REPLACED GIA 1 PER PHENOM 300 AMM 31-41·05. OPS CHECK GOOD AT THIS TIME. NO NORE ELECTIUCAL BURN SMELL. PRESENT AT THIS TIME.
EMERG EXIT SIGN/LIGHT: EMERGENCY "EXIT" SIGN OVER MAIN CABIN DOOR (ON CEILING). THE LEFT HALF OF THE LIGHT CLUSTER IS NOT WORKING (LOOKING FROM BOTTOM OF STAIRS, UPWARD. REMOVED AND REPLACED OVERHEAD EMERGENCY EXIT SIGN IAW EMB-505 MM CHAP 33-52-09. OPS CHECK GOOD.
DURING FLIGHT CABIN EMERGENCY LIGHTS STARTED FLICKERING AND CONTINUED TO DESTINATION. THEY GOT MUCH DIMMER BY THE TIME OF LANDING. C/W FIM 488S-33-50-00-8 I 0-802. AN INCORRECT FUNCTIONING OF THE EMERGENCY LIGHTS, FLICKER OR BLINKING. FOUND EMERGENCY BATTERIES 1 AND 2 HAD FAILED. FOUND BOTH BATTERIES FAILED TEST. REMOVED BOTH EMERGENCY BATTERIES AND INSTALLED REPAIRED UNITS IAW AMM 33-52-01-400-801-A OPS CHECKED GOOD.
EMERGENCY LIGHTS REMAIN ILLUMINATED WITH EMERGENCY LIGHT SWITCH IN THE ARM POSITION. ACCESS FWD AND AFT EMERGENCY BATTERIES FOR TROUBLESHOOTING. NOTED WITH THE FWD BATTERY CONNECTED AND THE AFT BATTERY DICONNECTED AND THE SWITCH IN THE ARM POSITION THE LIGHTS DID NOT COME ON. THE AFT BATTERY NEEDS REPLACED REMOVED AND REPLACED AFT EMERGENCY BATTERY REF EMB-300 MM 33-52-01/400 OPS CHECKED GOOD.
EMERGENCY EXIT DOORS: LOUD HUMMING NOISE HEARD IN THE CABIN DURING CRUISE FLIGHT AT FL290. THE NOISE STARTED AROUND 270K AND WAS CONSTANT AT SPEEDS ABOVE THAT. THE NOISE STOPPED WHEN THE SPEED WAS REDUCED TO 230K. UPON INSPECTING THE CABIN WE SUSPECT THE SOUND IS EMANATING FROM THE EMERGENCY EXIT. PERFORMED GVI OF EMERGENCY EXIT SEAL AND FRAME, WING ROOT SEALS, BELLY NACA DUCTS, FLAP SEALS, AND ANTENNAS WITH NO DEFECTS NOTED. PERFORMED PRESSURIZATION CHECK WITH NO DEFECTS NOTED. COULD NOT DUPLICATE. PER RICH @ MCC NO FURTHER ACTION REQUIRED. ALL WORK DONE IAW EMB-505 AMM CH 36-00-00 AND SL505505-00-0013
DURING ACCOMPLISHMENT OF EMBRAER SB 505-55-0004 REV 01 (DATED 06/24/2020), WE NOTED THE FOLLOWING LEFT ELEVATOR MASS BALANCE WEIGHT BELOW TOLERANCE: P/N PE915109-L33 (1 EACH) UNDER THE WEIGHT LIMITS (81 GRAMS), AS PRESCRIBED BY THE SB (MINIMUM 84 GRAMS). THE WEIGHT HAS BEEN REPLACED WITH A NEW MASS WEIGHT AND THE ELEVATOR BALANCING WAS PERFORMED WITH NO FURTHER DEFECTS NOTED. THE ELEVATOR WAS RE-IDENTIFIED TO P/N 505-11079-639, AS PER SERVICE BULLETIN INSTRUCTIONS. WE ALSO NOTED THE LEFT AILERON MASS BALANCE WEIGHT P/N PE915109-L31 WAS UNDER THE WEIGHT LIMITS (1739 GRAMS) AS PRESCRIBED BY THE SB (MINIMUM 1787 GRAMS), THE MASS WEIGHT HAS BEEN REPLACED WITH A NEW MASS WEIGHT AND THE AILERON BALANCING WAS PERFORMED WITH NO FURTHER DEFECTS NOTED. THE AILERON WAS RE-IDENTIFIED TO P/N 505-14736-619, AS PER SB INSTRUCTIONS. THIS REPORT COMPLIES WITH FAA 14 CFR PART 135 OPERATORâS GMM REQ.
LOUD BANG WHEN SELECTED DOWN AND 8-10 SECONDS FOR GREEN INDICATION. BLED THE AIR FROM THE LANDING GEAR UPLOCK BOXES. CHECKED & ADJUSTED HYDRAULIC ACCUMULATOR NITROGEN CHARGE REDUCING PRECHARGE PRESSURE. PERFORMED FUNCTIONAL CHECK ON THE LANDING GEAR EXTENSION & RETRACTION - OPS CHK GOOD. SERVICED HYDRAULIC RESERVOIR. NO FURTHER LOUD BANGING NOISES. WORK DONE IAW PHENOM 300 M/M 32-31-05-870-801, 32-31-00-720-801, 12-13-00-610-801, 29-10-00-860-801.
DURING CRUISE FLIGHT, A STRONG ODOR WAS PRESENT PRIMARILY IN THE COCKPIT. THE ODOR IS MOST NOTICEABLE WHEN THE AIR SYSTEM IS CALLING FOR AN INCREASE IN TEMPERATURE. COMPLIED WITH FIM 21-20-00, WITH NO DEFECTS NOTED AT THIS TIME. OK TO RELEASE AT THIS TIME.
BOTH CO-PILOTS SIDE BRAKES ARE SOFT. INSPECTED COPILOTS PANELS ATTACHING POINTS, MASTER CYLINDERS, LINES AND FITTINGS NO DEFECTS NOTED AT THIS TIME BLED BRAKES IAW MM CH 32-42-00 OPS CHECKS SATISFACTORY.
DURING CLIMBOUT AT APPROX 16,000' LT OIL TEMP GAUGE DROPPED TO ZERO ANDS THEN RETURNED TO NORMAL NUMEROUS TIMES. AFTER LEVELING AT 23,000' THE GAUGE DROPPED TO ZERO AND REMAINED THERE UNTIL DESCENDING THROUGH ABOUT 17,000'. GAUGE WAS NOMAL AFTER THAT.
ON TAKEOFF ROLL, PWR APPROX 80-90 PERCENT, LT THRUST REVERSER ARM AND UNLOCK LIGHT CAME ON. AS WE TAXIED IN, FULL T/R DEPLOYMENT ON LT SIDE; EMER SWITCH STOWED T/R. REENABLED T/R SYS. CYCLED T/R'S AND DUPLICATED MALFUNCTION. GAINED ACCESS TO THROTTLE GUARD. TR SWITCHES AND FOUND SWITCH CAM LOOSE AND NOT ADVANCING WHEN THROTTLES WERE ADVANCED. TIGHTENED CAM HARDWARE AND CK'D SWITCH RIGGING TR SYS OPS CK'D GOOD IAW MM CHP 78-31-00.
AT FL340 LOST PSI'T AT 2000 FPM TO MAX CAB ALT OF 12000. EMERGENCY ACTION TAKEN. TROUBLESHOT AND FOUND BYPASS DUCT SLEEVE BLOWNOFF OF CONDITIONED AIR DUCT IN TAILCONE. REINSTALLED BYPASS DUCT SLEEVE IAW MM 21-51-00. CHECK NORMAL.
DURING TAKEOFF, AIRCRAFT REQUIRED SIGNIFICANT FOWARD PRESSURE DURING CLIMBOUT. ALL OTHER INDICATIONS WERE NORMAL EXCEPT NOSE TRIM WHEEL INDICATOR WAS BEYOND THE FORWARD WHITE ARC. FOUND TRIM INDICATOR OUT OF ADJUSTMENT. RE-POSITIONED INDICATOR TO SHOW CORRECT POSITION OF ELEVATOR TABS. OPERATIONAL CHECK AND RIGGING CHECK SATISFACTORY. ALL WORK IAW MM 27-31-00.
ON FINAL APPROACH WITH LANDING GEAR DOWN AND LOCKED LOW PRESSURE POWER BRAKE AND ANTI-SKID LIGHTS ILLUMINATED. NORMAL LANDING WITH NO PRESSURE ON BRAKE PEDALS. UTILIZED EMERGENCY BRAKE LEVER TO STOP AIRCRAFT ON RUNWAY. REPLACED SWITCH S112B ON LANDING GEAR SELECTOR ASSY. BRAKE AND ANTI-SKID SYSTEM OPS CK GOOD. SYSTEM NORMAL WHEN GEAR HANDLE IS PLACED DOWN. ALL WORK DONE IAW CE560 MAINT MANUAL 32-31-01.
ON TAXI OUT BRAKES FELT SOFT. BLED BRAKES PER CESSNA 560 ULTRA MM 32-42-00. BRAKES FUNCTION NORMALLY AT THIS TIME.
CO-PILOTS LEFT PEDAL GOES TO THE FLOOR BEFORE ANY PRESSURE CAN BE BUILT UP. ITEM IS UNDER INVESTIGATION.
POWER BRAKE SYSTEM FAILURE ON TAXI. POWER BRAKE LOW PRESS AND ANTISKID INOP LIGHTS ON. NO BRAKE PRESSURE. TROUBLESHOT SYSTEM AND FOUND RT MAIN GEAR BRAKE LINE (FLEXHOSE) P/N AE2463509G0150, LEAKING WHEN GEAR IS OFF GROUND. REMOVED LINE AND INSTALLED A NEW ONE P/N AE2463509G0150, NO S/N. BLED BRAKES AND SERVICED RESERVOIR WITH NO FURTHER LEAKS NOTED. WORK DONE WITH REFERENCE CE560 ULTRA MM 32-42-00 AND 20-10-06.
DESCENDING THROUGH 18,000 FT, RECEIVED A PITCH MIS TRIM ANNUNCIATOR. ATTEMPTED TO HELP THE AUTOPILOT BY TRYING TO TRIM THE NOSE UP. DISCOVERED THAT THE PITCH WHEEL WAS FROZEN IN A VERY NOSE DOWN POSITION. AT 13,000 FT USED EXTREME FORCE TO MOVE THE PITCH WHEEL ENOUGH TO LAND. ITEM IS UNDER INVESTIGATION.
UPON GEAR RETRACTION THE GEAR UNLOCKED LIGHT WOULD NOT EXTINGUISH, GEAR EXTENDED NORMALLY ON RECYCLE WITH 3 GREEN LIGHTS, ACCOMPLISHED AIR TURN BACK TO TEB. PERFORMED GEAR SWING AND FOUND GEAR UNSAFE LIGHT EXTINGUISHED WITH EVERY RETRACTION AND EXTENSION OF LANDING GEAR. SYSTEM OPERATES NORMAL AT THIS TIME, ALL WORK PERFORMED IAW CE560 MM. CHAPTER 7-10-00 & 32-01-00.
UNSAFE INDICATION ON LEFT MAIN GEAR, BLEW GEAR DOWN. FOUND BOTH LIGHT BULBS ON MLG PANLE BURNED OUT, REPLACED BULBS.
